How to Fill Out Certified Mail: PS Forms 3800, 3811, and Fees

To fill out Certified Mail, you complete PS Form 3800 (the Certified Mail receipt with the tracking barcode) with the recipient’s address and your return address, and, if you want a signed receipt back, you also complete PS Form 3811 (the green Return Receipt card). Both forms are free at any post office, and the whole process takes about five minutes once you know which box holds what. One boundary worth stating up front: Certified Mail by itself does not include the green card. Return Receipt is a separate add-on you buy if you need the recipient’s signature returned to you.

What to Have Ready Before You Write

Gather these before you pick up a pen:

  • The recipient’s full name and complete mailing address. If you’re serving a legal notice, match the name exactly as it appears on the documents.
  • Your full name and return address. This determines where the signed green card comes back to.
  • Your sealed envelope or package. The barcoded label sticks directly to the mailpiece, so seal it first.
  • A blue or black pen. Print clearly. Illegible handwriting is a common reason green cards come back with wrong information or don’t come back at all.

PS Form 3800: The Certified Mail Receipt

PS Form 3800 is a multi-part label. The top portion carries the barcode and tracking number and goes on the envelope. The bottom portion is a detachable receipt you keep.

On the label, print the recipient’s full name and complete address in the “Sent To” section. Print your name and return address in the sender fields. The tracking number is already pre-printed, so there is nothing to write there.

Detach the receipt portion along the perforation once the addresses are filled in. Keep it. If you need this receipt to serve as legal proof of mailing, hand it to a clerk at the counter and ask for a postmark. The form itself states that a USPS postmark on the receipt is what makes it accepted as legal proof.1United States Postal Service. PS Form 3800 – Certified Mail Receipt Without that postmark, you still have a tracking number, but courts and agencies may not accept the receipt alone as evidence that you mailed the item on a specific date.

PS Form 3811: The Green Return Receipt Card

Skip this section if you don’t need a signature returned to you. If you do, fill in the card as follows.

On the front (green side):

  • Box 1, “Article Addressed To”: print the recipient’s full name and complete mailing address.
  • Box 2, “Article Number”: peel the thin white tracking-number strip from the top of your PS Form 3800 and stick it in this box. This is what links the green card to your mailing so USPS can match them.
  • Box 3, “Service Type”: check the box for Certified Mail. Leave the other service types unchecked unless one of them actually applies to your mailing.

On the back (white side): print your own full name and complete return address. This is where USPS mails the card after the recipient signs. If it’s blank or illegible, the signed card has nowhere to go.

How to Attach the Forms to the Envelope

Place the barcoded portion of PS Form 3800 on the front of the envelope, above the delivery address and toward the right side. Leave roughly three and a half inches from the top-right corner for postage. Do not fold or cover the barcode.

Attach PS Form 3811 to the back of the envelope. Peel off the adhesive strips on the back of the card and press it firmly to the envelope, green side out. Press down along every edge. The card has to survive sorting and delivery still stuck to the mailpiece.

Where You Submit It Changes What Your Receipt Is Worth

You have two ways to get the mailpiece into the system, and the choice matters.

At the counter, a clerk verifies the forms, weighs the item, collects postage and the Certified Mail fee, and postmarks your receipt. That postmarked receipt is the gold standard for legal proof of mailing.1United States Postal Service. PS Form 3800 – Certified Mail Receipt If your mailing is tied to a court deadline, tax filing, or legal notice, use this method.

If you don’t need the postmark, you can apply your own stamps (enough to cover First-Class or Priority postage plus the Certified Mail fee), attach the barcoded label, and drop the piece in a USPS collection box.1United States Postal Service. PS Form 3800 – Certified Mail Receipt You’ll still get tracking. Your receipt just won’t carry a postmark.

Signature Options That Change Who Can Sign

Standard Certified Mail lets anyone at the delivery address sign. If you need more control, three add-ons change that:

  • Restricted Delivery. Only the addressee or their authorized agent can sign. Family members or coworkers at the address cannot accept it. This matters when you need to prove the named individual was reached.2United States Postal Service. What is Restricted Delivery
  • Adult Signature Required. The signer must be 21 or older and show photo ID to confirm age.
  • Adult Signature Restricted Delivery. Combines both: only the named addressee can sign, must be 21 or older, and must show photo ID.

Ask for any of these at the counter when you submit the piece. The clerk marks the options on the forms.

What It Costs in 2026

Certified Mail fees are charged on top of regular postage. As of January 2026:3United States Postal Service. USPS Notice 123 – January 2026 Price Change

  • Certified Mail fee: $5.30
  • Return Receipt, green card (PS Form 3811): $4.40
  • Return Receipt, electronic: $2.82
  • Certified Mail Restricted Delivery: $13.70

A standard one-ounce First-Class letter sent Certified Mail with a physical green card runs roughly $10.43 to $11 depending on exact postage. The electronic return receipt saves $1.58 over the paper card and arrives faster, since USPS emails you an image of the signature instead of mailing back a card.

If the Green Card Never Comes Back

If your signed green card is lost or never arrives, you can request a duplicate using PS Form 3811-A. Two requirements catch people off guard: you must still have your original mailing receipt (the detached portion of PS Form 3800 showing you paid for return receipt service), and you must submit the request within 90 days of the mailing date.4United States Postal Service. Request for Delivery Information/Return Receipt – PS Form 3811-A

Bring the original receipt to a post office. A clerk will help you complete the form and verify it against your receipt, and USPS will research the delivery. That’s the reason to keep the detached receipt somewhere safe from the moment you leave the counter. Without it, there’s no way to request a replacement if the card goes missing.
