To fill out the Optum Financial Dependent Care reimbursement form, work through its four sections in order: your participant information, the details of the care (dependent, dates of service, and amount charged), the provider certification (either an itemized statement attached or your provider’s signature and Tax ID on the form itself), and your own signature certifying the expense. Then submit it through the Optum portal, mobile app, fax, or mail. Most claims process within two to four business days once Optum receives them.1Optum Financial. File a Claim for FSA
Get the Right Version of the Form
Optum publishes two dependent care forms: a standard claim form for a one-time reimbursement and a recurring reimbursement form that pays you a fixed amount each month during the plan year.2Optum. Recurring Dependent Care Reimbursement Request Form The standard form is also available as a direct PDF from Optum’s site.3Optum Financial. How to Submit a Claim for Dependent Care Accounts Sign in at optumfinancial.com and open the forms or resources section, or check your employer’s HR benefits portal. If neither works, call the number on the back of your Optum benefits card.
Use the official Optum version rather than a generic FSA form. The layout is built for Optum’s processing system, and another administrator’s form will delay or derail your claim.
Section 1: Participant Information
Enter your full legal name, the last four digits of your Social Security number, your employer’s name, and your mailing address. The employer name has to match how your company is registered in Optum’s system. If your company operates under a trade name that differs from its legal name, use the legal one. A mismatch here can route your claim to the wrong plan and cause a rejection.2Optum. Recurring Dependent Care Reimbursement Request Form
Section 2: Service and Expense Details
List each dependent who received care and their age, along with the dates of service, a description of the care, and the dollar amount charged. Two details cause most of the trouble here.
First, Optum wants the amount your provider charged, not the amount you paid.4Optum Bank. Manage Your Dependent Care Flexible Spending Account Claims If your provider billed $1,200 for the month and you paid $1,000 with $200 still due, report $1,200.
Second, use the dates the care was actually provided, not the date you made payment. Those are often different, and using the payment date is one of the most common reasons claims stall.
If you’re using the recurring form instead, you specify a fixed monthly dollar amount and the months you want it applied to. Your monthly reimbursement cannot exceed your monthly payroll contribution to the account, and the request expires at the end of the plan year, so you’ll need to submit a new one each plan year even if nothing about your care arrangement has changed.2Optum. Section 3: Provider Certification
You have two ways to satisfy this section: attach an itemized statement from your care provider, or have the provider sign the certification on the form.
An itemized statement has to include the provider’s name and address, dates of service, the dependent’s name, the type of service, and the amount billed.5FSAFEDS. Eligible Dependent Care FSA Expenses Credit card receipts, canceled checks, and balance-forward statements do not count.
If you don’t have that statement, the provider fills out and signs the certification section directly, including their Tax Identification Number. For a private caregiver like a neighborhood babysitter, that’s their Social Security number. Tax-exempt organizations, such as a church-run day care, write “tax-exempt” in the TIN field instead of a number.6Internal Revenue Service. About Form W-10 – Dependent Care Provider’s Identification and Certification Collect the TIN before you need to file. The IRS provides Form W-10 specifically for gathering this information from a care provider.
Section 4: Your Signature
Sign and date the form to certify that the expenses were incurred while you were covered by the plan and have not been reimbursed from any other source. Optum will not process an unsigned form.2Optum. Recurring Dependent Care Reimbursement Request Form
A Note on Whether the Expense Qualifies
Before you file, confirm the care itself is eligible. Dependent Care FSA reimbursements are limited to work-related care that enables you (and your spouse, if married) to work or actively look for work.7Internal Revenue Service. Publication 503 – Child and Dependent Care Expenses For a child, the child must be under age 13 when the care is provided; the day they turn 13, later expenses stop qualifying. Overnight camp, kindergarten tuition and above, tutoring, and care by your own tax dependent do not qualify. If any of that describes what you paid for, the form won’t fix it.

The portal and app are the fastest options. If you fax or mail a paper form, check that copies are legible. Optum may request additional documentation if the scan quality is poor.
What Happens After You Submit
Optum generally processes claims within two to four business days of receipt. Once approved, direct deposit lands in two to four business days if you’ve linked a personal bank account to your Optum profile. Without direct deposit set up, Optum mails a paper check, which takes seven to ten business days.1Optum Financial. File a Claim for FSA
One quirk catches people off guard: unlike a health FSA, your full annual election is not available on day one. Reimbursement is limited to what your employer has actually deducted from your paychecks so far. If you’ve contributed $2,000 through payroll by March and file a $3,000 claim, Optum pays $2,000 now and releases the remaining $1,000 as future payroll deductions come in.
Why Claims Get Denied
Most rejections trace back to a short list of fixable problems:
- Missing or unsigned provider certification, with no itemized statement attached.
- Wrong documentation type. Canceled checks, credit card statements, and balance-forward invoices don’t qualify. You need the provider’s signature on the form or an itemized statement showing dates, amounts, and services.5FSAFEDS. Eligible Dependent Care FSA Expenses
- Date-of-service errors. Submitting the date you paid instead of the date the care was provided.4Optum Bank. Manage Your Dependent Care Flexible Spending Account Claims
- Ineligible expense. Overnight camp, kindergarten and above, and care that isn’t work-related are the most common culprits.
- Unsigned claim form. Optum will not process forms missing your signature in Section 4.2Optum. Recurring Dependent Care Reimbursement Request Form
- Exceeding your available balance. Reimbursement is capped at what’s been contributed through payroll to date.
If a claim is denied, you can appeal. Under federal regulations governing employer-sponsored benefit plans, you generally have 180 days from the date you receive the denial notice to submit a written appeal.9eCFR. 29 CFR 2560.503-1 – Claims Procedure The denial notice should explain the reason and how to appeal. In most cases, resubmitting with corrected documentation resolves the issue faster than a formal appeal.
Deadlines That Control Your Filing Window
Dependent Care FSA funds follow a strict use-it-or-lose-it rule. There is no carryover for these accounts. Any money unspent after the deadline is forfeited.10FSAFEDS. What Is the Use or Lose Rule?
The care itself must occur during the plan year, usually January 1 through December 31. Your employer may offer a grace period of up to two and a half months after the plan year ends, through March 15 for a calendar-year plan, during which you can still incur and claim eligible expenses using the prior year’s funds.11Internal Revenue Service. IRS Notice 2021-26 – Taxation of Dependent Care Benefits Not every employer offers this, so check with your benefits administrator.
Separately, most plans provide a run-out period after the plan year (or grace period) ends. This does not extend the window for incurring expenses. It only gives you extra time to file claims for care that already happened. A typical run-out period is 90 days, though your employer’s plan documents control the exact deadline. After the run-out period closes, unfiled claims are gone.
