DAF Form 4446 is the Air Force Physical Fitness Readiness Assessment scorecard, and filling it out is a shared task: you complete Part I with your personal information before test day, the test administrator records your raw scores and points in Part II during the assessment, and Part III captures the signatures that turn the scorecard into an official record. Learning how to fill out DAF Form 4446 correctly is mostly about knowing which fields belong to you, which belong to the administrator, and what to check before you sign.
Start With the Current Version of the Form
Download DAF Form 4446 from the DAF e-Publishing website at e-publishing.af.mil. The Air Force Personnel Center fitness page also links to it, though access may require a CAC.1Air Force’s Personnel Center. Fitness Check the revision date in the lower-left corner against the most recent edition before you use it; an outdated form risks having your results flagged as invalid. Your Fitness Assessment Cell (FAC) or Unit Fitness Program Manager (UFPM) will normally have current copies on hand.
Part I: The Member Section You Complete Yourself

Gender and age are not just administrative entries. They determine which scoring table the administrator uses for every component of your test, so an error here produces an incorrect composite score. Double-check both before you hand the form over.
Part I also includes a field asking whether you are eligible for a diagnostic PFRA. A diagnostic assessment is recorded but does not count as your official score. Eligibility depends on factors listed on the form itself, such as testing before the 16th day of your due month or holding TR, IMA, or DSG status. During the 2026 transition window, every fitness test between March 1 and June 30 is diagnostic regardless of who is taking it.3U.S. Air Force. Come to the test knowing which exercise you plan to select in each component. Your choice locks in once the assessment starts, and the administrator will write your selection into Part II.
Part II: What the Administrator Records
You do not fill out Part II. The test administrator does, and your job is to watch it get filled in correctly. Part II captures every raw data point from your assessment:249th Force Support Squadron. DAF Form 4446 Air Force Fitness Scorecard
- Your height and weight
- The date of your Fitness Screening Questionnaire (FSQ)
- The PFRA date
- Which exercise option you selected in each component
- Your raw performance in each component (repetitions, times, or ratio)
- Whether you met each component minimum
- The point score for each component
- The composite score
- Any exemptions that apply
If something goes wrong during the test, whether an injury, illness, or inability to finish, the administrator marks “Did Not Finish” (DNF) and adds notes explaining the circumstances. The form itself states that a PFRA interrupted by injury becomes official unless the Unit Commander requests it be invalidated within six duty days, or by the conclusion of the next UTA for non-AGR Reserve members.
Accuracy in Part II matters enormously. A misrecorded run time or wrong repetition count changes your point total, and fixing it later means going back to the original signed form. Ask the administrator to read your results back to you before you sign anything.
Part III: Signatures That Make It Official
Part III turns the scorecard into a military record. Three signatures are required at minimum:249th Force Support Squadron. DAF Form 4446 Air Force Fitness Scorecard
- Your signature, confirming that you accept the results as your official PFRA and acknowledge that the scores reflect your performance
- The PFA Administrator’s signature, certifying that proper procedures were followed
- A FAC or UFAC representative’s signature, validating the record
The form also carries a Unit Commander signature block. It is used when a commander needs to validate or invalidate results, most commonly after an injury during the test triggers command review.
Before you sign, read Part II back against what you actually did. If a score is wrong, Part III includes a checkbox to dispute the results under DAFMAN 36-2905. Once you sign accepting the record, correction requires going through the dispute process.
Part III also holds two elections worth understanding. If you tested on a diagnostic PFRA and want the result to count as an official attempt, check the appropriate line before signing. The section also records your next PFRA due date and, where relevant, the expiration dates for any component exemptions covering strength, endurance, or cardio.
Medical Exemptions on the Form
When a medical provider grants a profile exempting you from one or more components, the exempted components drop out of your composite calculation and the remaining components are proportionally reweighted to fill the 100-point total.4Air Force Personnel Center. Members assessed on the 2-kilometer walk (because they are medically prohibited from running) are treated as component exempt for cardiorespiratory scoring. They receive a pass or fail on the walk, but no cardio points are awarded, and they cannot earn an Excellent rating.5Department of the Air Force. After You Sign: Upload and Retention
Once Part III is signed, the completed DAF Form 4446, together with your Fitness Screening Questionnaire and any AF Form 469 medical profile, gets uploaded into the myFITNESS digital platform.4Air Force Personnel Center. You do not upload the form yourself. Your FAC or UFPM handles the data entry.
Scores typically appear in your personnel record within a few business days after entry. The FAC or UFPM retains the physical copy of DAF Form 4446 for at least 12 months as a backup.4Air Force Personnel Center. If a data-entry error puts the wrong run time or repetition count into myFITNESS, the original signed scorecard is what gets pulled to fix it. Verify your results in myFITNESS once they post rather than finding a mistake months later at a promotion board.
Diagnostic Versus Official in 2026
The Air Force paused all official PFRA testing effective January 1, 2026 to allow the transition to the updated program.6Air Force. The form is completed the same way for diagnostic and official assessments; the only difference is how the result is classified in Part III. If you tested diagnostically and want that result to count officially, use the election line on the form before signing.
