To fill out AF Form 108, enter your identifying information and failed assessment scores in Section I, check at least one intervention program in Section II and document your remedial plan, log every session on page two as you complete it, and route the form for your signature, the Unit Fitness Program Manager’s signature, and your commander’s review in Section III. The form, officially titled “Air Force Fitness Education and Intervention Processing,” is a two-page working document that stays open until you’ve finished your interventions and retested.
When You’ll Be Given One
The form is triggered whenever you score below 75 on your composite Physical Fitness Readiness Assessment, or fall below the minimum on any single component even if your composite is 75 or higher.1Air Force Personnel Center. Enrollment in the Fitness Readiness Program has to begin within 10 duty days of the failed score. Air Reserve Component members get 60 calendar days. Where to Get the Form

Department of the Air Force E-Publishing In practice, your UFPM will usually hand you a copy or generate one as part of enrollment rather than sending you to download it.
Section I: Individual Information
The top block captures who you are and exactly how the failed assessment scored. Fill in:
- Name (last, first, middle initial)
- Rank
- Date of birth, duty phone, and unit
- Fitness assessment date, meaning the date of the PFRA you failed
- Profile: mark yes or no depending on whether you had an AF Form 469 on file at the time of the test
- Component scores and raw numbers: aerobic score and time, push-up score and reps, sit-up score and reps, and abdominal circumference score and measurement
- Composite score
- Height and weight
Every score entry has to match the myFitness record exactly. If a number is off, the UFPM will kick the form back for correction, so cross-check before you sign anything. The form does not ask for a Social Security Number; identification is by name, rank, date of birth, and unit.
Section II: Choosing and Tracking Interventions
Section II is where the remedial work is planned and, over time, recorded. Five intervention options appear as checkboxes:
- BE WELL Online, a self-paced digital course on exercise, nutrition, and behavior change
- Healthy Weight Program, focused on weight management with health promotion staff or dietitians
- Cardiovascular Intervention, targeted at your aerobic score
- Strength Intervention, focused on push-ups, sit-ups, or other muscular fitness gaps
- Military OneSource Health Coaching, telephonic coaching on fitness, nutrition, and lifestyle
You must check at least one. You and your commander can agree on more than one if your weaknesses call for it.4Air Force Medicine. The lower half of Section II is completion tracking. Each intervention has a yes/no/N/A field and a signature line for the facilitator. When you finish BE WELL Online, for example, submit the completion certificate to the UFPM and they’ll sign and date the corresponding line. The form isn’t a one-and-done document; it stays open and gets updated as each piece is finished.
Signatures and Section III: Commander’s Review
Three signatures close out Section II: yours, the UFPM’s, and your unit commander’s (or a designated representative’s). Your signature acknowledges that you’re enrolled in the Fitness Readiness Program and responsible for completing the interventions you selected. The commander’s signature directs you into the program.
Section III is a narrative block where the commander can add guidance, note concerns about your progress, or record any administrative actions taken alongside the fitness failure, then signs to close out the review.
If you refuse to sign, the assessment result and the program enrollment still stand. The form’s privacy notice states that disclosure is voluntary, but declining to sign does not remove your obligation to improve and retest.
Page Two: The Session Log
Page two is a running log of every intervention session you attend. Each row takes the intervention name (with a class number where it applies, such as “WM Class 2 of 4”), the date, notes about the session, and the instructor’s validation signature. Update it after every session. This page is the proof you showed up and did the work, and it’s the part reviewers look at to confirm you actually completed what Section II says you would.
Medical Profiles Before You Sign Anything
If you have a condition that limits your ability to complete one or more components, an AF Form 469 needs to be on file before the PFRA, and Section I’s profile field should reflect that. If you flagged a limiting condition on the Fitness Screening Questionnaire but don’t have a current AF Form 469, you have to see a medical provider before testing; the provider completes the screening questionnaire, issues the AF Form 469 if warranted, and you bring the paperwork back to the UFPM.2Department of the Air Force. The Retest Deadline the Form Is Driving Toward
Everything on the form is pointing at one date: you have to retest within 30 days of the failed PFRA. Fail the retest and you stay in the Fitness Readiness Program with another 30-day clock.
