To fill out DA Form 705, download the current April 2022 version titled “Army Fitness Test Scorecard,” enter the Soldier’s administrative data at the top, record raw results and scaled point scores for each of the five AFT events with grader initials in every row, mark whether the Soldier is being scored under the general or combat standard, and finish with the Soldier’s signature and the OIC or NCOIC signature at the bottom. Every field feeds the Digital Training Management System, so accuracy in the small boxes matters as much as the scores themselves.
Start With the Right Version of the Form
Pull DA Form 705-TEST from the Army Publishing Directorate. The current edition is dated April 2022 and is titled “Army Fitness Test Scorecard.”1Army Publishing Directorate. DA Form 705-TEST The PDF is fillable, so you can type entries on a computer or print blanks for use at the testing site.
Check the bottom-left corner of any pre-printed copy for “DA FORM 705-TEST, APR 2022” before writing on it. Older versions, especially any that reference the legacy Army Physical Fitness Test, will not clear a records audit. Since June 1, 2025, the Army Fitness Test has replaced the ACFT, dropping the Standing Power Throw and leaving five scored events.2The United States Army. Army Fitness Test
Complete the Administrative Section
The top of the scorecard identifies the Soldier and links the test to the correct personnel record. Every entry has to match official records exactly; a misspelled name or a transposed digit can stall or block the DTMS upload.
- Name: last name, first name, and middle initial as they appear in your personnel file.
- Sex: male or female, which determines the scoring table under the general standard.
- Unit/Location: assigned unit and the installation or testing site.
- Date: the test date in YYYYMMDD format.
- MOS: Military Occupational Specialty code.
- Pay Grade: current pay grade (E-4, O-3, and so on).
- Age: age on the date of the test, which sets the age bracket for scoring.
- Standard: mark “General” or “Combat.”
Record Each Event Result
The AFT has five events, and each occupies its own row on the scorecard. The rows are listed in reverse order from how the events are conducted on the field, so the deadlift sits near the bottom and the two-mile run sits near the top. - 3-Repetition Maximum Deadlift (MDL): record the heaviest weight lifted for three reps, in pounds. Check boxes are provided for standard weight increments.
- Hand-Release Push-Up – Arm Extension (HRP): record the total number of correct repetitions completed within two minutes.2The United States Army. Army Fitness Test
- Sprint-Drag-Carry (SDC): record the overall event time in minutes and seconds. The event covers five 50-meter shuttles: sprint, drag, lateral, carry, and sprint.
- Plank (PLK): record the total time the Soldier held proper position, in minutes and seconds.
- Two-Mile Run (2MR): record overall run time in minutes and seconds.
Graders initial next to each event they personally observed. If lanes are split across multiple graders, each one initials only the events they scored. A missing set of grader initials is one of the most common reasons a scorecard gets kicked back during review.
Alternate Aerobic Events
A Soldier with a medical profile that prevents running takes one of four alternate aerobic events in place of the two-mile run. Circle or select the option used on the dedicated row: 5,000-meter row, 1,000-meter swim, 12,000-meter stationary bike, or 2.5-mile walk.4Army Publishing Directorate. One rule catches people out: a Soldier who completes an alternate aerobic event within the maximum allowable time receives a flat 60 points for that event, no matter how quickly they finished.5Army University Press. Convert Raw Results to Points
After the raw result is recorded, convert it to a scaled point score using the official AFT scoring tables at army.mil/aft.3U.S. Army Recruiting Command. DA Form 705 – Army Fitness Test Scorecard Each event is worth up to 100 points. A Soldier needs at least 60 points on every event, for a minimum total of 300, to pass.6GoArmy. Enter each event’s points in the “Points” column, then add all five and put the sum in the “Total Points” box at the bottom of the scoring section.
Mark the Correct Standard
Near the top of the form is a small but consequential field for standard designation. Under the general standard, scoring tables adjust by age and sex. Under the combat standard, which applies to Soldiers in 21 direct-combat MOSs, scoring is sex-neutral: every Soldier in that MOS meets the same raw thresholds.7Joint Base San Antonio. Active-component Soldiers in combat MOSs must be scored under the combat standard starting January 1, 2026. Reserve-component Soldiers follow on June 1, 2026.2The United States Army. Body Composition Fields
The lower section of the scorecard collects body composition data: height in inches, weight in pounds, body fat percentage, and the date measured. Height and weight cannot be taken on the same day as the AFT. The form itself notes that body composition measurement should occur at least seven days before or after the fitness test when feasible.3U.S. Army Recruiting Command. Signatures at the Bottom
The scorecard has two signature blocks, not three. The Soldier signs to acknowledge the results and confirm that the administrative data is correct. The OIC or NCOIC signs to certify that the test was conducted properly and that the recorded scores are accurate, and prints their name in the adjacent field.3U.S. Army Recruiting Command. Graders do not sign at the bottom. Their initials go next to each event they scored, in the event rows above. Some units treat that as a third required signature and hold up otherwise complete paperwork looking for one; it isn’t there.
After the Form Is Signed
Once both signatures are in place, the OIC or NCOIC typically walks the scorecard to the unit training office. Training personnel verify that every field is filled and both signatures are present before entering the results in DTMS, the Army’s centralized training-records system. AFT and ACFT results in DTMS update the Soldier’s record automatically and become visible to promotion boards and school selection panels.8The United States Army. Units generally keep a paper copy of each DA Form 705 in the Soldier’s local file. Keep your own copy too. If a DTMS entry is wrong months later, the signed scorecard is your proof.
One boundary worth noting: only record tests produce a scorecard that goes into DTMS. Commanders can run diagnostic tests for training purposes, but a diagnostic AFT does not generate an official DA Form 705 for the Soldier’s record.6GoArmy. Army Fitness Test and Requirements
