Get the Right Form for Your Product

The life insurance withdrawal form and the annuity withdrawal form are two different documents that go to two different processing centers, so start by matching the form to your contract.

Annuity holders access theirs through the NYL Annuities site. You can also call 1-800-225-5695 to have a copy mailed to you, or request one through your registered agent.

What to Have Ready Before You Start

  • Your policy number, from the original contract, annual statements, or your online dashboard.

  • Banking details if you want electronic delivery: your bank’s nine-digit routing number and your account number. One extra step applies to larger withdrawals: if you are requesting $50,000 or more, your signature must be notarized or carry a Medallion Signature Guarantee. Filling Out the Five Sections

Section 1: Policy Information

Enter the policy number and the insured person’s name exactly as they appear on the contract. A name that doesn’t match what New York Life has on file is one of the most common reasons a request gets sent back.

Section 2: Withdrawal Request

Write the dollar amount you want, or check the box for the maximum available. If you hold a variable life insurance policy, this section also asks how to allocate the withdrawal: from specific investment divisions, from the fixed account, or pro rata across all divisions. Whole life and fixed annuity contracts skip the allocation step.

Section 3: Tax Withholding Election

Section 4: Delivery Instructions

Pick one of three delivery methods:

  • A check mailed to the address on file. Nothing extra to fill in.
  • Electronic funds transfer (EFT) into your bank account. If EFT isn’t already on file, provide your routing and account numbers and attach a voided check.
  • Wire transfer for faster delivery, which may carry a fee. You’ll need the bank name, ABA routing number, account number, and account type.

Section 5: Signature

Sign and date the form. A joint owner, if there is one, must also sign. For any withdrawal of $50,000 or more, add the notarization or Medallion Signature Guarantee here. Make sure your signature reasonably matches what’s on file; an obvious mismatch will delay processing.

Processing and Delivery Times

For annuity contracts, New York Life processes the request and releases funds on the next business day after receiving the form. Before You Sign: A Few Things Worth Confirming

Two issues sit outside the form itself but affect whether the amount you’re writing in is the amount you actually want.

The first is tax treatment. On most whole life and universal life policies, withdrawals come out of your premium basis first and are tax-free up to what you’ve paid in. If your policy is classified as a modified endowment contract (MEC), the order flips: gains come out first and are taxed as ordinary income, with a 10% additional tax on withdrawals before age 59½ unless an exception applies.9Internal Revenue Service. A partial withdrawal permanently reduces cash value and death benefit, and on a whole life policy the death benefit reduction can be larger than the cash you take, depending on whether the withdrawal pulls from paid-up additions or the base policy.
