This section establishes why you qualify. If you’re eligible in your own right, check the box that matches your status (Military, Civilian, DoD Contractor, Veteran, or another listed category) and select your branch affiliation.

If you’re joining through a family member or household member, check the sponsor box and provide their affiliation, your relationship, their access number or the last four digits of their Social Security number, their name, and their date of birth.3Navy Federal Credit Union. Sections H and I: Joint Owner

If you want a second person on the account, these sections collect their personal and employment information, mirroring Sections A and E. The joint owner needs their own Social Security number, government ID, and address details.

Section K: Signatures and Tax Certification

Sign and date the form. The tax certification asks you to confirm your Social Security number or ITIN, state whether you’re subject to backup withholding, and certify your U.S. citizen or resident alien status.
