To complete the VPay EFT enrollment form, gather your practice’s Tax ID, NPI, bank routing and account numbers, a voided check or bank letter, and a signed W-9; fill in the provider identification, banking, contact, and authorization sections exactly as those details appear on your bank, IRS, and NPPES records; and submit the whole packet together through the VPay Provider Portal, secure email, or your carrier’s designated fax line. From submission to your first live ACH deposit, plan on roughly ten business days.1UHCprovider.com. Provider Electronic Payment Options
What You Need Before You Start
Open the form only after everything below is in front of you. A missing piece or a mismatched name is what stalls most enrollments.
- Your nine-digit Tax Identification Number (TIN) or Employer Identification Number (EIN) as issued by the IRS.
- Your ten-digit National Provider Identifier (NPI). Verify the exact spelling of the registered name at the NPPES NPI Registry before you write anything down; the name on your enrollment form must match the registry.2Centers for Medicare & Medicaid Services. NPPES NPI Registry
- Your bank’s nine-digit routing transit number and your account number, taken directly from a check or bank statement.
- A voided check, or, if you don’t have checks, a letter on official bank letterhead showing the account holder’s name, the routing number, the account number, and a bank officer’s signature. The account holder’s name must match your practice’s legal business name.3Centers for Medicare & Medicaid Services. EFT Authorization Agreement
- A signed Form W-9 confirming your TIN and backup withholding status. The IRS doesn’t set an expiration date on the W-9, but VPay or the carrier may ask for one signed recently, so check the instructions in your packet.4Internal Revenue Service. Name mismatches are the single most common cause of a bounced application. If your bank account reads “John Smith MD LLC” and your NPI is registered under “Smith Medical Associates,” the enrollment will not clear verification. Fix the mismatch at its source before you submit. Starter checks and temporary checks are usually rejected as proof of account.
Where to Get the Right Form
There isn’t one universal form. VPay processes claim payments on behalf of several insurance carriers, and each carrier may issue its own branded version of the enrollment paperwork. Start at the VPay Provider Portal, where you can register an account and pull enrollment materials.6VPay Provider Portal. If you’ve already received a virtual credit card payment and want to switch that payer to ACH, the Explanation of Review (EOR) that came with the payment usually lists the VPay contact and enrollment link specific to that carrier. You can also email support@vpay.usa.com or call VPay’s Support Center at 866-919-0537 to request the correct form.
Filling Out the Form
The form itself is short. Every field needs to match a record that already exists somewhere else — at your bank, at the IRS, or at NPPES. Treat it as transcription, not composition.
Provider Identification
Enter your TIN or EIN and your NPI exactly as they’re registered. If your practice carries more than one NPI, say a group NPI and an individual provider NPI, confirm with the carrier which one is tied to your claims. The wrong NPI here routes payments to nowhere even when everything else is correct.
Banking Details
Enter the nine-digit routing transit number and your account number, and indicate whether the account is checking or savings. Check both numbers against the voided check or bank letter one character at a time. A single transposed digit will fail the pre-note test and reset your timeline by weeks.
Contact and Address
Name an administrative contact who can actually respond if VPay flags a problem during verification. Give a direct phone number and a monitored email address, not a general office line. The practice’s physical address and phone should match what the insurance carrier already has on file; a mismatch can trigger a fraud review.
Authorization Signature
An authorized representative of the practice has to sign — typically the owner, a managing partner, or an officer listed on the business’s banking documents. The signature authorizes VPay to deposit funds into the account you’ve named. Unsigned forms are rejected on receipt.
Submitting the Packet
VPay accepts enrollment through three channels:
- The VPay Provider Portal, which gives you the fastest receipt confirmation.
- Email. Because the packet contains banking details, send it through encrypted email or a secure file-sharing service rather than as a plain attachment.
- Fax. Some carriers maintain a dedicated secure fax line for enrollment documents. Confirm the correct number with your carrier’s enrollment instructions or by calling 866-919-0537.
Send the completed form, the voided check or bank letter, and the W-9 together in a single packet, whichever channel you use. Pieces submitted separately tend to sit in a queue while someone tries to match them manually.
What Happens After You Submit
Standard processing runs about ten business days, longer if documentation has problems.1UHCprovider.com. Provider Electronic Payment Options During that window VPay sends a pre-note test, a zero-dollar ACH transaction, to confirm the routing and account numbers are valid and that the account can receive electronic deposits. The pre-note is standard ACH network practice, not something unique to VPay.
If the bank rejects the pre-note, VPay will contact the administrative person you named and ask for corrected banking information before any real payment goes out. A rejection almost always points to a wrong digit in the routing or account number, or to an account that doesn’t accept electronic deposits. Fix the error, resubmit, and the ten-day clock starts over.
When verification succeeds, you’ll get a confirmation email. Your next reimbursement from that carrier should arrive as a direct deposit. Watch your first few deposits closely and compare the remittance amounts to your Explanation of Benefits statements to catch anything that doesn’t line up.
Changing Banks or Updating Account Information
If your practice switches banks or opens a new operating account, update your EFT enrollment before the old account closes. Contact VPay through the provider portal or at 866-919-0537 to request the change. You’ll provide the same documents as the original enrollment: a completed form with the new account details, a voided check or bank letter for the new account, and a signature from someone authorized on the account.
Allow five to ten business days for the change to take effect, and keep the old account open until you’ve confirmed that at least one deposit has landed in the new one. VPay runs a fresh pre-note test on the new account, so expect the same timeline as your first enrollment.
