What to Have Ready Before You Open the Form
A missing field or document sends the request back and restarts the clock. Pull these together first:
- Your contract number, which Prudential labels “account number” on the form. It appears on any statement or letter from Prudential.
- Social Security number or EIN. Joint owners each list theirs.

- Your federal and state tax withholding choices, decided in advance.
Trusts and Powers of Attorney
If a trust owns the annuity, every trustee signs with a “Trustee” designation after the name (for example, “Jane Smith, Trustee, under the Trust Agreement dated 01/15/2020”), and when there is more than one trustee, one of them must also submit a certification confirming that all trustees have signed.2Prudential. Which Form to Use and Where to Get It
Prudential uses separate forms for one-time and recurring withdrawals. A single lump sum uses the Annuity One-Time Withdrawal Form. Recurring payments use the Annuity Automated Withdrawal Form, which lets you set a start date, frequency (monthly, quarterly, or annually), and choose between a fixed dollar amount or a calculated required minimum distribution.

Filling Out the Form Section by Section
The Annuity Automated Withdrawal Form has nine sections. Not every section applies to every situation, but here is what each one asks and where errors slow things down.
- Section 1, owner information: your name, account number, phone, and email. Verify the account number against a recent statement.
- Section 2, program type: whether you are starting a new withdrawal program, updating an existing one, terminating one, or waiving out of an RMD program. On an update, you must re-elect your tax withholding; Prudential does not carry the prior choice forward.1Prudential. Annuity Automated Withdrawal Form
- Section 3, start date and frequency: the date you enter is the processing date, or the next business day if it lands on a weekend or holiday. For RMD programs, a blank or invalid date defaults to December 10. For systematic withdrawals, blank means the date Prudential receives the form.1Prudential. Annuity Automated Withdrawal Form
- Section 4, withdrawal amount: a dollar figure, or, for RMDs, a calculation based on IRS life expectancy tables.
- Section 5, investment options: if your contract has no living benefit rider, you can pick which subaccounts the withdrawal comes from. Skip this if a living benefit is attached, because Prudential applies its own allocation rules.
- Section 6, disability exception: check this if you are under 59½ and qualify for the disability exception to the 10% IRS penalty.
- Section 7, tax withholding: covered in the next section.
- Section 8, payment instructions: electronic funds transfer or mailed check. EFT requires your routing and account numbers.
- Section 9, signature: sign and date. Without a valid signature, the request will not process.
Tax Withholding on Section 7
Leave Section 7 blank and Prudential withholds 10% for federal income tax plus any mandatory state tax.1Prudential. State rules vary. Some states have no income tax and require nothing. Others impose mandatory withholding: Michigan, for example, withholds 4.25% from pension and retirement benefit payments unless the distribution is not taxable or you specifically opt out.1Prudential. Charges That Reduce What You Actually Receive
Before entering an amount in Section 4, find out whether your contract is still inside its surrender charge period. Prudential annuities offer surrender periods such as five or seven years, and withdrawing during that window triggers a charge that reduces the payout.6Prudential Financial. Most Prudential annuities allow a free withdrawal of up to 10% of account value each year after the first contract year. During the first year, some contracts permit 10% of total purchase payments penalty-free and others allow none.7Prudential Financial. Some contracts also apply a market value adjustment, which can raise or lower your payout based on interest rate movement since purchase. Rising rates can help; falling rates can hurt. The 10% IRS Penalty on Early Withdrawals
Separate from any Prudential surrender charge, the IRS adds a 10% penalty on the taxable portion of a distribution from a non-qualified annuity before age 59½.8Office of the Law Revision Counsel. 26 U.S. Code 72 – Annuities; Certain Proceeds of Endowment and Life Insurance Contracts That is on top of ordinary income tax, so an early withdrawal can be hit with both the IRS penalty and a Prudential surrender charge.
Common exceptions include reaching age 59½, death of the contract holder (distributions to beneficiaries), total and permanent disability, a series of substantially equal periodic payments under IRC 72(t), and contracts structured as immediate annuities from the start. Living Benefit Rider Warning
If your annuity carries a guaranteed minimum income benefit or similar living benefit rider, the size of the withdrawal relative to a contract limit determines how much it damages the guaranteed values. For many Prudential GMIB versions, withdrawals up to a 5% annual threshold reduce the protected value dollar for dollar; a $5,000 withdrawal lowers the protected value by $5,000. Go over the 5% limit and the reduction becomes proportional, which can cut the protected value by a much larger percentage than the cash you actually took.10Prudential Financial. The proportional reduction is the trap. If the account value has dropped but the protected value is still high (the whole point of the guarantee), one oversized withdrawal can permanently lower that guaranteed floor. Before you finalize Section 4, call Prudential and ask for a benefit impact illustration on your proposed amount.
Spousal Consent (ERISA Plans Only)
If the annuity sits inside an employer-sponsored retirement plan subject to ERISA, such as a pension or profit-sharing plan, federal law may require your spouse to consent in writing to the withdrawal, with the consent witnessed by a plan representative or notary.11Office of the Law Revision Counsel. 26 U.S. Code 417 – Definitions and Special Rules for Purposes of Minimum Survivor Annuity Requirements This does not apply to individual annuities or IRAs, so most Prudential annuity owners will not encounter it.
How to Submit the Form
You have three options: the online portal, fax, or mail. The portal is fastest and gives immediate confirmation of receipt. Fax and mail addresses vary by contract type, so use the address printed on the form you downloaded rather than an address you have used before for something else. Mail adds transit time on top of processing. Keep a signed copy of the form and any supporting documents for your records whichever route you pick.
Processing Time and What Happens After You Submit
Prudential processes the request as of the “good order date,” meaning the day it receives a complete, correctly filled form with all required signatures and supporting documents. From that day, plan on three to five business days for the funds to arrive.1Prudential. Annuity Automated Withdrawal Form A confirmation statement follows once processing is done.
Distributions handled through a different Prudential form, such as the beneficiary distribution form, break the timeline into steps: one to three business days for Prudential to process, then one to three business days for EFT or three to five for a mailed check.12Prudential. Expect a phone call from Prudential to the number on file to verify the transaction, especially for larger amounts. It is a fraud check. Missing that call delays the withdrawal, so keep your contact information current and pick up when they call.
Prudential will report the distribution on IRS Form 1099-R, sent by January 31 of the year after the withdrawal, with distribution codes indicating whether it was a normal distribution, an early one subject to the 10% penalty, or a coded exception.13Internal Revenue Service.
