PS Form 1221, the Advanced Sick Leave Authorization, is the document your USPS installation head signs to authorize an advance of up to 30 days (240 hours) of sick leave for a serious illness or injury. Despite the name, it is not a request form you fill out. Who Can Get Advanced Sick Leave
Advanced sick leave is available to career USPS employees. Under Section 513.511 of the Employee and Labor Relations Manual, up to 240 hours may be advanced when an employee has a serious disability or illness and management has reason to believe the employee will return to duty afterward.2United States Postal Service. If your condition is not expected to allow you to come back and earn the time back, the advance will not be approved.
You do not have to burn through your existing sick leave, annual leave, or donated leave first. The ELM specifically states that sick leave may be advanced “whether or not the employee has an annual-leave or donated-leave balance.”2United States Postal Service. Employee and Labor Relations Manual – 513 Sick Leave Management may still ask why you need an advance if you have hundreds of hours available, but there is no rule forcing exhaustion.

How to Submit the Request
Start by telling your immediate supervisor in writing that you need advanced sick leave. A short letter or memo is enough. Include the nature of your absence in general terms, the dates you expect to be out, and the number of hours you are asking for. Attach your medical documentation.
Your supervisor forwards the package to the installation head, meaning your postmaster or plant manager. That is the only person authorized to approve the advance, and they can do so without approval from anyone higher.2United States Postal Service. What the Medical Documentation Has to Say
Every advanced sick leave request must be supported by medical documentation.2United States Postal Service. Employee and Labor Relations Manual – 513 Sick Leave There is no special USPS medical form. A letter from your treating provider is fine, but it needs to clear a real bar.
Under ELM 513.364, the documentation should come from your attending physician or another practitioner acting within the scope of their practice. It has to describe the illness or injury clearly enough for management to understand that you cannot perform your normal duties during the absence. Vague phrasing like “under my care” or “received treatment” is specifically identified as unacceptable evidence of incapacitation.2United States Postal Service. A useful medical statement covers the general nature of the condition, explains why work is not possible, and gives an estimated return-to-work date. You are not required to disclose a specific diagnosis to your supervisor, but the letter should make plain that the condition is serious. The estimated return date carries real weight, because the entire approval turns on management’s belief that you will come back.
What Is on PS Form 1221

- Post office, state, and ZIP code of your installation
- Employee name and Social Security number
- Finance number for the installation
- Date you entered on duty
- Start and end dates of the advanced sick leave period
- Number of hours authorized, up to 240 for a full-time employee
- Pay period and year for the start and end dates
- Signature and date from the installation head
- A remarks field, with an explicit instruction that no medical information be entered there
Repaying the Advance
When you return to work, the advanced hours sit as a negative sick leave balance. The ELM allows two ways to pay it down:2United States Postal Service. Employee and Labor Relations Manual – 513 Sick Leave
- Newly accrued sick leave is applied against the negative balance each pay period until the advance is repaid.
- You can ask that an equivalent amount of annual leave be charged against the advance instead, but only if you make that request before the annual leave would otherwise be forfeited under the carry-over limit. This is a repayment mechanism, not a routine swap.
If you separate from federal service still owing advanced sick leave, you have to refund the outstanding amount, and the agency may deduct it from your final paycheck. Asking for More Hours Before Your First Advance Is Repaid
If recovery runs longer than expected, you can request additional advanced sick leave even before finishing repayment on an earlier advance. The one limit is that the total outstanding advance cannot exceed 240 hours at any given time. A new advance does not touch any sick leave you have already earned.2United States Postal Service. If Your Request Is Denied
Installation heads have wide discretion, and not every request is approved. Common reasons include medical documentation that fails to explain why you cannot work, a condition where management does not reasonably expect you to return, or circumstances that raise doubt about repayment.
If you believe the denial was unjustified, you can file a grievance through your union under the collective bargaining agreement. Arbitration decisions show that unions have successfully challenged denials argued to be arbitrary or inconsistent with ELM 513.5. Contact your shop steward or local union office as soon as you receive a written denial. Grievance timelines under Article 15 are strict, and missing them can end your right to challenge the decision.
