To fix an error on your Equifax credit report in Canada, you fill out the Equifax Canada credit report dispute form — either online through the Equifax Canada dispute page or on a printable PDF you mail to National Consumer Relations in Montreal — with your identification, the specific items you are challenging, and documents that prove the information is wrong. Online disputes take about 15 to 20 days; mailed disputes take about 20 to 25 days.1Equifax. Dispute Credit Report Information
What to Gather Before You Start
The form asks for your full legal name, date of birth, phone number, email address, and Social Insurance Number, plus your current address and any former addresses.2Equifax. Start Your Dispute Have your Equifax credit report open beside you so you can copy account numbers and creditor names exactly as they appear.
Equifax also requires two pieces of valid identification: one to verify your identity and one to verify your address.1Equifax. Dispute Credit Report Information A driver’s licence or passport covers identity. A recent utility bill, bank statement, or mortgage statement showing your current address covers the address side. Send copies, never originals.
Then pull together evidence that shows the entry on your report is wrong. Useful documents include:
- Bank or account statements showing a payment was made on time when the report says it was late, or showing a different balance than what is reported.
- Letters from creditors confirming an account is paid, settled, or closed.
- A police report or documentation from the Canadian Anti-Fraud Centre if the disputed account was opened fraudulently.
A dispute with no supporting documents forces Equifax to rely entirely on what the creditor says, which usually means nothing changes.
Filling Out the Form
The form is available through the dispute section of the Equifax Canada website.3Equifax. Contact Us You can complete it online or download a printable PDF. The online version guides you through category selection; the paper version leaves you to write in each detail.
Personal Information
Enter your full legal name exactly as it appears on your credit report. Even a small mismatch, like dropping a middle initial, can slow the process. Fill in your SIN, date of birth, current address, and any former addresses. If a previous credit report showed an Equifax Unique Number, include it — it helps Equifax pull your file faster.
Choose the Category of Dispute
Equifax organizes disputes by type: personal information (name, address, SIN, phone number), account information (balances, payment history, account status), and other items such as public records or collection accounts.2Equifax. Start Your Dispute Pick the category that matches your issue. If more than one item is wrong, you can list them all, but each needs its own explanation.
Describe Each Error Specifically
For every disputed item, write the creditor’s name, the account number or at least the last four digits, and a specific description of what is wrong. “This is incorrect” is not enough. Something like this works better: “This account shows a missed payment in March 2025, but the attached bank statement confirms the payment cleared on March 12, 2025.” Point to the exhibit that backs up each claim so the investigator can match your words to your evidence.
If you are completing the paper form by hand, print clearly. Illegible handwriting is a common reason disputes get returned. Typing into the PDF before printing avoids the problem entirely.

What Equifax Does Next
Once your dispute arrives, Equifax contacts the creditor or lender that reported the disputed information and asks them to verify it against their own records.4Financial Consumer Agency of Canada. Checking for Errors on Your Credit Report Two outcomes are possible:
- The creditor agrees there is an error. Equifax updates your report and sends you a confirmation of the result.
- The creditor says the information is accurate. The report stays as it is, and Equifax notifies you of the finding.
Credit bureaus must correct verified errors at no cost to you. It also helps to contact the creditor directly while the Equifax investigation runs, asking them to verify their files and send corrected data to the bureau. Disputing with the bureau and pushing the creditor at the same time tends to resolve things faster than either route on its own.4Financial Consumer Agency of Canada. Add a Consumer Statement
If the disputed information stays on your report, you can add a consumer statement so lenders see your side. Equifax Canada allows statements up to 800 characters, roughly two to three sentences. To add one, mail your written statement to the Equifax address above along with your name, address, date of birth, phone number, and copies of two pieces of valid identification.5Equifax CA. You can also file a formal privacy complaint with the Office of the Privacy Commissioner of Canada under PIPEDA. You must first try to resolve the issue directly with Equifax and keep copies of all correspondence. The OPC investigates and attempts resolution through mediation, but it cannot issue fines, order compensation, or force an organization to release information.6Office of the Privacy Commissioner of Canada.
