The isolved direct deposit form authorizes your employer to send your paycheck electronically into one or more bank accounts through the ACH network. You can complete it on paper or enter the same information in isolved’s Employee Self-Service portal. Either way, have your bank’s nine-digit routing number, your account number, and a voided check in front of you before you begin.
What to Gather First
Three pieces of information do the work, whether you submit on paper or online:
- The nine-digit routing number that identifies your bank in the ACH network. On a personal check, it is the first set of numbers along the bottom-left edge. Your bank’s online portal or mobile app also lists it under account details.1U.S. Bank. U.S. Bank Routing Number
- Your account number, which sits to the right of the routing number on a check. Together with the routing number, it points to your specific account.2TD Bank. How To Setup Direct Deposit and Authorization Form
- A voided check for each account you plan to list. The isolved form instructions specifically call for a voided check. Deposit slips will not substitute, because they often lack the external ACH routing number your employer needs.3Arion Care Solutions. Direct Deposit Authorization Form
If the account is a savings account or sits at an online-only bank that does not issue checks, ask the bank for a direct deposit authorization letter listing your name, routing number, account number, and the bank’s contact information. Most banks generate one through their website in a few minutes.
Filling Out the Paper Form
The isolved direct deposit authorization form is a single page with room for up to three accounts. Your HR team may hand it to you at onboarding or post it in an internal portal. Work through the fields in order:3Arion Care Solutions. Direct Deposit Authorization Form
- Employer Name and Employee Name, printed exactly as they appear in your company’s payroll records.
- Last four digits of your Social Security number. The form does not ask for the full number.
- Bank Name for each institution receiving a deposit.
- A mark showing whether each account is checking or savings.
- Routing Number and Account Number, copied carefully from the voided check or bank letter. A single transposed digit will cause the deposit to bounce.
- Allocation columns. For each account, choose one method: a specific dollar amount, a percentage of net pay, the entire net pay, or the remainder of net pay. If you list more than one account, designate one to receive the remainder so nothing is left unallocated.
- Your signature and the date. Signing authorizes isolved to deposit funds and to debit the account to recover any overpayment.
Attach a voided check for each account before you hand the form to HR. Missing supporting documentation is the most common reason employers send the form back.
Entering It in the isolved Portal
If your employer has enabled Employee Self-Service, you can skip the paper form. The path depends on which interface your company uses.
Classic Employee Self-Service
Log in and go to Employee Self-Service, then Direct Deposit Updates. Click Add New for a blank entry. You will see these fields:4employdrive. Employee Self-Service: Updating Direct Deposit
- Status. Set to Active for any account you want funded, or Inactive to stop deposits without deleting the record.
- Account Type. Options are Checking, Savings, Pay Card (Checking), and Pay Card (Savings).
- Sequence. With only one account, set this to Remaining Net. With multiple accounts, number the ones that take a fixed amount or percentage first, then mark your final account Remaining Net.
- Amount or Percent. Used only when splitting deposits.
- Routing Number and Account Number, entered exactly as they appear on your check or bank letter.
- Description. Optional label like “Savings” or “Joint Checking” to tell accounts apart later.
Click Save. If your employer runs an approval workflow, saving triggers a request the payroll administrator has to approve before the deposit goes live.4employdrive. Employee Self-Service: Updating Direct Deposit
Adaptive Employee Experience
In the newer interface, go to Pay and Tax, then Direct Deposit. Any accounts on file appear on the screen. Click Add New to enter a new account or click an existing entry to edit it.5employdrive. Adaptive Employee Experience: Direct Deposit Updates The fields and logic match the classic version; only the layout differs.
Splitting a Paycheck Across Multiple Accounts
Sending a fixed amount to savings and the rest to checking is a common setup, and it is where people trip up. The rule: accounts with a fixed dollar amount or percentage get funded first, in sequence order. The account marked Remaining Net gets whatever is left.
To send $200 per paycheck to savings and the rest to checking, set savings to Sequence 1 with an amount of $200, then set checking to Remaining Net.4employdrive. Employee Self-Service: Updating Direct Deposit To split by percentage, either assign percentages to both accounts that add up to 100, or give one a percentage and mark the second Remaining Net so it collects whatever is left.
On the paper form, the “Specific Dollar Amount,” “% Of Net Pay,” and “Remainder Of Net Pay” columns handle the same split. Leave “Entire Net Pay” blank on every entry unless a single account should receive your full paycheck.3Arion Care Solutions. Direct Deposit Authorization Form
One caution when your net pay fluctuates. If every account has a fixed dollar amount and a paycheck comes in lower than the total, the system cannot fund all accounts fully. Always leave at least one account as Remaining Net to absorb the variance.
What Happens After You Submit
Your first electronic deposit does not usually hit on the next payday. Most employers send a prenotification first, a zero-dollar test transaction through the ACH network that confirms your routing and account numbers reach a valid bank account. Under Nacha rules, the employer must wait at least three banking days after sending the prenote before processing a live deposit.6ACH Pro. What Are ACH Prenotes and How to Use Them In practice, many companies wait a full pay cycle or two, so expect a paper check in the interim.
If the prenote fails, the bank returns it with a rejection notice or a Notification of Change flagging what is wrong, such as a mismatched name, closed account, or wrong account type. Payroll will contact you to correct it and the prenote cycle restarts. If nothing comes back during the waiting period, the account is verified and live deposits begin.
Check the isolved portal after your next payday. The Direct Deposit section shows an Active status for each verified account. If you see Inactive or Pending, contact payroll before assuming something went wrong. Approval workflows move at different speeds.
Changing or Canceling a Direct Deposit
When you switch banks or close an account, update your information right away. In classic ESS, open Direct Deposit Updates and select the account you want to change. Every field is editable except the account number itself. To change the account number, use the dedicated “Update Acct. Number” field rather than typing over the main field.4employdrive. Employee Self-Service: Updating Direct Deposit
To stop deposits without erasing history, set Status to Inactive rather than deleting the record. In the Adaptive Employee Experience interface, if you delete an account outright, the system routes your net pay to whatever Remaining Net account is still active, or cuts a paper check if none is.5employdrive. Adaptive Employee Experience: Direct Deposit Updates
Any change to banking details will likely trigger another prenote before live deposits resume. Time the update so it does not collide with a payday. Submit at least five to seven business days before your next expected pay date.
Protecting Your Payroll Information
Direct deposit fraud typically starts with impersonation. Someone phishes your credentials or emails HR pretending to be you and asks payroll to reroute your paycheck. By the time you notice the missing deposit, the money is gone.
Turn on two-factor authentication in isolved if it is available. A text code or authenticator prompt blocks anyone who has only your password. Many employers also require secondary confirmation before processing a direct deposit change, which slows you down slightly and works in your favor.7Asure Software. How to Prevent Direct Deposit Fraud: Best Practices for Employees and Employers
Do not email banking details in plain text, even to your own HR department. If your employer wants the paper form submitted electronically, use the company’s secure upload portal or hand-deliver it. Read your pay stubs each cycle. Catching a missing or short deposit early is the fastest way to spot an unauthorized change.
What Your Employer Can and Cannot Require
Federal law lets your employer require direct deposit, with one limit that matters when you fill out the form: your employer cannot make you use a specific bank. The Electronic Fund Transfer Act prohibits requiring a consumer to open an account at a particular financial institution as a condition of employment.8Office of the Law Revision Counsel. 15 USC 1693k – Compulsory Use of Electronic Fund Transfers You pick the bank. The employer picks the payment method.
State rules layer on top. Some states bar any fee for setting up direct deposit; others require written consent before enrollment. If your employer’s policy feels restrictive, check with your state’s department of labor.
