What the Form Asks For

- The specific exam title (for example, “Real Estate Salesperson” or “Insurance Producer”) and the licensing program it falls under.

- A description of your condition and how it limits you in a timed testing environment.
- The specific accommodation the provider recommends, such as extended time, a reader, or a separate room.
- The provider’s name, title, and phone number.
- An original signature on professional letterhead.
Strong letters draw a straight line from diagnosis to functional limitation to requested accommodation, and include clinical test scores or diagnostic results where available. Weak letters name a condition and jump to a request without explaining the connection.
There is no universal federal rule on how recent your evaluation must be. Individual licensing programs can set their own recency standards, so check your Candidate Information Bulletin for any program-specific requirement before you gather documents.
Accommodations You Can Request

- Extended testing time, often 50% more than the standard allotment.
- Extra or extended breaks during the session.
- A separate room within the test center, away from other candidates.
- A reader or a sign language interpreter.

When multiple time-related accommodations are approved together, the combined extra time generally caps at 50% of the original time limit. Ask for what your documentation supports, not more; requesting adjustments your provider did not recommend can slow the review or produce a partial denial. ESL Accommodations

Scan or photograph documents at high enough resolution that signatures and clinical details are legible. Label each attachment clearly; “Dr_Smith_Letter_ADHD.pdf” is far better than “scan001.pdf.” If you fax, call to confirm receipt after a couple of days. The online portal sends an email confirmation.
What Happens After You Submit
Processing times vary by program. There is no penalty for filing well in advance.
PSI notifies you by email, specifying which accommodations were approved or, if not, why. If reviewers need more documentation, they will contact you. Every back-and-forth adds days, which is why vague first submissions are costly.
Do Not Schedule Before Approval
Do not book an exam date until you have the approval letter. PSI’s standard scheduling system cannot add accommodations to an existing booking, so sitting for a test before approval means testing without your adjustments. If You’re Denied or Need to Change the Plan
A denial letter should explain the reason. The most common problems are documentation that names a condition without connecting it to a testing limitation, and provider credentials that don’t match the diagnosis. You can resubmit with stronger evidence that addresses the specific deficiency PSI identified. Sending the same materials again, or dropping to a lesser accommodation without new documentation, rarely changes the outcome.

Cost and the Law Behind the Process
