How to Fill Out and Submit the Optimum Notice of Dispute Form

The Optimum Notice of Dispute form is the document you submit when customer service can’t resolve a billing or service problem and you want to trigger the company’s formal dispute process. Download the PDF from Optimum’s website, complete the sections describing your dispute and the relief you want, sign it, and mail it by USPS Certified Mail to Optimum Shared Services in Bethpage, New York. Receipt of the form starts a 60-day informal resolution period during which Optimum is expected to negotiate with you before either side can move to arbitration.

Where To Download the Form

The form lives on Optimum’s own site at www.optimum.net/NoticeOfDispute, the URL referenced in the company’s customer service agreement.1Optimum. Optimum Mobile Customer Service Agreement You can also reach it through Optimum’s General Terms and Conditions of Service page, in the arbitration section. The most recent publicly available PDF is dated October 1, 2021.2Optimum. Optimum Notice of Dispute Form

Use the official form. A freeform letter risks being routed to general customer service rather than the dispute resolution team, which is not the same queue.

What To Have Ready Before You Start

Pull these items together before you sit down with the PDF:

  • The account holder’s full name exactly as it appears on your Optimum bill, not a nickname or an authorized user.
  • Your 14-digit account number, which appears at the top of the bill.3Optimum. Ways to Pay Your Optimum Bill
  • The physical service address.
  • An email address and phone number you actually check, since these are what the disputes team will use to reach you.
  • Supporting documents: billing statements, screenshots of outage reports, chat transcripts, or anything else that backs up your claim.

If you’ve hired an attorney to handle the dispute, the form also requires a signed statement authorizing Optimum to share your confidential account records with that attorney.1Optimum. Optimum Mobile Customer Service Agreement

Filling Out the Form

Describing Your Dispute

The form asks you to “briefly describe the nature and basis of your claims or dispute (including where applicable specific dates).”2Optimum. Optimum Notice of Dispute Form Stick to facts and dates. A useful description reads something like: “On March 3, 2026, my internet service went down and remained out until March 10. I called customer service on March 4 (reference number 12345678) and was told a credit would be applied. My April bill showed no credit and included a $15 late fee for the previous month, which I had paid on time on February 28.”

Skip vague complaints like “service has been terrible for months.” If your problem covers a long stretch, pick the most concrete incidents and list them with dates. Attach supporting documents behind the form.

Stating the Relief You Want

The second narrative section asks you to describe the specific relief you want from Optimum.2Optimum. Optimum Notice of Dispute Form Be specific with a number. “I want a $75 credit for seven days of internet outage and removal of the $15 late fee” is far more useful than “I want fair compensation.” If you’re disputing a rate increase, state the monthly rate you believe you’re owed and the period it should apply to. Concrete requests give the disputes team something to approve, counter, or explain.

Signing the Form

The form requires your physical or electronic signature. An unsigned form is technically incomplete under the terms of service.1Optimum. Optimum Mobile Customer Service Agreement

Where To Mail the Form

The form must be sent by certified U.S. mail to:

Optimum Shared Services
1111 Stewart Avenue
Bethpage, NY 11714
Attn: Customer Disputes2Optimum. Optimum Notice of Dispute Form

Send it USPS Certified Mail with Return Receipt Requested. The return receipt proves the date Optimum received your notice, and that date starts the resolution clock. Photocopy the completed form and all attachments before mailing.

There is no official email address or online portal for the Notice of Dispute. The form itself specifies certified U.S. mail, and the publicly available terms of service offer no digital alternative for this document. Don’t confuse this with the noarbitration@alticeusa.com address, which is only for opting out of arbitration, not for filing disputes.4Optimum. General Terms and Conditions of Service (Residential)

What Happens During the 60-Day Resolution Period

Once Optimum receives your form, a 60-day informal resolution window begins. Both sides are expected to negotiate in good faith to settle the dispute without arbitration.2Optimum. Optimum Notice of Dispute Form A representative from the customer disputes group will typically contact you by phone or email to discuss your claim and make a settlement offer.

You can accept, reject, or counter any offer. Nothing obligates you to agree to a resolution you consider inadequate. “Good faith” gives both sides a structured chance to work it out; it doesn’t force you into whatever the company proposes.

If you signed up for Optimum before October 2021, your customer agreement may specify a 30-day resolution period instead of 60 days.5Optimum. Optimum Notice of Dispute Form Check the version of the terms that applies to your account.

If the Resolution Period Ends Without an Agreement

Three paths are available once the window closes without a settlement.

Arbitration Through the AAA

The most direct next step under the customer agreement is filing a Demand for Arbitration with the American Arbitration Association. You cannot file this demand until the full resolution period has elapsed, and an early filing may be rejected as premature.2Optimum. Optimum Notice of Dispute Form The AAA applies its Consumer Arbitration Rules; consumer filing fees are published at adr.org.

Small Claims Court

Optimum’s terms preserve your right to file in small claims court as an alternative to arbitration, provided the court in your jurisdiction has authority over the claim.4Optimum. General Terms and Conditions of Service (Residential) For a billing dispute involving a few hundred dollars, small claims is often faster and less formal than AAA arbitration. Filing fees vary by jurisdiction. The pre-arbitration waiting requirement applies to arbitration specifically, not to small claims filings.

An FCC Complaint

You can file a complaint with the Federal Communications Commission through its Consumer Complaint Center at consumercomplaints.fcc.gov.6Federal Communications Commission. FCC Complaints The FCC accepts complaints about internet and TV service billing, availability, and speed. An FCC complaint doesn’t replace arbitration, but it creates a federal record and often prompts a faster response. Optimum’s terms acknowledge that customers may bring issues to federal, state, and local agencies regardless of the arbitration agreement.4Optimum. A Note for Brand-New Customers

If you opened your Optimum account within the last 30 days, you can opt out of the arbitration agreement entirely, either by emailing noarbitration@alticeusa.com or by mailing a written opt-out notice to Altice Shared Services, 1111 Stewart Avenue, Bethpage, NY 11714, Attn: Arbitration.4Optimum.
