DAF Form 113 is the Department of the Air Force’s official Physical Fitness Assessment scorecard. Assessors use it to record each Airman’s PFA results, including the waist-to-height body composition measurement, and the completed form is uploaded to the Air Force Fitness Management System (AFFMS II) as the permanent record of that assessment.1Department of the Air Force. The form is filled out by the Unit Fitness Program Manager or a Fitness Assessment Cell member, not by the Airman being tested. You can pull a blank copy from the Department of the Air Force e-Publishing site at e-publishing.af.mil under Forms. Use only the current version posted there.
What Goes on the Form
DAF Form 113 captures the Airman’s identifying information and every scored component of the PFA. That includes name, rank, unit of assignment, date of assessment, and results for aerobic fitness, muscular endurance, and body composition. Step-by-Step for Assessors
Start with the identification block: full name, rank, unit, and the date of the assessment. Then record results for each PFA component as the Airman completes them. The form must be filled in at the time of the assessment, not reconstructed later.2Department of the Air Force. DAFMAN 36-2905 Fitness Program
For the body composition section, enter the raw waist circumference in inches, the Airman’s height in inches, and the calculated waist-to-height ratio (WHtR). If the Airman fails any component, document that failure on the form before anyone signs. Both the assessor and the Airman sign to confirm the results are accurate.
Taking the Waist and Height Measurements
Waist circumference is measured at the midpoint between the last palpable rib and the top of the iliac crest, roughly at or just above the belly button, with the tape parallel to the floor. Height runs from the soles of the feet to the top of the skull. Both go on the form in inches, rounded down to the nearest half inch.2Department of the Air Force. DAFMAN 36-2905 Fitness Program
Measure the waist twice. If the two readings differ by more than one inch, take a third measurement and average the two closest. That average is what goes on DAF Form 113. Divide waist by height to get the WHtR, then score it against the current Physical Fitness Readiness Assessment charts.
Waist-to-Height Ratio Scoring

- WHtR of 0.49 or below — 20.0 points
- WHtR of 0.50 — 19.0 points
- WHtR of 0.51 — 18.0 points
- WHtR of 0.52 — 17.0 points

- WHtR of 0.55 — 12.5 points
- WHtR of 0.56 — 10.0 points
- WHtR of 0.57 — 7.5 points

What a Failing Body Composition Score Triggers
An Airman whose WHtR exceeds 0.55 on an initial assessment is enrolled in a 12-month informal self-directed Body Composition Improvement Program. This first enrollment is not treated as a failure to meet the standard, and commanders are specifically directed not to take administrative action during this period. The Airman schedules a medical evaluation at their Medical Treatment Facility, reviews the Health and Readiness Optimization (HeRO) Body Composition Guide, and submits a written improvement plan to their Body Composition Monitor and unit commander.1Department of the Air Force. If the Airman still exceeds 0.55 after the informal program, they enter the formal self-directed BCIP. That enrollment counts as the first official failure to meet the standard. The member reassesses during their next birth month or within 12 months of the last body composition assessment, whichever applies, and continues or adjusts the improvement plan.1Department of the Air Force. Repeated failures carry career consequences. When no medical condition prevents the Airman from meeting the standard, commanders can pursue administrative actions up to and including separation, and members who exceed body composition standards are less likely to receive promotions. Each result, pass or fail, is recorded on DAF Form 113 and stays in the permanent fitness record.
Where the Form Is Filed and How to Correct Errors
Once signed, DAF Form 113 is uploaded to AFFMS II. Unit Fitness Program Managers are responsible for entering forms promptly and accurately. Commanders, supervisors, and the Airman can all access the record, which feeds into promotion decisions, retention reviews, and any administrative actions tied to repeated failures.2Department of the Air Force. If a score is recorded incorrectly, start with the Unit Fitness Program Manager and bring supporting documentation. Active-duty, Reserve, and Air National Guard members can also work through their local Military Personnel Section. Medical Exemptions
An Airman with a documented medical condition that prevents participation in the body composition assessment may receive an exemption. A military medical provider documents the condition, and the exemption applies only to the specific component affected. An Airman exempted from the BCA still completes the remaining PFA components, and the exemption defers the requirement until the condition resolves rather than removing it.2Department of the Air Force. What DAF Form 113 Is Not Used For
DAF Form 113 does not document Letters of Counseling, Letters of Admonishment, or Letters of Reprimand. Those administrative actions are governed by DAFI 36-2907 and use different paperwork. Verbal counseling is recorded on DAF Form 174 (Record of Individual Counseling). If you received a body composition failure on DAF Form 113 and a separate letter of counseling or reprimand, those are two distinct records moving through two different processes. The scorecard tracks the assessment; the administrative letter handles conduct or performance under the adverse actions instruction.
