The Morgan Stanley IRA Distribution Form is the paperwork you use to withdraw money from a traditional, Roth, SEP, SIMPLE, or inherited IRA held at the firm. You can download it from Morgan Stanley Online, request a copy from your Financial Advisor, or pick one up at a branch. After you complete it, attach any required documents, and submit it, most straightforward requests are processed within a few business days and paid out by check, ACH, or wire.
One thing to confirm before you start: Morgan Stanley uses separate distribution forms for its Wealth Management accounts and its Morgan Stanley Funds (mutual fund) accounts. Make sure the version in front of you matches your account type.
Where to Get the Form
The fastest route is logging into Morgan Stanley Online and downloading the form directly. If you don’t have online access, your Financial Advisor can email or mail you a copy, and branch offices keep printed versions on hand.
Filling Out the Form Section by Section
The form is organized into numbered sections. For a standard lifetime withdrawal you’ll complete most of them. For narrower requests like removing an excess contribution or claiming a death benefit, the header at the top of the form tells you which sections to skip.
Account Information and Distribution Type
Start with your full name, Social Security number, and Morgan Stanley account number. Then choose the reason for the distribution:
- Normal distribution — you’re 59½ or older and simply withdrawing funds.
- Required Minimum Distribution (RMD) — you’ve reached the age at which the IRS requires annual withdrawals. Under SECURE 2.0, that age is 73 if you were born between 1951 and 1959, or 75 if you were born in 1960 or later.1Congress.gov. Required Minimum Distribution (RMD) Rules for Original Owners
- Early distribution — you’re under 59½. This triggers a 10% additional tax unless an exception applies.2Internal Revenue Service. Substantially Equal Periodic Payments
- Death distribution — you’re a beneficiary withdrawing from a deceased owner’s account.
- Disability distribution — you’re withdrawing because of a qualifying disability.
- Divorce transfer — assets are being moved to a former spouse’s IRA under a divorce decree.
You’ll also specify whether you want a specific dollar amount, a percentage of the balance, or a full liquidation.
Tax Withholding
The IRS treats most traditional IRA distributions as ordinary income. Unless you elect otherwise, the default federal income tax withholding on a non-periodic distribution is 10%.3Internal Revenue Service. Pensions and Annuity Withholding You can request a higher rate or elect zero withholding by completing IRS Form W-4R and attaching it to your distribution request.
State withholding rules vary. Some states require a minimum withholding whenever federal taxes are withheld; others let you opt out entirely. If your address of record is a P.O. Box, Morgan Stanley is required to withhold federal income tax at the default rate unless you also provide a residential address.
Electing zero withholding doesn’t erase the tax; it just shifts payment to filing time. If you expect to owe a large amount, estimated tax payments can help you avoid an underpayment penalty.
How You Want to Receive the Money
Three delivery options:
- ACH transfer to a linked bank account, typically arriving in two to three business days.
- Wire transfer, often settling within one business day. Morgan Stanley charges $25 per outgoing domestic wire.4Morgan Stanley. Access Direct Pricing
- Paper check mailed to the address on file. Allow extra time for postal delivery.
If you want the check mailed to a different address, or the ACH or wire instructions on the form don’t match what’s already on the account, expect to need a signature guarantee.
Signature and Medallion Signature Guarantee
Every distribution requires your signature and date. A Medallion Signature Guarantee, a special stamp from a bank, broker-dealer, or credit union that verifies your identity, is also required in these situations:
- The distribution exceeds $100,000 and is going to the address on file.
- The distribution involves death benefits or a divorce transfer.
- The payee is someone other than the account holder.
- The check is being sent to an address different from the one on file.
- Wire or ACH instructions on the form don’t match what’s already on the account.
- Your name has changed since the account was opened. In that case you need a “one-and-the-same” signature guarantee stating both names.
A notary stamp does not substitute for a Medallion Signature Guarantee.5Morgan Stanley Investment Management. Morgan Stanley Traditional IRA Distribution Form Most banks and brokerages provide the guarantee for free to their own customers, but call ahead to confirm availability.
Supporting Documents for Special Distributions
A routine withdrawal by the account owner at age 60 doesn’t need anything beyond the completed form. Other situations require extra paperwork.
Death Distributions
If you’re a beneficiary claiming funds from a deceased owner’s IRA, attach a certified copy of the death certificate and a notarized affidavit of domicile and debts.6Morgan Stanley. IRA Distribution Form If the beneficiary is a trust, Morgan Stanley also asks for excerpts from the trust agreement showing the trustee’s name, investment powers, and signature pages. A signature guarantee is required. If you already hold the money as an inherited IRA in your own name at Morgan Stanley, these extra documents aren’t needed for routine withdrawals from that inherited account.
Divorce Transfers
IRAs don’t require a Qualified Domestic Relations Order. Assets are divided under a divorce decree or separation agreement, and the transfer is tax-free as long as it goes directly to the former spouse’s IRA.7Office of the Law Revision Counsel. 26 USC 408 – Individual Retirement Accounts Attach a certified copy of the divorce decree. Morgan Stanley’s legal team reviews the document before processing, and a signature guarantee is required.
Disability Distributions
To qualify for the early withdrawal penalty exception on disability grounds, the IRS requires that you be unable to perform any substantial gainful activity because of a physical or mental impairment expected to result in death or to last indefinitely. In practice, that means documentation from a physician confirming the diagnosis and its expected duration. Checking the disability box on the form isn’t what protects you from the 10% additional tax; the supporting medical documentation is.
Other Early Withdrawal Exceptions
Several other situations let you withdraw before 59½ without the 10% penalty:
- First-time home purchase, up to $10,000 in lifetime IRA withdrawals for buying, building, or rebuilding a first home. You must use the funds within 120 days of receiving them.8Internal Revenue Service. Retirement Topics – Exceptions to Tax on Early Distributions
- Unreimbursed medical expenses that exceed 7.5% of your adjusted gross income.
- Substantially equal periodic payments based on your life expectancy under an IRS-approved method. Payments must continue for five years or until you turn 59½, whichever comes later.2Internal Revenue Service. Substantially Equal Periodic Payments
- Qualified higher education expenses for you, your spouse, or dependents.
- Health insurance premiums while unemployed, if you’ve received unemployment compensation for at least 12 consecutive weeks.
Even when the penalty is waived, the withdrawn amount is still taxable as ordinary income for traditional IRA distributions. You may need to file IRS Form 5329 to claim the exception.
Submitting the Form and What It Costs
Morgan Stanley accepts the completed form through several channels:
- Online upload through the secure message center in Morgan Stanley Online. This is the fastest option.
- Delivery to your Financial Advisor, in person at a branch or through the firm’s secure email tools.
- Fax to the processing center. Your advisor or the client service line can provide the current number.
- Mail. Certified mail gives you a paper trail. Confirm the mailing address with your advisor, since the firm routes paperwork to different service centers depending on account type.
Before sending, double-check that the form is signed and dated, all supporting documents are attached, and any required signature guarantee has been obtained. Missing one of these is the most common reason requests get returned.
Fees to plan for:
- Account termination fee of $125 if the distribution closes your IRA entirely. This is deducted from the balance before the final payout, so a full liquidation lands $125 short of the raw account value.9Morgan Stanley. Morgan Stanley Wealth Management Schedule of Miscellaneous Account and Service Fees
- Outgoing domestic wire fee of $25.4Morgan Stanley. Access Direct Pricing
- No additional fee for ACH transfers to a linked bank account or for checks mailed to your address on file.
How Long Processing Takes
Straightforward distributions, meaning a normal withdrawal by the account owner with no legal documents involved, generally process within a few business days after Morgan Stanley receives the completed form. Requests involving death benefits, divorce transfers, or trust review take longer because the firm’s legal team must verify each document. You can track status through Morgan Stanley Online or by contacting your advisor, and you’ll receive a confirmation through the portal or by mail once the distribution is processed.
RMDs on the Distribution Form
If you own a traditional, SEP, or SIMPLE IRA and have reached the RMD age set by SECURE 2.0, use the form to take your annual withdrawal.1Congress.gov. Required Minimum Distribution (RMD) Rules for Original Owners Your first RMD must be taken by April 1 of the year after you reach the applicable age, and every subsequent RMD is due by December 31.
Select the RMD distribution type. If you want Morgan Stanley to calculate the amount based on your account balance and life expectancy factor, indicate that on the form. Otherwise, enter the exact dollar amount yourself. Missing an RMD carries a 25% excise tax on the shortfall, dropping to 10% if you correct it within the IRS window and file Form 5329.10Office of the Law Revision Counsel. 26 USC 4974 – Excise Tax on Certain Accumulations in Qualified Retirement Plans11Internal Revenue Service. Instructions for Form 5329
Roth IRAs do not require RMDs during the original owner’s lifetime.
Inherited IRAs
The rules depend on when the original owner died and your relationship to them. For deaths on or after January 1, 2020, most non-spouse beneficiaries must empty the inherited IRA by December 31 of the tenth year after the owner’s death.12Internal Revenue Service. Retirement Topics – Beneficiary If the original owner had already started RMDs, you also owe annual distributions in years one through nine. If they hadn’t, you can distribute on any schedule as long as the account is empty by year ten.
Certain eligible designated beneficiaries, including surviving spouses, minor children of the deceased, disabled or chronically ill individuals, and people no more than ten years younger than the original owner, can stretch distributions over their own life expectancy instead. A surviving spouse can also roll the inherited IRA into their own IRA.
On the Morgan Stanley form, inherited IRA distributions are handled differently from death distributions on the original owner’s account. If you already hold the account as an inherited IRA in your own name, you typically complete the standard distribution sections without the extra death-benefit paperwork.
Roth Withdrawals and Rollovers
Roth IRA withdrawals work differently. You can always pull out your original contributions tax-free and penalty-free, regardless of age or how long the account has been open. The IRS applies an ordering rule: contributions first, then conversions, then earnings.
Earnings become tax-free and penalty-free only through a qualified distribution, which requires two things: you must be at least 59½ (or disabled, or using up to $10,000 for a first-time home purchase), and at least five tax years must have passed since your first contribution to any Roth IRA. That five-year clock starts on January 1 of the tax year of your first Roth contribution, so a contribution made for tax year 2021 starts the clock on January 1, 2021, no matter when you actually deposited the money. If you withdraw earnings before meeting both conditions, those earnings are taxable and may face the 10% penalty. The Roth distribution type you select on the form determines how the transaction is coded for tax reporting.
If you take a distribution and then want to deposit the money into another IRA instead of keeping it, you have exactly 60 days from the date you receive the funds to complete the rollover. Miss the window and the entire amount counts as a taxable distribution, plus the 10% penalty if you’re under 59½.13Internal Revenue Service. Retirement Plans FAQs Relating to Waivers of the 60-Day Rollover Requirement You can only do one IRA-to-IRA indirect rollover in any 12-month period, regardless of how many IRAs you own.14Internal Revenue Service. Rollover Chart Direct trustee-to-trustee transfers, where the money moves between custodians without touching your hands, aren’t subject to either limit. If you’re moving IRA assets to a new custodian, ask Morgan Stanley to process it as a direct rollover.
The 1099-R You’ll Receive
After any distribution, Morgan Stanley issues IRS Form 1099-R reporting the gross amount, the taxable portion, any federal and state tax withheld, and a distribution code in Box 7 that tells the IRS what type of withdrawal occurred.15Internal Revenue Service. About Form 1099-R Morgan Stanley must furnish the form to you by January 31 of the year following the distribution.16Internal Revenue Service. General Instructions for Certain Information Returns
When it arrives, check that the Box 7 code matches the type of withdrawal you actually took. An incorrect code, for example Code 1 (early distribution, no known exception) when you qualified for an exception, can trigger an unnecessary penalty notice from the IRS. If you spot an error, contact Morgan Stanley immediately to request a corrected 1099-R before you file your return. If you claimed a penalty exception, you’ll also file Form 5329 to document it.
