The JSX pet form is a one-page liability agreement every passenger flying with a dog or cat must complete before boarding. You fill in your booking and contact details, add your pet’s name, breed, and weight, sign a declaration that the animal is trained and fit to fly, and hand the completed form to staff at the hangar during check-in.1JSX. JSX Pet Acceptance Form Its formal name is the Pet Acceptance Form, and by signing it you accept full financial responsibility for any loss, injury, or damage your pet causes.2JSX. How to Get the Form
The route to the form depends on your pet’s size. If your cat or small dog will travel in a carrier under the seat, you can add the pet to your booking through the JSX website or app, and download the Pet Acceptance Form as a PDF from the JSX pet policy page. The under-seat option costs a flat $100 each way.3JSX. JSX Pet Policy
If your dog is too large to fit in an under-seat carrier, you cannot book online. JSX requires you to call its reservations center to purchase an adjoining seat for the dog. The agent will walk you through the policy and email the Pet Acceptance Form to you before your flight.3JSX. JSX Pet Policy Either way, you need a completed copy ready at the hangar.
What the Form Asks For
The form has two parts. The top collects identification:
- Customer name as it appears on the reservation
- Reservation number (the confirmation code for your JSX booking)
- Customer address and phone number
- Pet name, breed, and weight
These fields let the crew match the animal at the hangar to the paperwork on file.1JSX. JSX Pet Acceptance Form
The bottom section is the legal declaration. By signing, you confirm that your animal has been trained to behave in a public setting, has no history of aggressive or disruptive behavior, does not pose a threat to others, and is fit to fly.1JSX. JSX Pet Acceptance Form You also accept full liability: if the animal causes any loss, injury, damage, or expense to JSX, its customers, or anyone else, you agree to reimburse JSX for those costs.2JSX. JSX Pet Acceptance of Liability Form False attestations here create real exposure if something goes wrong mid-flight.
Carrier Requirements to Confirm Before You Sign
The form only gets you on board if the carrier itself passes inspection. For small pets, JSX sets maximum carrier dimensions at 13 inches wide, 11 inches tall, and 17 inches long. The carrier must be leak-proof and well-ventilated, and the pet needs to fit comfortably inside for the entire flight alongside any personal items stowed in the same space.3JSX. JSX Pet Policy
Check the dimensions at home with the pet inside. A soft-sided carrier that looks fine empty can bulge past the limit once loaded, and a carrier that fails the visual check at the hangar means the animal will not be allowed to board, form or no form.
Submitting the Form at Check-In
Bring a completed physical or digital copy of the form to the JSX hangar. Check-in opens roughly 20 to 30 minutes before departure on most flights.4JSX. JSX – Book Non-Stop Flights and Airfare A crew member reviews the form, verifies that the information matches the animal and carrier in front of them, and confirms everything is in order before issuing your boarding pass. JSX keeps the signed form with the flight’s records; there is no post-flight paperwork.
Note that emotional support animals are classified as pets under JSX policy, not service animals, so they use this same Pet Acceptance Form and pay the standard fee. A maximum of five pets travel in the cabin per flight.3JSX. JSX Pet Policy
If You Have a Service Animal, You Need a Different Form
Trained service dogs do not use the Pet Acceptance Form. They travel under Department of Transportation rules, which require handlers to complete the DOT Service Animal Air Transportation Form attesting that the dog is trained, vaccinated for rabies, and free of fleas, ticks, and disease.5U.S. Department of Transportation. Service Animal Air Transportation Form JSX makes the DOT form available through its pet policy page, and a printed copy is required at check-in. A vest, harness, or registration card alone is not accepted as proof.3JSX. JSX Pet Policy Service animals travel without a pet fee and do not count toward the five-pet cabin cap.
