How to Fill Out FS Form 3500: Continuation Sheet for Securities

FS Form 3500 is a one-page continuation sheet from the Bureau of the Fiscal Service, officially titled “Continuation Sheet for Listing Securities.” You attach it to another TreasuryDirect form when the primary form runs out of rows for listing savings bonds by issue date, face amount, serial number, and inscription.1TreasuryDirect. What FS Form 3500 Is (And What It Isn’t)

A number of online descriptions incorrectly label FS Form 3500 as a “Direct Deposit Sign-Up Form” or a bank-change form. It is neither. Its only job is to give you extra space to list securities that don’t fit on the primary form you’re filing.2TreasuryDirect. If your goal is to change the bank account linked to your TreasuryDirect account, the form you need is FS Form 5512, “TreasuryDirect Redemption and/or Bank Change Request.” FS Form 3500 only comes into play if you’re attaching it to that form (or another TreasuryDirect form) because you ran out of listing space.

When You Need It

You’ll reach for FS Form 3500 any time a primary TreasuryDirect form doesn’t have enough rows for all the securities or accounts you need to identify. The most common trigger is filing FS Form 5512 for a larger portfolio: that form has limited space for securities, and its instructions direct you to attach FS Form 3500 if you need to list more.3TreasuryDirect. If the rows are full, this sheet picks up where they leave off.

How to Fill It Out

Download the current version from the “Forms” link in the footer of TreasuryDirect.gov. Print it and complete it in ink or by typewriter, as the form itself instructs.2TreasuryDirect. FS Form 3500 – Continuation Sheet for Listing Securities

At the top of the sheet, write in the number of the primary form this continuation belongs to. If you’re attaching it to FS Form 5512, enter “5512” in the blank after “The bonds described on this form are included in the attached FS Form ___.”

The body is a table with four columns. For each bond, enter:

  • Issue Date. The month and year printed on the bond.
  • Face Amount. The denomination, such as $50, $100, or $500.
  • Bond Number. The serial number printed on the bond certificate.
  • Inscription. The full name, middle name or initial, address, and complete Social Security Number of each person listed on the bond, exactly as they appear on the bond itself.

Copy every field directly from your bond certificates or your TreasuryDirect account records. If the inscription doesn’t match what the Bureau of the Fiscal Service has on file, it can delay processing of the primary form the continuation sheet is attached to.

Signature Certification

When FS Form 3500 is attached to FS Form 5512, it inherits that form’s signature certification requirement. Acceptable seals and stamps include a financial institution’s official seal or stamp, a signature guarantee seal, an endorsement guarantee stamp, a corporate seal, an issuing or paying agent stamp showing the institution’s name, location, and identification number, or a medallion stamp from a Treasury-recognized signature guarantee program such as the Securities Transfer Agents Medallion Program.3TreasuryDirect. Only original signatures are accepted. Don’t send photocopies or forms with stamped signatures. A trackable mailing method is worth the small added cost given that the envelope contains your Social Security Number and bank details. The request will not be processed until the Bureau of the Fiscal Service receives and approves the form along with any required supporting documentation.5TreasuryDirect. How Do I…?
