How to Fill Out the Disney FMLA Form: Sedgwick Certification Steps

If you’re a Disney Cast Member preparing to request job-protected leave, here’s how to fill out the Disney FMLA form and get it back to the company’s leave administrator without the delays that trip most people up. Disney routes all Family and Medical Leave Act requests through Sedgwick, a third-party administrator. You open a claim with Sedgwick, your health care provider completes a federal medical certification form, and you return the signed form through the Sedgwick portal or by mail within the deadline Sedgwick sets.

Pick the Right Certification Form

There isn’t one Disney-branded FMLA form. Disney uses the standard U.S. Department of Labor certification forms, and the one you need depends on why you’re taking leave.1U.S. Department of Labor. FMLA Forms

  • Form WH-380-E if the leave is for your own serious health condition.
  • Form WH-380-F if you’re caring for a spouse, child, or parent with a serious health condition.

Both are available on the Department of Labor’s website and typically through the Disney Hub. Sedgwick will also send you the correct form in your claim packet once you open a case, so you don’t have to guess. If your leave is for the birth or placement of a child, or for a military family situation, Sedgwick will direct you to the appropriate certification during intake.

Gather What You Need Before You Start

Filling out the paperwork goes faster when you have the basics in front of you:

  • Your Cast Member ID, so Sedgwick can match the claim to your employment record.
  • Your work location and the name of your leader.
  • The reason for leave and, if you know it, the expected start date and duration.
  • Your health care provider’s name, address, phone, and fax number — the provider fills in the medical portion, and Sedgwick may need to contact them.
  • Whether you expect the leave to be continuous (one block of time) or intermittent (episodes over weeks or months).

Open the Claim With Sedgwick

Start the process through the Disney Hub or by calling Sedgwick directly. You’ll answer a series of questions about the reason for leave, expected timing, and whether the need is foreseeable or sudden. Sedgwick then opens a claim, assigns you a claim number, and issues a packet that includes the correct certification form and a deadline for returning it.

Once the claim is open, you can upload the completed certification to the Sedgwick portal or mail it to the address in your packet. Keep a copy of everything you send.

Complete the Employee Section

You fill out the top portion yourself: your name, contact information, employer (Disney), and a brief description of the leave you’re requesting. This section is straightforward. Match the information exactly to what Sedgwick has on file — mismatched names or addresses can slow processing.

Have Your Health Care Provider Complete the Medical Section

The bulk of the form is completed by your health care provider. This is the part that most often comes back incomplete, so it helps to know what the provider needs to address:

  • Confirmation that your condition (or your family member’s condition) meets the legal definition of a serious health condition under FMLA. That generally means inpatient care or continuing treatment by a health care provider.2eCFR. 29 CFR 825.113 – Serious Health Condition
  • The approximate date the condition began and its probable duration.
  • Whether the condition will require continuous absence, intermittent absence, or a reduced work schedule.
  • For intermittent leave, the estimated frequency and duration of episodes — for example, “two to three flare-ups per month, one to two days each.” Sedgwick tracks every absence against your entitlement, so vague answers here cause problems later.
  • Whether you’re unable to perform essential job functions (for your own condition) or whether your family member needs care you can reasonably provide (for family leave).

The provider does not have to disclose your specific diagnosis. They only need to give enough information to establish that the condition meets the legal threshold. If your provider is uncomfortable with a question, they can note that the information is not medically necessary to disclose, but they should still complete every question that goes to whether the condition qualifies.

Check the Form Before You Send It

Incomplete forms are the single most common reason FMLA claims get held up. Before you hand the certification back to Sedgwick, run through this list:

  • Every section your provider was supposed to complete is filled in — no blanks, no “see attached” without the attachment.
  • The provider has signed and dated the form.
  • The frequency and duration estimates for intermittent leave are specific, not left as “unknown.”
  • Your name and identifying information at the top match your Disney employment record.
  • You’ve kept a copy for yourself.

Return the Form on Time

Sedgwick will give you a deadline for returning the completed certification, generally at least 15 calendar days from the date you’re asked for it. Missing that deadline without a good reason can lead to denial of the leave.

Notice timing matters separately from the certification deadline. What Happens After Sedgwick Receives Your Form

Within five business days of receiving your leave request, Disney (through Sedgwick) must send you a Notice of Eligibility and Rights & Responsibilities. This tells you whether you meet the basic eligibility criteria and what documentation you still owe.5U.S. Department of Labor. After the medical certification is reviewed, you’ll get a Designation Notice within another five business days confirming whether the leave is approved and how much time counts against your 12-week entitlement (or 26 weeks for military caregiver leave).6eCFR. 29 CFR 825.300 – Required Notices This notice is also where Disney tells you if a fitness-for-duty certification will be required before you return.

If the medical certification is incomplete or unclear, Sedgwick must identify the specific problems in writing and give you at least seven calendar days to fix them. If You’ll Need Recertification Later

For intermittent leave or ongoing conditions, Sedgwick can ask you to recertify periodically. Generally, the company must wait at least 30 days between recertification requests, or until the minimum duration on your original certification expires, whichever is longer. For conditions expected to last more than six months, recertification can be requested every six months in connection with an absence.8eCFR. 29 CFR 825.308 – Recertifications The recertification uses the same WH-380-E or WH-380-F form. Treat it the same way as the original: complete employee section, provider completes medical section, return by the deadline.

Make Sure You’re Actually Eligible Before You File

The form doesn’t create eligibility — federal law does. Before you invest time in the paperwork, confirm you meet all three requirements:

  • At least 12 months of employment with Disney. The months don’t have to be consecutive, though gaps of seven years or more generally don’t count. Seasonal Cast Members who return year after year can piece together the service time.9eCFR. 29 CFR 825.110 – Eligible Employee
  • At least 1,250 hours of actual work in the 12 months before your leave begins. Only hours physically worked count; paid time off, vacation, and holidays don’t add to the total. Part-time Cast Members should check their hours in payroll before filing.
  • Your worksite has at least 50 Disney employees within a 75-mile radius. This is met at virtually every Disney location.

If you don’t meet all three, the form won’t rescue the claim. If you do, filling it out carefully is the difference between a leave that starts on time and one that gets held up in the cure period while you’re already dealing with a medical situation.
