How to Fill Out and Submit the VyStar Direct Deposit Form

The VyStar direct deposit form tells your employer or benefits payer to send money electronically into your VyStar account. To complete it you need VyStar’s routing number, 263079276, and your personal account number, plus your account type (checking or savings) and a signature. Fill it out, hand it to your payroll or HR department, and expect the first deposit to land within one or two pay cycles.1VyStar Credit Union. Direct Deposit

What You Need Before You Start

Two numbers do most of the work. VyStar’s routing number, 263079276, is the same for every member and identifies the credit union within the national banking system.2VyStar Credit Union. Switching from Another Bank Your account number is unique to you.

If you have checks, your account number is printed along the bottom. If you don’t, you can find it in a few other places:1VyStar Credit Union. Direct Deposit

  • On any recent paper or electronic statement.

Many employers hand out their own authorization form instead. The information you’ll enter is the same: routing number, account number, account type, personal details. Some employers also ask you to attach a voided check. If you don’t have checks on the account, a printout of your account details from Online Banking or a recent statement showing your name and account number usually works. Ask payroll what they’ll accept before you submit.

Filling Out the Form

VyStar’s standard form follows the SF-1199a format used by federal agencies and many private employers. It has three sections.4VyStar Credit Union. Direct Deposit Sign Up Form

Section 1: Your Information

Enter your full legal name (last, first, middle initial), mailing address, and phone number. If the payment is a government benefit, include your claim or payroll ID number from your benefit check or award letter. Check “Checking” or “Savings,” then write in your VyStar account number.

The form also asks what kind of payment you’re setting up: company payroll, Social Security, VA compensation, federal salary, military pay, railroad retirement, or civil service retirement. Pick one. If you receive more than one type of payment, use a separate form for each.

If you want only part of your pay to go to VyStar and the rest somewhere else, look for an allotment section where you can specify a dollar amount. Not every employer supports partial deposits on this form; some handle splits through their payroll portal instead.

Section 2: Employer or Agency Information

Write in the name and address of the company or agency making the payment. Your payroll or HR department can confirm the exact legal name if you’re unsure.

Section 3: Financial Institution Details

On VyStar’s version of the form, this section is pre-printed with the credit union’s name, address (P.O. Box 45085, Jacksonville, FL 32232-5085), and phone number.4VyStar Credit Union. Direct Deposit Sign Up Form The routing number should read 263079276. If you’re filling out an employer’s blank form, write this in yourself and double-check every digit. A single wrong number sends your pay to the wrong institution.

Sign and date the form. Most employers accept digital signatures, but some still require ink. Ask before you submit a digitally signed copy so it doesn’t get kicked back.

Submitting the Form

Where it goes depends on who’s paying you.

Employer Payroll

Hand the completed form to HR or payroll, or upload it through your employee self-service portal if your workplace has one. Some employers want a voided check or bank letter along with the form as extra verification.

Social Security and VA Benefits

Federal law requires that Social Security, SSI, VA benefits, and virtually all other non-tax federal payments be delivered electronically.5eCFR. 31 CFR 208.3 – Payment by Electronic Funds Transfer You don’t submit VyStar’s form to the Social Security Administration. Instead, set up or change direct deposit through your my Social Security account, the Treasury’s Go Direct site at godirect.gov, by phone at 1-800-333-1795 or 1-800-772-1213 (TTY 1-800-325-0778), or at a local Social Security office.6Social Security Administration. For VA disability compensation, pension, or education benefits, update your direct deposit through your VA.gov profile using VyStar’s routing number and your account number.7Veterans Affairs. Change Your Direct Deposit Information Veterans who prefer paper can use VA Form SF-1199a, which uses the same format as VyStar’s form.

IRS Tax Refunds

Tax refunds work differently. To send your federal refund to VyStar, enter the routing number (263079276), your account number, and account type directly on your Form 1040 when you file. No separate direct deposit form is needed for a single account.8Internal Revenue Service. Form 8888 (Rev. December 2025)

If you want to split your refund across two or three accounts, attach Form 8888, Allocation of Refund. Each deposit must be at least $1, the total must equal your full refund, and the IRS limits direct deposits to three refunds per account per year. The name on each account generally must match the name on the return. Crossed-out or whited-out entries will get your deposit request rejected.8Internal Revenue Service. Form 8888 (Rev. December 2025)

After You Submit

Payroll systems need time to process the new banking instructions. Expect one or two full pay cycles before the first deposit lands. Keep any previous payment method active during the transition, and don’t close an old bank account until you’ve confirmed the deposit arrived at VyStar. Check your transaction history in the VyStar app or Online Banking on payday to verify the funds posted.

If the deposit doesn’t appear when expected, start with your payroll department. The usual causes are a transposed digit in the account number, the wrong account type, or the form simply not being processed yet. VyStar’s contact center at 800-445-6289 can confirm whether an incoming ACH transfer was received or returned.3VyStar Credit Union. Contact Us

If Something Goes Wrong With a Deposit

Regulation E, the federal rule for electronic fund transfers, caps your liability for unauthorized transactions based on how fast you report:10eCFR. 12 CFR 1005.11 – Procedures for Resolving Errors

  • Within 2 business days: liability is capped at $50 or the amount of unauthorized transfers before you notified VyStar, whichever is less.
  • After 2 business days but within 60 days: liability can rise to $500.

Once you notify VyStar of an error by phone, in person, or in writing, the credit union has 10 business days to investigate and report the results. If the investigation takes longer, VyStar can extend it to 45 days but must provisionally credit your account within the first 10 business days so you aren’t left short. If the investigation finds no error, VyStar can reverse the provisional credit after notifying you in writing.10eCFR. 12 CFR 1005.11 – Procedures for Resolving Errors
