DA Form 4187, titled “Personnel Action,” is the Army’s general-purpose request form for actions like reenlistment, reassignment, and name changes. It has historically been the catch-all document a soldier submits when the action they need doesn’t have its own dedicated form. That role is shrinking: the Integrated Personnel and Pay System-Army (IPPS-A) now absorbs most 4187 functions digitally through its Personnel Action Request feature, and Army regulations are being rewritten to prescribe IPPS-A in place of the paper form.1The United States Army. Where to Download the Current Version
The Army Publishing Directorate at armypubs.army.mil is the only authoritative source for the current DA Form 4187.2Army Publishing Directorate. Check the date printed in the lower-left corner of the form against the version listed on APD before you start filling it in. The 4187 is an XFA-based PDF. That means it contains interactive drop-downs, auto-populating fields, and digital signature blocks that standard browsers and basic PDF readers cannot render. You need a full version of Adobe Acrobat or an equivalent XFA-capable reader to complete and sign it. If your signature block appears grayed out or the check boxes won’t respond, the reader is the problem, not the form.
Information to Have Ready Before You Start
Gather your data before you open the PDF. You will need your ten-digit Department of Defense identification number, which appears on the back of your Common Access Card.4Defense Travel Management Office. Having these details in front of you keeps a straightforward request from bouncing back and forth for days.
Watch the date format. DA form date fields use YYYYMMDD, so March 15, 2026 is entered as 20260315. Army correspondence like memorandums and letters uses a different convention under AR 25-50, where the same date is written out as “15 March 2026.”5Army Publishing Directorate. Supporting Documents and Common Reasons for Return
Most 4187 actions require attachments. The instructions section of the form lists what those are for each type of request. Skipping this step is the most common reason a Personnel Action gets returned. Read the instructions before you route the form, not after.
Signing and Routing
Digital signatures require a Common Access Card inserted in a reader connected to a government workstation. The CAC’s cryptographic certificate authenticates your identity and locks the document from further editing once you sign.6Common Access Card. A completed 4187 moves up the chain of command. A typical personnel action goes from the soldier to a squad leader or section NCO, to the platoon sergeant, to the company commander, and finally to the battalion S1 shop for processing. Each reviewer checks the form for accuracy and adds their own digital signature before passing it forward.
Do not sign for someone else, and do not submit a form you know contains false information. Forging a signature or submitting a fraudulently signed document can be prosecuted under UCMJ Article 132 for fraud against the United States.7GovInfo. 10 USC 932 – Art. 132. When to Use IPPS-A Instead
The biggest change to the 4187 in decades is that most of what the form used to do is now done in IPPS-A. A June 2026 ALARACT message reinforced that all units must use IPPS-A for leave, awards, pay queries, and other personnel actions that previously required a paper or PDF submission.9IPPS-A. Inside IPPS-A, the Personnel Action Request (PAR) has replaced most functions of the 4187. Preformatted templates cover transitions, retirements, and officer actions, and the system routes the request through the approval chain automatically.10U.S. Army Garrison Bavaria. IPPS-A Personnel Action Requests (PAR) If your unit still asks you to fill out a paper 4187 for something the PAR handles, that is a sign the local process has not caught up to current policy. Where the Completed Form Ends Up
Personnel actions that affect a soldier’s career are uploaded to the Interactive Personnel Electronic Records Management System (iPERMS), the Army’s official digital archive for the Army Military Human Resource Record.11iPERMS. iPERMS RMA Verify that your key documents actually appear there by conducting an annual record review. Retention rules for Army records are governed by AR 25-400-2, which directs users to the Records Retention Schedule-Army maintained on the ARIMS website for the specific disposition tied to each record number.13U.S. Army National Guard. Army Records Management Program
Privacy Handling
The 4187 collects personally identifiable information, so it carries a Privacy Act Statement explaining why the data is collected and how it will be used. Under AR 25-22, “The Army Privacy Program,” documents containing personal information are treated as For Official Use Only, and access is restricted to people with a direct need-to-know.15U.S. Army. A completed form sitting in a physical inbox should be kept face-down or in a folder marked FOUO.
If the Error Is Already in Your Record
A 4187 requests a new personnel action going forward. It is not the vehicle for fixing a mistake already sitting in your file. If your military records contain an error or an unjust entry, DD Form 149, “Application for Correction of Military Record,” is the form that goes to the Army Board for Correction of Military Records. It covers pay errors, missing awards, incorrect evaluation reports, and derogatory information, and the Army offers an online application portal that is faster than mailing the paper form.16U.S. Department of War.
