What You Can Change With the Form

By Email to Your Maintaining Branch

FATCA Section for U.S. Persons

PNB’s customer information forms include a FATCA declaration. If you are a U.S. citizen, permanent resident, or otherwise qualify as a “U.S. Person” under the Foreign Account Tax Compliance Act, you must complete a separate Consent and Authorization form along with IRS Form W-9.
