How to Fill Out and Submit the Vanguard 401(k) Withdrawal Form

To complete the Vanguard 401(k) withdrawal form, log in to the participant portal at my.vanguardplan.com, open the withdrawals or distributions section, and work through the electronic request: pick a distribution type, set your federal and state tax withholding, enter your bank details for direct deposit, and submit. If your plan requires physical signatures or spousal consent, the portal generates a PDF you print, sign, and return. You can also call Vanguard’s retirement plan services line at 800-523-1188 to have a representative walk you through the request or mail you the correct forms.

Where to Get the Form

Vanguard’s employer-plan portal at my.vanguardplan.com is a different site from the personal investor site at investor.vanguard.com, and the withdrawal form lives only on the plan portal. Log in with your participant credentials, then look under account management for withdrawals or distributions. Many plans allow the full request to be completed online with no printing.

When a signature is required, the portal produces a PDF form you download, sign, and return. Some plans also require a separate spousal consent form. Completed paper forms can be mailed to Vanguard at P.O. Box 982902, El Paso, TX 79998, or faxed to the number listed in your plan-specific instructions. Use a trackable mail service; these forms carry your Social Security number and bank information.

What to Have Ready Before You Start

Gather these items first. Missing information is the most common reason a request stalls.

  • Your plan number, from an account statement or the portal dashboard.
  • Your Social Security number, for identity verification and tax reporting.
  • Your bank routing and account numbers for ACH direct deposit. A transposed digit sends the money back to Vanguard and adds days to the process.
  • If you’re rolling funds out, the receiving institution’s name, account number, and mailing address. Vanguard can generate a letter of acceptance if the destination is a Vanguard IRA.

Spousal Consent

If your plan is subject to the Retirement Equity Act of 1984, your spouse must sign a written consent waiver before Vanguard will process certain distributions. Federal regulations require the signature to be witnessed in the physical presence of either a notary public or an authorized plan representative. Notary fees typically run between $2 and $25 depending on the state. Handle this before you submit; a missing witnessed signature is a leading cause of kicked-back requests.

The 402(f) Rollover Notice

Before any eligible rollover distribution, your plan must give you a written explanation of your rollover options, tax consequences, and withholding rules. The notice has to arrive at least 30 days before the distribution date, though you can waive that waiting period for faster processing. It can be delivered up to 180 days in advance.

Choosing a Distribution Type

The form asks you to pick how the money leaves the account. The main options:

  • Lump-sum withdrawal: your entire vested balance, paid at once.
  • Partial withdrawal: a specific dollar amount, with the rest left invested.
  • Direct rollover: funds move straight from your Vanguard 401(k) to another qualified plan or IRA. Nothing is withheld because you never take possession.1Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions
  • Indirect rollover: funds are paid to you with 20% withheld for federal taxes. You then have 60 days to deposit the full original amount into another qualified account.

The direct rollover is almost always the better choice when you’re moving money to another retirement account. With an indirect rollover, you have to make up the withheld 20% out of your own savings to complete a full rollover, and you don’t recover that money until you file your tax return.

Tax Withholding

For any distribution paid directly to you rather than rolled over, federal law requires the plan to withhold 20% for federal income taxes.2Office of the Law Revision Counsel. 26 U.S. Code 3405 – Special Rules for Pensions, Annuities, and Certain Other Deferred Income You cannot elect a lower rate on eligible rollover distributions; 20% is the floor. You can elect a higher rate if you expect to owe more, which avoids a surprise bill in April. The form also has a line for state income tax withholding, and some states require mandatory withholding on retirement distributions.

Hardship withdrawals follow different rules. Because they are not eligible for rollover, the mandatory 20% does not apply; they’re subject to the standard 10% voluntary withholding rate unless you elect otherwise.

Payment Method and Pricing

Select ACH direct deposit to your bank account or a mailed check. Direct deposit is faster and removes the risk of a lost check. For a direct rollover, enter the receiving institution’s information so Vanguard sends the funds or makes the check payable to the new custodian.

When Vanguard processes the withdrawal, it sells the investments in your account at that day’s net asset value. Requests submitted before the New York Stock Exchange closes (typically 4:00 p.m. Eastern) get that day’s closing price; requests after the close get the next business day’s price.3Vanguard. Submitting and Tracking

Submitting online sends the request straight to Vanguard’s processing team. Paper forms go to P.O. Box 982902, El Paso, TX 79998, or to the plan-specific fax number.

Administrative review typically takes a few business days. During that window, the plan recordkeeper verifies your identity, confirms eligibility, checks for required spousal consent, and validates your banking information. Once approved, ACH transfers generally arrive within two to three business days. A mailed check takes seven to ten business days. You can monitor status through the message center on the participant portal or by calling 800-523-1188.

Are You Actually Eligible to Withdraw Right Now?

The form only succeeds if you have a triggering event under the plan and federal law. Federal law imposes a 10% additional tax on most 401(k) distributions taken before age 59½, on top of regular income tax.4Office of the Law Revision Counsel. 26 U.S. Code 72 – Annuities; Certain Proceeds of Endowment and Life Insurance Contracts The main pathways:

If none of these fit and you’re still employed with the plan sponsor, your plan may not permit the withdrawal at all, regardless of what the form allows you to select.

The 60-Day Indirect Rollover Trap

If you take a distribution check made out to you and want to avoid taxes, you have exactly 60 days from the day after you receive the payment to deposit the full amount into another eligible retirement account. The clock starts when the check arrives, not when it was mailed. Miss the deadline and the IRS treats the entire amount as a taxable distribution, with the 10% early withdrawal penalty on top if you’re under 59½.1Internal Revenue Service. The withholding compounds the risk. Vanguard withholds 20% before sending you the check.2Office of the Law Revision Counsel. 26 U.S. Code 3405 – Special Rules for Pensions, Annuities, and Certain Other Deferred Income On a $50,000 account, you receive a check for $40,000. To complete a full rollover, you have to deposit $50,000 into the new account, coming up with the $10,000 out of pocket. You recover the withheld amount as a tax refund when you file. Deposit only the $40,000 you received and the IRS treats the missing $10,000 as a taxable distribution. Choose the direct rollover on the form if you’re moving the money.
