Information You Need to Fill In
The form asks for a short list of details. Getting them right is the difference between a smooth setup and a rejected deposit.

- Account number, printed just to the right of the routing number on your checks, or shown in online banking under the same menu.
- Account type. You need to specify checking or savings. Pick the wrong type and the transaction can bounce back to your employer’s payroll system.
- Your full legal name, address, and signature. The signature is what authorizes payroll to deposit funds into the account.
- Employer or payer details, usually the company name and address, and sometimes a company ID or payroll contact.
Many employers also ask you to attach a voided check. It independently confirms your routing and account numbers. If you don’t have checks, request a direct deposit verification letter from KeyBank at a branch or through customer service. Splitting Pay Across Accounts
If you want part of your pay in a KeyBank checking account and the rest in savings or a different bank, most payroll systems let you split deposits. Fill out a separate line, or a second form, for each account with its own routing number, account number, and account type. Splits are usually set as fixed dollar amounts or as percentages, and your employer’s system decides how many accounts you can use. Changing or Canceling Your Direct Deposit
