What the Form Certifies
The form has a narrow purpose: it certifies duty performed as ordered for compensation and confirms that the duty was satisfactorily completed. It is not a discharge document. A DD Form 214 is issued for active duty of 90 days or more and captures a full service record. Who Uses It
The form applies to Reserve Component soldiers who completed a period of active duty under federal orders. Inactive duty periods do not go on this form. Monthly weekend drills, also called Battle Assemblies or Inactive Duty Training, are not active duty, and Form 25-R specifically references an active duty period tied to numbered orders from a headquarters.

Filling In Each Field

- FROM: your unit address, including the Personnel Accounting Symbol (PAS). It is on your orders, and your unit administrator can confirm it.
- Rank, Name, SSN: your current rank, full legal name, and Social Security Number as they appear in your personnel records.
- Number of days: the total days in the active duty period, including travel days.
- Order Number, Issuing HQ, and Date: pull these directly from the orders that authorized the duty. Do not estimate.
- Inclusive dates of duty: start and end dates of duty actually performed, in YYYYMMDD format. The form states this includes travel time.
- Soldier’s Signature and Date: your signature and the date you sign, certifying that the information is correct.
A wrong order number, mismatched dates, or an outdated unit designation can hold up pay processing. Matching the form to the orders line for line is the fastest way to avoid that.
Verifying Official and Routing
Once you sign, the form goes to a verifying official. That person must have personal knowledge or documentation supporting the fact that the duty was satisfactorily performed.1U.S. Army Reserve. The verifying official fills in their printed name, title, phone number, signature, and date. If they were not physically present during your duty, they can rely on sign-in rosters, evaluation reports, or other unit records to support the certification.
The completed form is processed through your unit’s S-1 personnel section, which handles administrative records and pay documentation. Get the original or a certified copy back for your personal file once both signatures are in place.
Penalties for a False Claim

After It Is Signed
Keep the original or a certified copy indefinitely.
