How to Fill Out and Submit the OSAP Disability Verification Form

The OSAP Disability Verification Form is a two-part document you complete once to have your disability recognized by Ontario’s Ministry of Colleges and Universities: you fill in Section A with your personal details and consent, a qualified health care professional completes Section B describing your functional limitations, and you submit the signed form through your OSAP account or your school’s financial aid office before the deadline that matches your application type.1Ontario Student Assistance Program. OSAP Disability Verification Form Once approved, your verified status stays on your OSAP account and opens the door to non-repayable grants and a reduced course load option for future study periods.

What to Gather Before You Start

A few things save time later. You will need your Social Insurance Number, the name and address of your post-secondary school, and access to a health care professional whose scope of practice includes diagnosing. A family doctor, psychiatrist, or medical specialist can handle most physical and mental health conditions. For a learning disability or ADHD, the form asks whether a psychoeducational assessment has been performed by a registered psychologist, psychological associate, or neuropsychologist and whether a learning disability was confirmed.1Ontario Student Assistance Program. If you need a psychoeducational assessment and do not already have one, plan ahead. Private assessments in Ontario typically cost several thousand dollars, and wait times for publicly funded options can stretch for months. Some colleges and universities offer assessment referrals through their accessibility services office, so check with your school before paying out of pocket. If you already have an assessment on file, bring a copy to the appointment so your professional can reference it when completing the form.

How to Fill Out Section A

Section A is your portion. You enter your first name, last name, and Social Insurance Number, then sign a consent statement authorizing the health care professional who completes Section B to share personal health information with the ministry and your post-secondary school.1Ontario Student Assistance Program. OSAP Disability Verification Form A second consent clause authorizes the ministry and your school to contact the professional if anything on the form is unclear or illegible. Both consents are required. Leave either unsigned and the form will not be processed.

Read the consent before you sign. Knowingly providing false information on any OSAP form is an offence that can result in a fine of up to $25,000 and up to one year in prison, in addition to losing access to future OSAP assistance.2Ministry of Colleges and Universities. 2021-22 OSAP Application for Part-Time Students – Section: Application Agreement

How the Health Care Professional Completes Section B

Section B is filled out by your health care professional and has six parts. Every part must be completed. If any questions or fields are left blank, you will not be considered to have a disability for OSAP purposes.1Ontario Student Assistance Program. OSAP Disability Verification Form Here is what each part asks for:

  • Part 1 covers professional information. The professional selects their category (physician, psychologist, audiologist, and so on), provides their Canadian provincial or territorial licence number and address, and places their office stamp on the form. If they do not have an office stamp, they must sign and attach their official letterhead.
  • Part 2 asks two yes-or-no questions: whether you have a permanent disability and whether you have a persistent or prolonged disability.
  • Part 3 records the nature of the disability. The professional checks applicable disability types and, for learning disabilities, indicates whether a psychoeducational assessment was performed, whether a learning disability was confirmed, and the date of the most recent assessment.
  • Part 4 covers mobility, movement, and sensory impacts, with checkboxes and a space to describe them in the professional’s own words.
  • Part 5 uses the same format for cognitive and behavioural impacts.
  • Part 6 is the declaration and signature, in which the professional certifies that the information is accurate and that you experience the functional limitations indicated on the form.

The most common reason forms come back is vague language in the description fields. The form’s own instructions warn professionals to avoid terms like “suggests” or “is indicative of” and instead provide clear, direct statements about your functional limitations and educational barriers.1Ontario Student Assistance Program. Mentioning this before the appointment can save you a return visit.

Permanent vs. Persistent or Prolonged

Part 2 asks about two categories, and the difference is duration, not benefit. A permanent disability is an impairment expected to remain with you for your expected life. A persistent or prolonged disability involves the same types of impairments and functional restriction but has lasted or is expected to last at least 12 months without necessarily being lifelong.3Ontario Student Assistance Program. OSAP Disability Verification Form Both categories qualify you for the same disability-related grants and the reduced course load option.

How to Submit the Form

Once Section B is signed, you have two submission routes. The fastest is uploading a scan: log into the OSAP website, open your OSAP or Ontario Learn and Stay Grant application, and use the “Print or upload documents” feature.1Ontario Student Assistance Program. OSAP Disability Verification Form Make sure every page of the scan is legible and all edges are visible. A blurry stamp or cut-off signature is a common reason for rejection. You can also drop the form off at your school’s financial aid office or mail it in, though mailing adds transit time you may not have.

Save a high-quality scan or photocopy of the signed form before you submit. If the original is lost or flagged as illegible, having a backup lets you resubmit the same day rather than booking another appointment.

Deadlines

Deadlines depend on the type of OSAP application you filed:

  • For full-time, part-time, or Ontario Learn and Stay Grant applicants, the completed form must reach your financial aid office no later than 40 days before the end of your study period.4Ontario Student Assistance Program. OSAP Disability Verification Form
  • For micro-credential applicants, the form must be received no later than 5 days after the end of your study period.4Ontario Student Assistance Program. OSAP Disability Verification Form

If you also plan to apply for the Bursary for Students with Disabilities or the Canada Student Grant for Services and Equipment, that separate application has a tighter deadline of 60 days before the end of your study period.4Ontario Student Assistance Program. After You Submit

Log into your OSAP account and check the Message Centre regularly. The ministry sends notifications through the portal when it updates your disability status or needs more information. No official processing timeline is published, so checking weekly is the practical move. If your form is approved, your OSAP profile will show your verified disability status and your funding summary will update to reflect any new grants.

The form normally only needs to be completed once.1Ontario Student Assistance Program. OSAP Disability Verification Form Once verified, your disability status stays on your OSAP account for future applications. The ministry may request a new form if your circumstances change or if it has reason to review your file, but for most students this is a one-time process.

What Verification Unlocks

Completing the form is the gateway to several non-repayable supports. The Canada Student Grant for Students with Disabilities provides up to $2,800 per academic year (August 1 to July 31), pending government approval through the end of the 2026-2027 school year, and no separate application is needed once your disability status is verified and you submit your annual OSAP application.5Government of Canada. Canada Student Grant for Students with Disabilities

The Ontario Bursary for Students with Disabilities covers disability-related services and equipment needed for your studies, such as note-takers, tutors, or assistive technology. The maximum is $2,000 per academic year, with individual items subject to their own caps based on your specific functional limitations.6Ontario Student Assistance Program. Canada Student Grant for Services and Equipment – Students with Disabilities The Canada Student Grant for Services and Equipment works alongside the bursary to cover additional eligible costs. Unlike the base disability grant, these require a separate application completed with your school’s office for students with disabilities or accessibility services office.7Ontario Student Assistance Program. Canada Student Grant for Services and Equipment – Students with Disabilities

Verification also unlocks the reduced course load option. Students with a verified permanent or persistent disability can be considered full-time for OSAP purposes while taking as little as 40 percent of a full course load, compared with the standard 60 percent threshold.8Ontario Student Assistance Program. Full Course Load Full-time status determines your eligibility for most grants and affects your loan repayment schedule, so this provision lets you study at a sustainable pace without losing financial aid.
