To fill out the Navy BCA score sheet, download the current NAVPERS 6110/10 (dated December 2025) as a fillable PDF from MyNavy HR, enter the Sailor’s identifying information and the measurements taken by two certified Command Fitness Leaders or Assistant CFLs following the Navy’s rounding rules, mark the pass/fail determination from the two-step Body Composition Assessment, print the sheet for all three signatures, and hand it to the CFL for entry into PRIMS.1MyNavy HR. Understand the Two-Step BCA Before You Type
Which fields on the sheet actually need numbers depends on where the Sailor’s assessment ends. The current BCA has two steps, and Step 1 can end it.
- Step 1 is the waist-to-height ratio. Divide waist by height. If the result rounds down to 0.5499 or less, the Sailor is Within Standards and Step 2 is not required. Weight is recorded either way.3MyNavy HR. Guide-4 Body Composition Assessment
- Step 2 applies when the WHtR is 0.5500 or greater. The CFL/ACFL team takes additional circumference measurements and reads body fat percentage from the Navy’s official tables. The ceiling is 26 percent for males and 36 percent for females. Anyone above that is Not Within Standards.3MyNavy HR. Guide-4 Body Composition Assessment
Substitute methods are not allowed. Underwater weighing, skinfold calipers, BMI, and bio-impedance devices are all prohibited, and external calculators or spreadsheets cannot be used to derive the official body fat percentage.3MyNavy HR. Guide-4 Body Composition Assessment
How the Measurements Reach the Sheet
Two certified CFL/ACFLs, designated in writing, must conduct every official BCA together. Private, one-on-one measurements are prohibited, and Sailors are not entitled to same-sex measurers.3MyNavy HR. Guide-4 Body Composition Assessment
Height and Weight
Shoes off, socks on. No inserts of any kind; gel inserts and similar height enhancements can draw UCMJ action. The Sailor stands at attention on a flat surface, head level, chin parallel to the deck, eyes forward. Height is rounded up to the nearest half inch. Weight is rounded to the nearest whole pound (down if under half a pound, up at half a pound or more).3MyNavy HR. Waist and Other Circumferences
Take each circumference measurement twice and round each reading down to the nearest half inch. If two readings differ by more than one inch, take a third and average the two closest rounded readings. That averaged value is recorded as-is and is not rounded again.3MyNavy HR. Guide-4 Body Composition Assessment The Sailor is in the physical training uniform, and abdominal measurements are taken on bare skin.
If a CFL/ACFL sees signs the Sailor tried to alter measurements temporarily, such as skin redness or pressure lines, the Sailor is turned away, must wait at least 72 hours before another official BCA attempt, and the incident is reported to the command triad.3MyNavy HR. Body Fat Percentage
When Step 2 is triggered, use the official Navy estimation tables (Table 2 for males, Table 3 for females) found in Section 4 of Guide 4, or record the PRIMS-generated value.3MyNavy HR. Guide-4 Body Composition Assessment Nothing else is authorized as the source of that number.
Fields on the Score Sheet
With the measurements taken, complete the fillable PDF. The sheet is short, but each field has a job.
- Identification: full legal name, DoD ID number, gender, age, rank, and unit or command. Gender and age drive which body fat limit applies and how the record maps in PRIMS.
- PFA cycle designation, so the record ties to the correct testing period.
- Height and weight, using the rounding rules above.
- Waist-to-height ratio, calculated to the fourth decimal place and rounded down (for example, 0.5499).
- Circumference data for Step 2 only: individual site readings, their averages where a third reading was needed, and the body fat percentage from the official table or PRIMS.
- The pass/fail determination: Within Standards or Not Within Standards, based on either the WHtR result or the body fat calculation.
Entries need to be legible and match the numbers observed during the session. Once everything is typed, print the sheet for signatures.
Signatures Make It Official
The printed sheet requires signatures from the Sailor and from both CFL/ACFLs who conducted the assessment.3MyNavy HR. This signed paper document is the source record behind the PRIMS entry.
One rule that raises the stakes on getting the numbers right the first time: the official BCA is the first and only BCA authorized once the command’s PFA cycle has begun. Retests are not authorized.3MyNavy HR. Guide-4 Body Composition Assessment
Enter the Data in PRIMS
After signatures, the CFL manually enters the score sheet data into the Physical Readiness Information Management System (PRIMS), the authoritative database for active and reserve component readiness records. All data for a PFA cycle, including waivers and non-participation justifications, must be entered no later than 30 days after the command’s official PFA cycle ends, unless the Chief of Naval Personnel directs otherwise.4MyNavy HR. CFLs and ACFLs cannot enter their own PFA data. The ACFL enters the CFL’s data, and the CFL enters the ACFL’s data.4MyNavy HR. Guide-7 PRIMS Administration After the data syncs to the Sailor’s permanent electronic record, the command retains the original signed paper copy per Navy records management policy.
When a Waiver Changes What Goes on the Sheet
A medical condition that prevents a Sailor from completing the BCA is handled on a separate form, NAVMED 6110/4, recommended by a healthcare provider, routed through the CFL, and decided by the Commanding Officer. A new waiver is required each PFA cycle. Two limits are worth knowing before you fill anything out. A Sailor cleared for the full PRT, including any alternate cardio, does not need a NAVMED 6110/4; the provider documents clearance on the Physical Activity Risk Factor Questionnaire.5MyNavy HR. Mistakes That Invalidate the Sheet
Most bad BCA records come from technique or paperwork errors. Check the sheet against these before anyone signs.
- Wrong rounding direction. Height rounds up to the nearest half inch. Circumference measurements round down. Reversing them changes the WHtR and can flip the result.
- Only one CFL/ACFL present. Two certified, designated measurers are required, and both sign the sheet. A one-on-one assessment violates policy.3MyNavy HR. Guide-4 Body Composition Assessment
- Shoes or inserts during height measurement. Even thin soles or gel inserts alter the WHtR. Socks only.3MyNavy HR. Guide-4 Body Composition Assessment
- Unauthorized calculation tools. Body fat percentage must come from the official Navy tables or the PRIMS-generated value, not apps, spreadsheets, or web calculators.3MyNavy HR. Guide-4 Body Composition Assessment
- Missing the PRIMS 30-day deadline after the command’s PFA cycle closes.4MyNavy HR. Guide-7 PRIMS Administration

Because retests aren’t authorized once the cycle begins, the sheet you sign is the sheet that stands. Verify the rounding, redo the WHtR math, and confirm the table lookup before signatures go on the page.
