How to Fill Out and Submit the NAVPERS 1070/602 Dependency Application

The NAVPERS 1070/602 Dependency Application is the Navy form you use to claim dependents for pay and allowance purposes, and you complete it through your self-service Electronic Service Record at nsips.cloud.navy.mil or on paper through your Command Pay and Personnel Administrator.¹ Personnel offices still call it the “Page 2.” It supports dependency allowances, household goods moves, travel claim processing, and the secondary dependency packages sent to the Defense Finance and Accounting Service.²

One boundary to set before you start: the 1070/602 no longer handles emergency data or death gratuity beneficiary designations. Those live on the DD Form 93 now. If you are trying to update who receives the death gratuity, who gets notified if you are injured, or your missing-status allotments, that is the DD Form 93, not the 1070/602.² Some commands still use a legacy version that combines both on one sheet, which is why the two forms get mixed up. Most sailors update them together after a marriage, divorce, or birth, and that is a reasonable habit.

Documents to Gather Before You Start

Missing a single document will stall the update, and while it sits unfinished, your allowances may not reflect your actual family situation. Pull everything together first.

Marriage certificate — an original or certified copy from the county or jurisdiction where the marriage took place.
Birth certificates for each dependent child. For a child born overseas to a U.S. citizen parent, apply for a Consular Report of Birth Abroad through the nearest U.S. Embassy or Consulate.³
Divorce decree, if a previous marriage ended. It must be the final decree signed by a judge. The Navy expects sailors to comply with the terms of court orders and decrees from courts of competent jurisdiction.⁴
Social Security information for every dependent you are listing.
Full names, addresses, and living status for both of your birth parents. If a parent is deceased, that is noted directly on the form in place of the address.⁵

Certified copies of vital records generally run $15 to $35 from the issuing jurisdiction. Replacement Consular Reports of Birth Abroad come from the State Department’s Vital Records Office.³

Completing the Form

Every sailor, officer or enlisted, should have a self-service ESR account at nsips.cloud.navy.mil.¹ From there, open the RED/DA module and enter or update your information. If you cannot get into the electronic system, your Command Pay and Personnel Administrator can process the update on paper.

Names and dates on the form must match your certificates exactly. A single letter off between your marriage certificate and the form can hold up a housing allowance change.

Blocks That Carry Real Weight

Not every block applies to every sailor, but these are the ones that most often cause problems if they are wrong.

Blocks 33–37, parent information. Enter each birth parent’s full name and current address. For a deceased parent, write “DECEASED” in the address field. Put the mother’s maiden name in parentheses, for example “Mary Jane (Jones) Smith.”⁵
Block 47, other notification contacts. Name anyone not already listed whom you want notified if you are seriously injured or killed. A stepparent, close friend, or mentor is common here.⁵
Block 53, beneficiary for unpaid pay and allowances. You can designate anyone. This is pay the government owes you at the time of death, separate from the death gratuity and from life insurance.
Block 57, missing-status allotment recipient. This person must be a relative. A fiancé does not qualify. You can allocate up to 80 percent of your pay; the rest is held in reserve.⁵
Blocks 64–66, non-SGLI insurance. Record any private life insurance policies. The form explicitly does not designate or change Servicemembers’ Group Life Insurance beneficiaries. SGLI changes require a separate SGLV 8286.
Block 78, primary and secondary next of kin. Enter name (or relationship), phone number, and address. Precede each entry with “PNOK” or “SNOK.”⁵

Sign Block 79 when the form is complete. Your approving officer signs Block 80.

Submission and the 30-Day DEERS Step

Once you have finished the entry, an approving officer reviews and certifies it. The electronic signature transmits the updated record to your Official Military Personnel File for permanent storage. Any paper you submit is scanned into the digital record and then destroyed; the scan becomes the official original.⁶

After the RED/DA update is applied, you have 30 days to bring a printed copy of the updated 1070/602, along with your supporting documents, to a DEERS/RAPIDS office (the base ID card lab) to get your dependents enrolled or updated in DEERS.⁷ Missing the window does not trigger an automatic penalty, but until DEERS reflects the change, your dependents may not have valid ID cards or access to medical benefits.

Verify the update posted correctly. Log back into your ESR at nsips.cloud.navy.mil and check every beneficiary name, percentage, and dependent entry against your certificates. Report any discrepancy to your personnel office right away. A typo in a dependent’s name creates downstream problems with BAH, TRICARE enrollment, and travel orders.

Why Accuracy Matters

The most common financial hit from an outdated dependency record is a Basic Allowance for Housing overpayment. BAH is paid at either a with-dependents or without-dependents rate; the count of dependents does not change the rate, only whether you have at least one.⁸ A sailor who gets divorced and never updates the 1070/602 keeps drawing the higher rate. When the error surfaces, DFAS recoups the difference for every month of overpayment, and the paycheck deductions to clear the debt can be steep.

Dependency data also drives family separation allowance, overseas housing allowance, and travel entitlements for permanent change of station moves. An outdated record can cost you benefits you were entitled to or leave you holding debts for benefits you were not.

Deliberately putting false information on the form is a different problem entirely. Article 107 of the Uniform Code of Military Justice covers false official statements, and a document submitted through NSIPS or to a personnel office qualifies as official because it directly affects military functions.⁹ The government must prove the sailor signed a false document, knew it was false, and intended to deceive. Maximum punishment is five years of confinement and a dishonorable discharge, which permanently strips VA benefit eligibility.

Fixing a Mistake After the Fact

If you find an error in your record later, the path depends on what kind of error it is.

For routine clerical mistakes, a misspelled name, an outdated address, a data entry slip, work through your personnel office or Transaction Service Center. They can correct the entry directly in NSIPS.

For a contested entry, or a record you believe reflects an injustice, the formal route is the Board for Correction of Naval Records. Submit a DD Form 149 (Application for Correction of Military Record) with evidence supporting your claim. The Board prefers applications by email at BCNRApplication@us.navy.mil.¹⁰ If the Board denies the request, you can submit a new DD Form 149 with evidence that was not considered the first time around.¹¹
