The Optum HSA Withdrawal Correction Form lets you return money to your Health Savings Account after an accidental non-medical withdrawal, so the distribution isn’t treated as taxable income. It’s a single-page PDF you download from Optum, fill in with your account details and the amount you’re putting back, and mail with a check payable to Optum Bank to P.O. Box 271629, Salt Lake City, UT 84127.1Optum Bank. Withdrawal Correction Form Done correctly and on time, it erases both the income tax and the 20 percent penalty that would otherwise apply to the withdrawal.2Internal Revenue Service. When You Can Use the Form
The IRS allows corrections only for distributions that resulted from a “mistake of fact due to reasonable cause.”3Internal Revenue Service. Instructions for Forms 1099-SA and 5498-SA In practice, that covers two common situations: your insurer reprocesses a claim after you’ve already paid the bill from your HSA, or an expense you reasonably believed was qualified turns out not to be.
What it does not cover is spending you knew wasn’t medical at the time. Using the HSA debit card at a restaurant, then wanting the money back, isn’t a mistake of fact. The IRS expects “clear and convincing evidence” behind the claim, so keep the corrected invoice, explanation of benefits, or refund notice that documents the change.
What the Form Cannot Fix
The form itself states it “cannot be used to correct an Excess Contribution Return.”4Optum Financial. HSA Withdrawal Correction Form If you contributed more than the annual limit, you need Optum’s separate Excess Contribution Removal Form.5Optum Financial. Health Savings Account (HSA) Excess Contribution Removal Form
It also won’t help with a deliberate non-medical withdrawal you later regret. Two alternatives may still work in that case:
- A 60-day rollover. If fewer than 60 days have passed since the distribution, you can redeposit the funds as a rollover contribution, limited to one such rollover per 12-month period.
- Offset with saved receipts. Qualified medical expenses you paid out of pocket after opening the HSA, and never reimbursed yourself for, can offset a non-qualified distribution. There’s no time limit on that reimbursement.
Filling Out the Three Sections
Download the fillable PDF from Optum’s forms library.4Optum Financial. HSA Withdrawal Correction Form The form has three short parts.
Account Holder Information
Enter your full name, Social Security number, mailing address, phone number, and Group ID. The Group ID comes from your employer’s benefits plan and usually appears on your Optum debit card or welcome materials. If you can’t find it, ask your HR or benefits department.
Redeposit Correction Information
This section wants your HSA account number, the year the original distribution happened, and the dollar amount you’re redepositing. The year matters because Optum posts the correction to that tax year, not the year you’re mailing the form.4Optum Financial. HSA Withdrawal Correction Form The redeposit amount doesn’t have to match the full withdrawal. If only part of it was mistaken, enter just that portion.
Account Holder Authorization
Read the authorization statement, then sign and date. Your signature confirms you’ve enclosed a check and understand the correction may produce a corrected 1099-SA for that tax year.1Optum Bank. Withdrawal Correction Form There’s no box to select a reason; the form treats every submission as a correction of a distribution made in error.
One warning printed on the form: do not put any debit card numbers on the document. Optum won’t process forms containing card information and will destroy them for security purposes.1Optum Bank. Withdrawal Correction Form
Submitting the Form and the Repayment Deadline
Write a personal check or money order for the redeposit amount, payable to Optum Bank. Write your HSA account number on the check so the funds credit to your account directly. Mail the check and completed form together to:

The IRS deadline for repaying a mistaken distribution is the due date of your federal tax return, typically April 15, for the first year you knew or should have known the distribution was a mistake. A filing extension does not extend this deadline.6Internal Revenue Service. Form 1099-SA – Distributions From an HSA, Archer MSA, or Medicare Advantage MSA If you discovered in December 2025 that a withdrawal earlier that year was mistaken, your repayment must reach Optum by April 15, 2026. Build in at least a week for mail transit.
Optum does not publish a specific processing time. Check your online transaction history about two to three weeks after mailing to confirm the redeposit posted.
How the Correction Shows Up on Your Tax Forms
Once Optum processes the correction, it may issue a corrected Form 1099-SA for the year in question. The original 1099-SA would have shown the full distribution in Box 1; the corrected version reduces or removes that amount so it no longer appears as a taxable distribution. Optum also reflects the redeposit on Form 5498-SA.3Internal Revenue Service. Instructions for Forms 1099-SA and 5498-SA
Because the funds post as a correction rather than a new contribution, the redeposit does not count against your annual contribution limit.4Optum Financial. HSA Withdrawal Correction Form For 2026, those limits are $4,400 for self-only coverage and $8,750 for family coverage.7Optum Bank. HSA Contribution Limits
If you already filed your return for the year the mistaken distribution occurred, you may need to amend it with Form 1040-X after the corrected 1099-SA arrives. Form 8889, which reports HSA activity, would also need to reflect the corrected figures.8Internal Revenue Service. Instructions for Form 8889
