- Visit a branch. Any Fifth Third location can set it up in person, and you can book an appointment through the website first.
One thing worth checking before you print anything: if your employer runs payroll through a self-service portal like ADP or Workday, you probably won’t need Fifth Third’s form at all. Those systems ask you to type your routing and account numbers straight into your profile. The paper form is for employers that still collect deposit authorizations manually.
How to Fill Out the Form
The form has three parts: your personal information, your bank account details, and your signature. Every field matters. One wrong digit in the routing or account number and your paycheck lands somewhere it shouldn’t or bounces back to your employer.
Employee Profile
The top section asks for your name, home address, phone numbers, date of birth, Social Security number, and email. Use the exact name that appears on your payroll record. Bank Account Information
Here you fill in your bank name, the nine-digit routing number, and your account number. Check the box for checking or savings. Picking the wrong account type will cause the transaction to fail. Signature and Date
Sign and date the bottom. Others insist on ink. Ask HR if you aren’t sure.
Finding the Right Routing Number
Fifth Third uses different routing numbers depending on where you originally opened your account, and it’s the field readers get wrong most often. Your routing number is tied to the state or region where you opened the account, not where you live now, so if you’ve moved since opening it, don’t guess based on your current address.6Fifth Third Bank. The safest way to get the right number is to log in to the mobile app or online banking and pull the routing number Fifth Third has attached to your specific account. That number will always match. Getting the Form to Your Employer
The completed form goes to your employer’s payroll or HR department. Not to Fifth Third. How you deliver it depends on the workplace: some want a physical copy, some want a scanned PDF uploaded through their payroll portal, and some use a self-service system where you enter your routing and account numbers directly and attach the signed form as backup.
A lot of employers also ask for a voided check stapled to the form as a second layer of verification. If you don’t have physical checks, a screenshot of your account and routing numbers from the app usually works, or you can request a printed direct deposit authorization letter from the bank. Ask payroll what they’ll accept before you print anything.
Splitting Pay Across Accounts
If you want part of your paycheck going to savings and the rest to checking, most payroll systems let you split deposits by a fixed dollar amount or a percentage. A common setup routes a set amount, say $200, to savings and sends the rest to your primary checking account. When you’re splitting across three or more accounts, pick one method and stick with it, either all fixed amounts or all percentages, to avoid calculation conflicts. One account has to be designated as the remaining-balance account so there’s somewhere for the leftover to land.
When Your First Deposit Will Arrive
Your first electronic paycheck usually shows up within one to two pay periods after your employer submits the form, depending on when their payroll cycle picks it up.1Fifth Third Bank. Part of the wait is a prenote: a zero-dollar test transaction your employer’s payroll system sends through the ACH network to confirm the routing number, account number, and account type all check out. If the prenote comes back with an error, payroll will ask you to resubmit a corrected form, and the clock starts over.
Once direct deposit is running on an eligible account (Momentum Checking, Momentum Savings, Express Banking, or Preferred Checking), Fifth Third’s Early Pay feature can make your paycheck available up to two days before the scheduled payment date. There’s no enrollment step and no fee. Exactly how early depends on when your employer transmits payroll data. Qualifying deposits include paychecks, tax refunds, government benefits like Social Security, some retirement payments, and child support. Changing or Canceling Direct Deposit Later
To move your pay to a different account, submit a new Fifth Third form to your employer with the updated bank details and check the Change Bank Information box.3Fifth Third Bank. Direct Deposit Employee Authorization Form Keep the old account open until at least one paycheck has landed in the new one, because the switch takes another one to two pay cycles and you don’t want a deposit with nowhere to go.
To stop direct deposit entirely, check the Cancel Direct Deposit box and give the form to your employer. You can also notify payroll verbally or in writing without using the form. If Payday Comes and Nothing Shows Up
Start with your employer, not the bank. Payroll problems account for most missing deposits: a delayed submission, a wrong pay date, a form that never got processed. Confirm with HR that the deposit was actually sent and ask for the ACH trace number, which identifies the transaction inside the banking network.
