To fill out CAPF 2B, the unit commander (not the departing member) completes the form’s four sections — member identification, the reason for termination checked in either the cadet or senior section, a written summary of circumstances if the termination is for cause or misconduct, and the commander’s signed certification — then distributes three copies, with the original going to CAP National Headquarters. The form, titled “Personnel Action Request — Termination of CAP Membership,” has been in continuous use since October 1972 and is the only form used to close a CAP membership, cadet or senior.1Civil Air Patrol. Who Fills Out CAPF 2B
The unit commander or a designated representative prepares CAPF 2B. That’s true even when the member is leaving voluntarily. If you’re a member who wants to resign, you submit a written resignation to your commander, and the commander completes the form and attaches your resignation to it.2Civil Air Patrol. Interim Change Letter – CAPR 35-3, Membership
A quick boundary before going further: CAPF 2B is not a membership application. If you’re trying to join CAP, you want CAPF 12, the adult membership application.3Civil Air Patrol. Civil Air Patrol Adult Membership Application
How To Resign as a Member
If you’re leaving CAP, you don’t touch the form. You write a resignation — a letter or an email works, as long as the email comes from you personally — and include your full name and CAP ID number. Send it to your unit commander.2Civil Air Patrol. Interim Change Letter – CAPR 35-3, Membership
Before your record is closed out, return your CAP membership card and any other CAP property to your unit of assignment. That includes issued equipment, ID badges, and organizational materials.4Civil Air Patrol. Commander’s Quick Reference Guide to Membership
Completing the Form Section by Section
The current CAPF 2B is a PDF download from the CAP National Headquarters forms page at gocivilairpatrol.com.5Civil Air Patrol National Headquarters. Forms The 1972 origin date on the form is not a typo; that version remains current.
Section I: Personal Data
Enter the departing member’s last name, first name, and middle initial. Add the CAPSN (CAP serial number), current grade, unit charter number, duty assignment if applicable, wing, and unit name. Verify the CAPSN digit by digit. A transposed number here can process the termination against the wrong member’s record.
Section II or III: Reason for Termination
Cadet terminations go in Section II. Senior member terminations go in Section III. Check one box only.
Section II lists cadet-specific reasons including voluntary resignation, dropping out of school, marriage after age 18, lack of interest (three consecutive missed meetings without an acceptable excuse), joining the armed forces, moving from the area, failing to progress, misconduct, and failing to maintain an acceptable academic record. Some of these — marriage, enlistment, and attendance — are automatic terminations where the commander is documenting something that has already happened.6Civil Air Patrol. Commander’s Quick Reference Guide to Terminations
Section III covers senior members. The two paths are voluntary resignation and termination for cause. Cause categories include conduct involving moral turpitude, conduct unbecoming a CAP member, felony conviction, separation from the armed forces with other than an honorable discharge, serious violations of CAP regulations, making false statements to or about CAP, habitual failure to perform duty, substandard performance over an extended period, failure to obey orders, insubordination, and financial irresponsibility. If none of the listed reasons fit, use the “Other” box and write in an explanation.1Civil Air Patrol. CAPF 2B Form
Section IV: Summary of Circumstances
Section IV is required whenever the termination is for cadet misconduct or senior member cause. Write a concise but thorough summary of the events that led to the termination. If you run out of space, use a separate sheet of plain bond paper and attach it. For voluntary resignations and routine automatic terminations, Section IV can be left blank.
Certification and Signature
The certification at the bottom of the form is where the commander confirms the member was notified of the right to appeal under CAP Regulation 35-3. Print the commander’s name and grade, then sign and date. Electronic submission is allowed, but the form must be printed, signed by hand, and scanned; a typed name in the signature block is not accepted.5Civil Air Patrol National Headquarters. Forms
Attachments and Copies
What gets attached depends on the reason for termination:
- Voluntary resignation: attach the member’s written resignation.
- Termination for cause where the member appealed and lost: attach a copy of the approving authority’s decision upholding the termination, along with the appeal proceedings.1Civil Air Patrol. CAPF 2B Form
Prepare three copies and distribute them:
- Original to National Headquarters at CAP/DP, 105 S. Hansell St., Bldg 714, Maxwell AFB, AL 36112.
- Second copy to the member’s last known address. For appeal-upheld terminations, this copy goes to the appropriate approving authority instead of the member.2Civil Air Patrol. Interim Change Letter – CAPR 35-3, Membership
- Third copy retained in the unit’s file.

A Timing Note on Appeals
For involuntary terminations, the form does not go to National Headquarters until the appeal window has closed or the appeal has been resolved. If the member declines to appeal, they return their card and property, and the commander submits the form. If the member appeals and the approving authority upholds the termination, the commander then submits the form with the appeal decision attached.2Civil Air Patrol. Common Mistakes That Delay Processing
- Checking a box in Section II for a senior member, or Section III for a cadet. The sections aren’t interchangeable, and the mismatch creates a record problem at National Headquarters.
- Leaving Section IV blank on a misconduct or for-cause termination. The summary of circumstances is required, and without it the form will bounce back.
- Forgetting to attach the resignation letter (for voluntary departures) or the approving authority’s decision (for upheld appeals).
- Sending copies to fewer than all three destinations. If the member never receives their copy and later disputes the termination, that gap becomes a problem.
- Submitting an unsigned or typed-signature form. Electronic submissions still require a scan of a hand-signed original.
