Vanguard account forms live in two online libraries: personal investor forms at vanguard.com/us/literature, and employer-sponsored retirement plan forms at retirementplans.vanguard.com. Whichever form you need, the process is the same in shape: find the current version, gather your identifiers and any third-party paperwork the transaction requires, complete every section, and submit through the channel the form’s own instructions specify. Getting any of those steps wrong is what turns a routine update into a rejected submission.
Where To Find the Right Form
The personal forms and literature library is reachable from the main Vanguard navigation under “Forms” or “Support,” or directly at vanguard.com/us/literature. If you’re logged in, the library filters to forms that match the account types you already hold. If you’re not logged in, you see the full catalog and can filter by account category: IRAs, brokerage accounts, employer plans, or 529s.
Employer-sponsored plans like 401(k)s and 403(b)s use a separate library at retirementplans.vanguard.com. Forms such as the IRA Direct Rollover Authorization and plan-specific beneficiary designation kits sit there rather than in the personal investor library. If a form isn’t turning up where you expect it, check the other library before calling.
Before you print or fill anything, verify the version. Vanguard updates forms periodically, and an outdated PDF pulled from an old email or a bookmark can be rejected even when everything on it is correct. Compare the version number or revision date on your copy against the one currently posted in the library.
Information To Gather Before You Start
Most Vanguard forms ask for the same core identifiers. Having these in front of you avoids stop-and-start filling:
- Social Security number or ITIN. Required on account-opening forms like the IRA Adoption Agreement, on ownership changes, and on any form that triggers tax reporting. Vanguard uses it to report dividends, capital gains, and distributions to the IRS.
- Vanguard account number. The specific number for the account you’re changing. If you hold a brokerage account and an IRA, confirm you’ve entered the right one; applying a change to the wrong account creates delays.
- Bank routing and account numbers. Needed to link an external bank for electronic transfers. The nine-digit routing number sits at the bottom left of a check, with your account number next to it. Take them from a check or your bank, not from memory.
A transposed digit in a routing number can send money to the wrong institution, and a mismatched Social Security number can hold your form in compliance review. Accuracy on these fields is worth more than speed.
Beneficiary Designation Forms
A beneficiary form replaces every prior designation for that account type. List every beneficiary you want to keep, not just the one you’re adding or changing. For each person you’ll need their full legal name, including middle initial, and date of birth.1Vanguard. You designate two tiers. Primary beneficiaries inherit first; contingent beneficiaries inherit only if no primary beneficiary survives you. The percentages within each tier must add up to exactly 100%. Three primary beneficiaries at 30% each totals 90%, and the form comes back.
Transfer and Rollover Forms
Moving assets into Vanguard from another firm means completing a transfer form that names both institutions. You need the full name and address of the firm currently holding your assets, your account number there, and ideally a recent statement showing your holdings. Transfer forms typically ask whether you want to move positions in-kind (as-is) or liquidate them into cash before the move. If you want to keep specific funds or stocks intact, list each one clearly.
For retirement money, Vanguard supports two mechanisms. A direct rollover, sometimes called a trustee-to-trustee transfer, moves funds straight from one custodian to another without passing through your hands. An indirect rollover sends you a check, and you have 60 calendar days from the date you received the distribution to deposit the full amount into the new retirement account. Miss the window and the IRS treats the entire distribution as taxable income, plus a 10% early withdrawal penalty if you’re under 59½. Employer plans withhold 20% for federal taxes on indirect rollovers, so you’d have to cover that 20% from other funds to roll over the full amount.
Non-retirement brokerage transfers are simpler but can still require a Medallion Signature Guarantee. Vanguard may require one when the delivering firm isn’t enrolled in an electronic transfer service, when you’re moving assets between different account types (a joint account into an individual account, for example), or when the name on your Vanguard account doesn’t exactly match the name at the other firm.3Vanguard. Trust and Custodial Account Forms
Opening a trust account requires the standard application plus specific pages from the trust document. Vanguard doesn’t need the entire trust agreement; it asks for the pages showing the trust’s name and date, the current trustees’ names, and all signature pages. For a testamentary trust, you’ll also need evidence that the will was filed with the probate court, such as letters testamentary or a court-stamped copy. If a successor trustee is stepping in for one who has died or become incapacitated, Vanguard needs documentation of the reason for the change: typically a death certificate or a physician’s letter confirming incapacity.

Agent Authorization and Power of Attorney
Vanguard offers two levels of agent authorization, and the difference matters. A limited agent authorization lets your designated person buy investments, transfer assets between your Vanguard accounts, and request distributions sent to your address or bank on file. That agent cannot write checks, move money to a third party, change account ownership, update beneficiaries, or close accounts.7Vanguard. Investment Account Authorizations: Types and Requirements
A full agent authorization gives the designated person broad control: buying, selling, transferring, withdrawing, updating personal and banking information, and closing the account without your prior approval. It functions as a durable power of attorney and remains effective if you become incapacitated, as long as you established it before the incapacity. Full authorization requires a notarized signature with two witnesses present and is only available for personal accounts, not trusts or organizational accounts.7Vanguard. If you already have an attorney-drafted power of attorney, Vanguard may accept it, but the firm encourages clients to use its own full agent authorization form instead. Submitting an external POA typically means uploading the document and waiting for Vanguard’s legal team to review it, which usually takes longer than the in-house form.8Vanguard. Documents Vanguard Can’t Generate for You
Some transactions depend on paperwork issued by a court, a government office, or another financial institution. Have these in hand before you submit:
- Death certificate. Required when claiming assets as a beneficiary if Vanguard can’t independently verify the account holder’s passing.9Vanguard. Inheriting an Account
- Letters testamentary or letters of administration. If you’re acting as an executor, Vanguard needs the probate court document confirming your authority, not the will itself. The exact name varies by state.10Vanguard. Helping You, an Executor, Through the Inheritance Process
- Qualified Domestic Relations Order. Dividing retirement plan assets in a divorce requires a court-issued QDRO that meets federal ERISA standards. A divorce decree or separation agreement isn’t sufficient; the order must specifically identify the plan, the participant, the alternate payee, and the amount or percentage to be distributed.11U.S. Department of Labor. - Medallion Signature Guarantee. Available from a bank, credit union, or broker-dealer that participates in a Medallion program. A notary stamp is not a substitute. Call ahead; not every branch has a qualified guarantor on staff.
How To Submit Your Completed Forms
Vanguard accepts forms through several channels, and the right one depends on the form type and whether original signatures are required.
- Online upload. Log in and use the secure message or document upload feature to send completed PDFs. This is the fastest route for most forms. Scans or photos should be clear and uncropped; blurred or partial documents get rejected.

- Fax. Available for some non-monetary forms. Check the specific form’s instructions for the applicable fax number, since Vanguard routes different form types to different departments.
Forms that require original signatures, such as the full agent authorization with its notarized signature and witness requirements, generally need to go by mail or courier rather than digital upload. The instructions printed on each form specify which submission methods it accepts.
What Happens After You Submit
Vanguard’s electronic delivery agreement provides that transaction and account maintenance confirmations become available one business day after the change is processed.13Vanguard. When something is wrong, the form comes back as “Not in Good Order” (NIGO) with a notice explaining what needs to be corrected. The most common causes are missing signatures, incomplete sections, beneficiary percentages that don’t total 100%, mismatched account holder names, and outdated versions of the form. Comparing the version date on your PDF against the current one in the library before you mail anything prevents the last of these on its own.
Time-sensitive transactions leave no room for a second round. If you started an indirect rollover and the receiving-end paperwork bounces, the 60-day clock keeps running from the date you received the distribution. Fix and resubmit immediately; once those 60 days pass, the entire amount becomes taxable.
