The PPL direct deposit form, officially called the Payment Method Form, is what Public Partnerships LLC uses to send your caregiver pay electronically instead of by paper check. Fill it out with your banking details, sign it, and submit it to the address, fax, or email listed on your state program’s version of the form. Expect one to two pay periods before electronic payments begin; PPL pays you by mailed paper check in the meantime.1Public Partnerships LLC. Payment Method Form
The Four Payment Options
The form asks you to pick one payment method:
- Direct deposit to a checking or savings account at your bank.
- Direct deposit to a third-party money app like Venmo or Chime, which generate their own routing and account numbers.
- Deposit to an existing prepaid or reloadable debit card, using the card’s routing and account numbers (not the number on the front of the card).
- A new debit card issued by PPL, which requires no banking details but does require a correct mailing address on file.
If you provide care for more than one consumer through PPL, all of your pay routes to the same payment method. You don’t submit a separate form for each consumer.1Public Partnerships LLC. Payment Method Form
What to Gather First
Have these ready before you start writing:
- Your PPL ID number. PPL assigns this when you enroll as a Personal Assistant, and it appears on your enrollment paperwork, paystubs, and correspondence. If you can’t find it, call PPL’s general line at 855-243-8775 or the number on your program’s materials.2Public Partnerships LLC. Contact Us
- Your bank’s nine-digit routing number. On a personal check, it’s the leftmost set of numbers along the bottom. Your bank’s app, website, or branch can also confirm it.
- Your account number, which sits to the right of the routing number on a check. For a money app, look under “direct deposit” or “account details” in the app’s settings.
Some state programs also require a voided check or a bank letter confirming the account, attached to the form.3Public Partnerships LLC. Public Partnerships LLC Direct Deposit Form Check the instructions printed at the bottom of your specific state’s form. If a bank letter is required and you don’t have checks, ask your bank branch for one on letterhead showing your full name, account number, and routing number.
Filling Out the Form
The form is usually a single page, but accuracy matters. PPL warns that a paycheck deposited to the wrong account may be difficult to recover, and the company can withhold future payments until the error is resolved.1Public Partnerships LLC. Payment Method Form
Your Name and PPL ID
Print your first and last name exactly as they appear on your enrollment documents, then enter your PPL ID. Payroll uses that ID to match the form to your account, and a missing or transposed digit is one of the fastest ways to stall the process.
Payment Method and Account Details
Check the box for the option you want. For a bank or money-app deposit, mark the account type (checking, savings, or third-party app) and fill in the bank or app name, routing number, and account number, writing each digit clearly in its own space. For an existing debit card, use the routing and account numbers tied to the card. For a new PPL debit card, checking the box is all you need to do.1Public Partnerships LLC. Payment Method Form
Paystub Preference
There’s an opt-out checkbox for paper paystubs. Check it and you’ll view stubs in PPL’s Time4Care or PPL@Home app. Leave it blank and paper copies keep coming by mail.
Signature
Sign, date, and print your name below the signature. An unsigned form gets returned, which adds at least another pay cycle to setup.
Where to Send It
Submission channels vary by state, and the correct addresses, fax numbers, and email addresses appear at the bottom of your program’s form. As an example, caregivers in New York’s CDPAP program can submit by:
- Mail: 17 Plaza Dr., Latham, NY 12110
- Fax: 1-833-951-0828
- Email: nycdpap@pplfirst.com
If you’re in another state, don’t use the New York contact information; use your own form or your state program’s resource page. Sending the form to the wrong office is a common cause of weeks-long delays. When faxing, save the transmission confirmation. When emailing, confirm the scan or photo is legible, since blurry images of routing numbers get bounced back.
How Long Before Direct Deposit Starts
PPL runs a verification step called a prenote, a zero-dollar test transaction that confirms your routing and account numbers work and the account accepts electronic deposits.4Public Partnerships LLC. NY CDPAP Resources and Payroll Information Guide
Plan on one to two pay periods before your new payment method goes live. During that window, PPL mails a paper check to the address on file.1Public Partnerships LLC. Participant Direct Deposit Form You won’t miss a paycheck; the paper check covers you until electronic payments take over.
When a Deposit Fails
If a bank rejects a deposit because the account is closed, the account number is invalid, or the routing number doesn’t match the bank, the funds return to PPL through the ACH network, typically within two banking days. PPL reissues the payment as a paper check. To restart electronic payments, submit a corrected Payment Method Form with the right details. Repeated failures caused by incorrect information can lead PPL to withhold future payments until any overpayment or misdirected deposit is resolved.1Public Partnerships LLC. Payment Method Form
Changing Banks Later
Switching banks, closing an account, or moving to a different payment type means submitting a new Payment Method Form. The prenote runs again, so expect another one to two pay periods of paper checks. Don’t close your old account until you’ve confirmed deposits are arriving in the new one. Closing early can strand a paycheck and trigger the withholding described above.
Timesheets Still Have to Be On Time
Direct deposit only helps if your timesheet meets its deadline. Payment frequency varies by state. In New York’s CDPAP program, Personal Assistants are paid weekly on Thursdays, with timesheets due by midnight on the Saturday that ends each pay period.6Public Partnerships LLC. NY CDPAP Payment Schedule PPL cannot guarantee on-time payment for late timesheets, so a missed deadline can push your deposit back a full pay cycle even when direct deposit is fully active. Other programs pay biweekly or semi-monthly; your program’s payroll calendar shows every deadline and posting date.
A Tax Note for Live-In Caregivers
Wages paid through PPL are generally subject to federal income tax and FICA withholding, and PPL handles the withholding and reporting as the fiscal intermediary. For 2026, Social Security and Medicare taxes apply once a household employee earns $3,000 or more in cash wages during the year.7Internal Revenue Service. Publication 926 (2026), Household Employer’s Tax Guide
One exception is worth knowing about because it changes what shows up on your W-2. Under IRS Notice 2014-7, Medicaid waiver payments made to a caregiver who lives in the same home as the person receiving care can be excluded from gross income as “difficulty of care” payments under Section 131 of the Internal Revenue Code. The care recipient’s home must also be your home, the place where you sleep, eat, and carry out daily life. If you keep a separate residence and travel to provide care, the exclusion doesn’t apply.8Internal Revenue Service. Certain Medicaid Waiver Payments May Be Excludable From Income If you qualify and PPL issued a W-2 that includes these payments, contact PPL to request a corrected W-2C. The exclusion does not cover vacation pay or private payments the care recipient makes to you from personal funds.
