A T. Rowe Price withdrawal form is the paperwork you use to take money out of an IRA, mutual fund account, or employer-sponsored retirement plan held at the firm. Most IRA and mutual fund distributions can be completed online after logging in; employer-sponsored plans and a few special requests require a PDF form. All of the forms sit on the “Forms” page at troweprice.com under the “Request a distribution or gift” heading.1T. Rowe Price. Forms The steps below walk through picking the right form, completing it, handling tax withholding, and getting it back to T. Rowe Price cleanly.
Pick the Form That Matches Your Account
Submitting the wrong form is one of the most common reasons a request gets rejected. Match your situation to one of these:1T. Rowe Price. Forms
- IRA distribution, for traditional, Roth, or rollover IRAs. Available online or as a PDF.
- Employer-Sponsored Retirement Plan Distribution, for 403(b) and Individual 401(k) plans. PDF only, because it needs a plan administrator signature.
- SEP-IRA and SIMPLE IRA distribution, a separate PDF for these small-business accounts.
- Mutual fund distribution, for taxable brokerage redemptions. Available online.
- Required minimum distribution, a dedicated online tool that calculates the minimum for you.
- Qualified charitable distribution, for sending IRA funds directly to a charity. Online or PDF.
- IRA excess contribution withdrawal, for removing contributions that exceeded annual limits along with any earnings on them.
If you are 73 or older and haven’t taken this year’s required minimum distribution, use the RMD tool rather than the general IRA form. The RMD age is currently 73 for anyone who turns 73 before January 1, 2033, and rises to 75 after that.2Congress.gov. a href=”https://www.congress.gov/crs-product/IF12750″ target=”_blank” rel=”noopener”>Required Minimum Distribution RMD Rules for Original Owners
What to Have in Front of You Before You Start
Incomplete submissions are the biggest cause of delay. Gather the following first:
- Your T. Rowe Price account number, or Plan ID for an employer-sponsored account, along with your Social Security number.
- The amount you want, expressed as a dollar figure, a percentage of the balance, or a full liquidation. For RMDs the tool calculates the minimum, but you can always take more.
- The specific fund or funds the withdrawal should come from if your account holds more than one. Pulling from the wrong fund can create unintended capital gains or sell shares you meant to keep.
- Your bank’s nine-digit ABA routing number and account number if you want an electronic transfer. A transposed digit puts your money into a rejection loop.
- Your federal and state tax withholding elections, covered below.
Spousal Consent on Employer Plan Forms
If you are married and withdrawing from an employer-sponsored retirement plan, your spouse may need to sign a consent waiver. Federal law requires spousal consent when a married participant elects a distribution in any form other than a qualified joint and survivor annuity, unless the total lump-sum value of the benefit is $5,000 or less.3Internal Revenue Service. Fixing Common Plan Mistakes – Failure to Obtain Spousal Consent The spouse’s signature typically has to be witnessed by a plan representative or a notary public. Missing this step is an automatic rejection.
Completing the Form
Online distributions for IRAs and mutual funds are largely self-explanatory. Log in, choose “Make a withdrawal” or the equivalent, and follow the prompts. Paper forms take more attention.
Employer-Sponsored Plan Form
The Employer-Sponsored Retirement Plan Distribution form (referenced internally as FRP4RPDT) runs ten sections. Complete a separate form for each plan if you participate in more than one.4T. Rowe Price. Employer-Sponsored Retirement Plan Distribution
Section 1 collects your plan type (403(b) or Individual 401(k)), Plan ID, and participant details. Section 2 is where most people slow down. You must check exactly one qualifying reason for the distribution: termination of employment, reaching age 59½, plan termination, permanent disability, financial hardship (Individual 401(k) and profit-sharing plans only), relief under federal disaster provisions, alternate payee under a qualified domestic relations order, or beneficiary of a deceased participant.4T. Rowe Price. Employer-Sponsored Retirement Plan Distribution
The remaining sections cover the distribution amount, the receiving account or rollover destination, tax withholding, and signatures. For Individual 401(k) plans, a plan administrator must also sign in Section 10B. If you are both the participant and the plan administrator, which is common with solo 401(k)s, you sign twice.
IRA Distribution Form
The IRA distribution PDF is simpler. You identify the account, specify a dollar amount or full balance, choose a check to your address on file or an electronic transfer to your bank, and enter your tax withholding election. There is a section for direct rollovers to another IRA or employer plan. Use it if you are moving the money rather than spending it, because a direct rollover avoids the 20% withholding that applies when funds are paid directly to you from an eligible rollover distribution. For questions while completing the IRA form, T. Rowe Price’s IRA line is 800-225-5132.5T. Rowe Price. IRA Distribution
Tax Withholding Elections
The withholding section is where a form has real consequences. The wrong choice can create a surprise tax bill in April or tie up money you didn’t need withheld.
Federal Withholding
The default federal withholding rate depends on the type of distribution.
- Nonperiodic IRA distributions default to 10% of the taxable amount. You can elect any rate from 0% to 100% by completing IRS Form W-4R, which is typically attached to or incorporated into the withdrawal form.6Internal Revenue Service. Pensions and Annuity Withholding
- Eligible rollover distributions from employer-sponsored plans have a mandatory 20% withheld when paid directly to you. You cannot opt out. A direct rollover to another qualified plan or IRA is the only way around it.7Internal Revenue Service. Pensions and Annuity Withholding – Section: Eligible Rollover Distributions
- Non-eligible-rollover distributions from employer-sponsored plans default to 10% if you don’t submit a completed W-4R.4T. Rowe Price. Employer-Sponsored Retirement Plan Distribution
If your effective tax rate will be higher than the default, elect a higher percentage. Underwithholding on a large distribution is a common way people end up owing an estimated tax penalty the following spring.
State Withholding
State income tax withholding varies by where you live. Some states have no income tax and require none. Others mandate a minimum withholding percentage on retirement distributions that you cannot waive. The form has a state withholding section; fill it out based on your state’s rules, and check your state revenue department’s website if you are unsure of the current rate.
Signature and Medallion Guarantee Requirements
Every withdrawal form needs the account holder’s signature. Online submissions authenticate through your login. Paper forms require a physical signature.
Some transactions also require a Medallion Signature Guarantee, a stamp from a participating bank, credit union, or broker-dealer that verifies your identity and protects against unauthorized transfers.8Investor.gov. Medallion Signature Guarantees: Preventing the Unauthorized Transfer of Securities T. Rowe Price requires a signature guarantee when you are adding a bank account where you are not an owner, or when distribution proceeds are going to a differently owned account.9T. Rowe Price. Account Services Not every bank branch offers this service, so call ahead. A notary stamp is not the same thing and will not be accepted.
How to Submit the Form
Three delivery methods are available, and the right one depends on the account type and how quickly you need the money.
- Online. IRA distributions, mutual fund redemptions, RMDs, and QCDs can all be initiated inside your T. Rowe Price online account. This is the fastest route.1T. Rowe Price. Forms
- Mail. Send the signed paper form to the mailing address printed on the form. Use the P.O. Box for regular mail or the street address for FedEx or UPS delivery; the P.O. Box won’t accept courier packages. A trackable service gives you proof of delivery.
- Fax. T. Rowe Price accepts faxed forms for many distribution types at the number printed on the form. Keep the transmission confirmation as your receipt.
For general account questions, mutual fund customers can reach T. Rowe Price at 1-800-537-6172 and brokerage customers at 1-800-225-7720.
After You Submit
Operations reviews the form for completeness. If a signature, plan administrator approval, or Medallion Guarantee is missing, they will contact you and the clock resets. Clean submissions move faster.
When Funds Arrive
Delivery speed depends on how you asked to be paid, based on T. Rowe Price’s published timelines for processed distributions:10T. Rowe Price. Account Withdrawal Options
- Electronic transfer to a bank account, typically two to three business days after processing.
- Standard check by mail, seven to ten business days.
- Priority check, roughly three business days.
You can track status through the online account portal, and T. Rowe Price sends a confirmation by email or letter once the transaction processes.
Form 1099-R at Tax Time
Any distribution taken during a calendar year generates a Form 1099-R, which T. Rowe Price must send you by January 31 of the following year.11Internal Revenue Service. General Instructions for Certain Information Returns It reports the gross distribution, the taxable amount, federal and state tax withheld, and a distribution code in Box 7 that tells the IRS what type of withdrawal it was.12Internal Revenue Service. Instructions for Forms 1099-R and 5498 You need it to file your tax return, and multiple distributions during the year may produce multiple 1099-Rs.
If You Took Out the Wrong Amount
If you take a distribution and realize you didn’t need the money, or it went to the wrong account, you have 60 days from the date you receive it to roll the funds back into an IRA or qualified plan. A completed 60-day rollover avoids both income tax and the early withdrawal penalty on the returned amount.13Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions
Two limits matter. You generally get only one IRA-to-IRA rollover per 12-month period across all of your IRAs. A second rollover in that window is treated as a taxable distribution and may also be classified as an excess contribution subject to a 6% annual penalty if deposited into another IRA.13Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions The 60-day deadline is firm; the IRS grants waivers in narrow circumstances such as natural disasters or bank errors, not for forgetting.
To skip the rollover mechanics entirely when moving money between retirement accounts, request a direct trustee-to-trustee transfer. These are not rollovers, don’t count against the once-per-year limit, and sidestep the 20% mandatory withholding on employer plan distributions.
