How to Fill Out Form PIT-V: Lines, Mailing, and Online Payment

To fill out Form PIT-V, download the voucher from the City of Portland Revenue Division, enter the tax year, your Social Security Number (and your spouse’s if filing jointly), your name and mailing address, and the payment amount, then mail it with a check or money order payable to “City of Portland” to PO Box 9250, Portland, OR 97207-9250.1Portland.gov. Pay Your Personal Tax If you owe both the Metro Supportive Housing Services (SHS) tax and the Multnomah County Preschool for All (PFA) tax, you need a separate voucher and a separate check for each one.

When You Need the Voucher

Form PIT-V is only for mailed payments in three situations: a quarterly estimated payment, a payment sent on the original due date while you file an extension, and a balance-due payment mailed separately from your return.2City of Portland. Form PIT-V, Metro/Multnomah County Personal Income Tax Payment Voucher

Skip the voucher if you are paying through Portland Revenue Online at pro.portland.gov, or if you are including your check with an original or amended return. In both cases the payment is already tied to a record, and a second voucher only creates a duplicate.

What To Enter on Each Line

Download the voucher from the City of Portland Revenue Division website. Both a fillable PDF and a printable version are posted.1Portland.gov. Pay Your Personal Tax The form is short, but the Revenue Division processes it through high-speed scanners, so handwritten entries need to be legible.

  • Tax year. The year the payment applies to. An incorrect year sends your money to the wrong period.
  • Social Security Number. Yours, plus your spouse’s if you file jointly.2City of Portland. Form PIT-V, Metro/Multnomah County Personal Income Tax Payment Voucher
  • Name and address. Your full legal name and current mailing address, matching your federal and state returns.
  • Payment amount. For a balance due, the figure from your SHS or PFA return. For a quarterly estimate, one-quarter of your projected annual liability, or one-quarter of your prior-year liability if you are using that safe harbor.

Separate the voucher from any instruction pages before mailing. The voucher itself is the only sheet that belongs in the envelope with your check.

Preparing the Check and Mailing It In

Make the check or money order payable to “City of Portland.” On the check, write your name, your SSN, the tax program (Metro SHS or Multnomah County PFA), and the tax year.2City of Portland. Send both to:

The Revenue Division does not send a receipt for mailed payments. Use tracked mail and watch your bank statement for the check to clear. Allow roughly two to four weeks, longer around the April filing peak.

If You Owe Both SHS and PFA

Fill out a separate PIT-V for each program and write a separate check for each one.1Portland.gov. Pay Your Personal Tax Combining both taxes on a single check is the fastest way to have the money posted to the wrong account. Two vouchers, two checks, one envelope is fine.

Using PIT-V With a Filing Extension

The Revenue Division grants an automatic six-month extension to file, with no separate form required. There is no extension of time to pay: the full balance is still due on the original due date, generally April 15.1Portland.gov. Pay Your Personal Tax

If you need more time to file but expect to owe, mail Form PIT-V with your best estimate of the balance by April 15, then file the return when it is ready. Paying late, even under a valid filing extension, triggers a late payment penalty of up to 25% of the total tax liability.3Portland.gov. Tax Administration Policy – Penalty Assessments Rounding your estimate up and getting a refund later is cheaper than underpaying.

Using PIT-V for Quarterly Estimated Payments

Starting with the 2026 tax year, quarterly estimated payments are required for each program if your annual liability for that program is $5,000 or more and your prior-year liability was $1,000 or more.4Portland.gov. For calendar-year filers, the quarterly due dates are April 15, June 15, September 15, and January 15 of the following year.6Metro. Supportive Housing Services Taxes Frequently Asked Questions Send one PIT-V per program per quarter. If you owe estimates for both SHS and PFA, that is two vouchers and two checks each quarter.

You can avoid the underpayment penalty by meeting either safe harbor: pay at least 90% of the current year’s liability, or 100% of the prior year’s liability, by the quarterly due dates.4Portland.gov. Personal Income Tax Filing and Payment Information The prior-year method is simpler because the number is already known. Miss both, and the underpayment penalty is 5% of the shortfall, with a $5 minimum.3Portland.gov. Tax Administration Policy – Penalty Assessments
