Information You Need Before You Start
Every version of the form asks for the same three things: your U.S. Bank routing number, your account number, and whether the account is checking or savings.
Routing Number
The routing number is a nine-digit code that identifies U.S. Bank within the banking system, and it depends on the state where you opened your account. Confirm the number through the app, a check, or a statement. Don’t guess based on a general web search. The wrong routing number is the most common reason a direct deposit setup fails, and a failed transaction can push your pay back by an entire cycle.
Account Number

Account Type
The form asks whether deposits should go to checking or savings. The ACH system codes those two transaction types differently, and picking the wrong one can cause the deposit to bounce back to your employer even when the account and routing numbers are correct.
How to Fill Out the Form
The form is short. The newer consumer version asks for five fields: your name, bank name (U.S. Bank), account type, account number, and routing number.1U.S. Bank. If you use the older version, its instructions tell you to attach a U.S. Bank check with “VOID” written across the front. Don’t sign the voided check. Many U.S. Bank accounts don’t come with paper checks. If yours doesn’t, use the newer consumer version, which works without a voided check, or use the prefilled PDF generated from online banking. Both already contain verified account details.
Where to Send It and When Deposits Start
The completed form goes to your employer’s human resources or payroll department, not to U.S. Bank. Some employers accept it through a digital portal or email; others want a physical copy. Allow one to two pay cycles for the change to take effect.1U.S. Bank. Don’t close a previous bank account until you’ve confirmed at least one deposit has arrived at U.S. Bank. Keeping both accounts open for 30 to 60 days gives you time to catch any automatic payments or deposits that haven’t switched over yet.
Once the first deposit is expected, log in to U.S. Bank online banking or the mobile app to confirm it posted. If a deposit fails because of incorrect account details, the ACH network typically returns the funds to your employer within two banking days, and your employer then has to reissue the payment on the next pay cycle.
Splitting a Paycheck Between Accounts
If you want part of your paycheck going to U.S. Bank and part to a different bank, you handle that through your employer’s payroll system, not through the U.S. Bank authorization form. Most payroll systems let you designate a primary account for the remainder plus one or more secondary accounts that get a fixed dollar amount or percentage. Not every employer supports split deposits, so ask payroll before filling out multiple forms. Federal Benefits Use a Different Process
The U.S. Bank authorization form is built for employer payroll. If you want Social Security, SSI, VA, railroad retirement, or federal civil service pension payments to land in your U.S. Bank account, that goes through the federal government, not through your employer. After Setup: Watching the First Deposits
Check your account regularly during the first month of direct deposit. Once the setup is live, unauthorized transactions are covered by the Electronic Fund Transfer Act, but your liability depends on how quickly you report a problem. Notify U.S. Bank within two business days of discovering an unauthorized transfer and your maximum liability is $50. Report between two days and 60 days after your statement arrives and the cap rises to $500. For a reported error such as a missing deposit or an unauthorized withdrawal, U.S. Bank has 10 business days to investigate, with the option to extend to 45 days if it provisionally credits your account within that first 10-day window.
