Confirm You Actually Qualify
Before you spend time on the form, run through the requirements. You need an active Spectrum Internet subscription, since the mobile buyout is only offered to Internet customers.1Spectrum. Filling Out and Submitting the Form
The form lives on Spectrum’s website, on the contract buyout or mobile switch page. It asks for your Spectrum account details, the phone numbers you ported, and the device balance amounts from your old carrier’s bill. Enter each balance line by line, matching each figure to the ported number it belongs to.
Upload a clear, legible copy of the bill. Include every page that shows relevant balance information. Blurry scans and cut-off pages get rejected. After you submit, save the electronic confirmation and tracking reference you receive. That confirmation is your proof of on-time filing if anything goes wrong later.

What the Reimbursement Actually Covers
The buyout pays only the remaining device installment balance on your ported lines. Each ported line earns up to $500, and that credit pools across the switched lines. Several things are excluded. Early termination fees, interest, leasing costs, and any fees beyond the raw device balance do not qualify. Tablets and wearables like smartwatches are excluded even if they’re on an installment plan. Leased devices don’t qualify, and neither do phones that are already paid off.1Spectrum. Tracking Your Claim and Fixing Denials

If a claim is denied, the usual causes are a ported phone number that doesn’t match the number on the installment agreement, a bill that doesn’t clearly show the remaining device balance, or a submission that fell outside the 30-day window. Fix the first two before resubmitting; the third one you can’t fix.
How You Get Paid
Approved claims are paid on a Visa prepaid reward card issued by Pathward, N.A. It works anywhere Visa debit cards are accepted in the United States: online, in-store, and through digital wallets. The 12-Month Commitment That Can Reverse the Payment
If you cancel any of your qualifying lines within 12 months of receiving the buyout, you owe the entire buyout amount back, not just the share for the canceled line.1Spectrum.
