How to Fill Out and Submit the Invesco Retirement Account Transfer/Rollover Form

Invesco’s Retirement Account Transfer/Rollover Form is a single PDF that moves eligible retirement assets into an Invesco Traditional, Roth, SEP, SIMPLE, or SARSEP IRA, whether the money is coming from a former employer’s 401(k) or 403(b) plan or from an IRA held at another custodian. Download it from Invesco’s forms page, complete the numbered sections with your information and your current custodian’s details, choose your Invesco funds, sign in ink, and mail the packet with a recent statement to Invesco Investment Services in Kansas City. The steps below walk through each part of that process and flag the choices that decide how the money gets taxed.

Where to Download the Form

Go to invesco.com/us/en/accounts/forms.html and look for “Retirement Account Transfer/Rollover Form.”1Invesco. Invesco Forms and Literature The PDF downloads straight to your computer. The same form covers direct rollovers from employer-sponsored plans (401(k), 403(b), profit-sharing), trustee-to-trustee transfers between IRAs, conversions from a Traditional IRA at another custodian into an Invesco Roth IRA, and beneficiary IRA transfers.

A few related forms sit on the same page. If you already have an Invesco Traditional or SEP IRA and want to convert it to an Invesco Roth, use the Invesco IRA to Invesco Roth IRA Conversion Form instead. If the account owner has died and you’re moving inherited assets, the IRA Beneficiary Transfer/Distribution Form applies. If you missed the 60-day window for an indirect rollover, Invesco has a Self-Certification for Late Rollover Contribution Form.1Invesco. Invesco Forms and Literature

Decide Which Kind of Move You’re Making

The form handles two different transactions, and picking the wrong one can create a tax problem. Sort this out before you start filling in boxes.

Direct Rollover From an Employer Plan

A direct rollover moves money from a 401(k), 403(b), or similar qualified plan straight to your Invesco IRA. The check is payable to Invesco, not to you. Because the funds never pass through your hands, no federal tax is withheld and the 60-day deadline doesn’t apply.2Internal Revenue Service. 401(k) Resource Guide – Plan Participants – General Distribution Rules The plan will release funds only after a distributable event, typically leaving the job, retiring, or becoming disabled.3Invesco. Traditional and Roth IRAs

IRA-to-IRA Transfer

A transfer moves assets from an IRA at one custodian to an IRA of the same type at Invesco. It’s a trustee-to-trustee move. No tax is withheld, and the once-per-year rollover limit doesn’t apply because the IRS doesn’t classify a direct custodian-to-custodian move as a rollover.4Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions You can do as many transfers as you want in a year. This is the cleanest way to consolidate multiple IRAs.

Indirect Rollover

An indirect rollover means the old custodian writes the check to you, and you have 60 calendar days to deposit the full amount into the new IRA.5Internal Revenue Service. Topic No. 413, Rollovers From Retirement Plans If the distribution comes from an employer plan, 20% is withheld for federal tax before the check is cut, so rolling over the original gross amount means covering that 20% from other savings.2Internal Revenue Service. 401(k) Resource Guide – Plan Participants – General Distribution Rules Miss the 60 days and the whole distribution becomes taxable income; if you’re under 59½, the 10% early withdrawal penalty under IRC Section 72(t) also applies.6Office of the Law Revision Counsel. 26 U.S. Code 72 For IRA-to-IRA indirect rollovers, the IRS limits you to one per 12-month period across all your IRAs combined.4Internal Revenue Service. Rollovers of Retirement Plan and IRA Distributions

The direct route is almost always the better choice. The form supports both, but a direct rollover or trustee-to-trustee transfer keeps the money in tax-advantaged status without withholding or a deadline hanging over you.

What to Have in Front of You

Gathering these items before you start prevents the round-trips that stall processing:

  • Your Social Security number and, if you’re already an Invesco client, your Invesco account number.
  • The current custodian’s full legal name, mailing address (a physical street address if overnight delivery may be needed), phone number, and your account number there.
  • A recent statement from the current custodian. Invesco asks for a copy, and it helps the processing team verify the account.
  • The Invesco fund names and fund numbers you want to invest in. If you’re not sure, call Client Services at 800-959-4246.7Invesco. Retirement Plans
  • A decision on whether you’re moving the whole balance or a partial amount, and if partial, whether in dollars or as a percentage.

Filling Out the Form Section by Section

The form is organized into numbered steps. Layouts can change between revisions, so check the revision date on the copy you downloaded.

Account Owner Information

Enter your full legal name, Social Security number, date of birth, and mailing address. If you already hold an Invesco IRA, put that account number here. If you’re opening a new IRA at Invesco as part of this move, submit an Individual Account Application alongside the transfer form; the forms page has that document too.

Current Custodian Information

Fill in the name, phone number, mailing address, and account number of the institution you’re moving money from. Use the custodian’s physical street address rather than a P.O. box where possible, since Invesco may need to send a letter of acceptance by overnight mail. Attach a copy of your most recent statement.

Type of Transfer or Rollover

This section tells Invesco what kind of account the money is coming from and how the receiving account should be classified. Options typically cover a transfer from a Traditional IRA, SEP IRA, or Roth IRA, or a rollover from a qualified employer plan such as a 401(k), 403(b), SIMPLE IRA, or profit-sharing plan. If any of the funds are inherited, flag that here so Invesco sets up the account with the correct beneficiary IRA designation and reporting.1Invesco. Invesco Forms and Literature

Match pretax to pretax and Roth to Roth. Rolling Roth 401(k) money into a Traditional IRA, or the reverse, creates a taxable event.3Invesco. Traditional and Roth IRAs

Amount and Liquidation Instructions

Choose the full balance or a specific partial amount. For a partial, you’ll typically specify a dollar amount. If the current account holds fund positions, you may also indicate whether to liquidate everything or transfer specific holdings as shares. Vague instructions cause delays, so be precise.

Investment Elections at Invesco

Tell Invesco where to put the money once it arrives. List each Invesco fund by name and fund number with the dollar amount or percentage going to each. Allocations need to total 100% of the transfer amount. If you haven’t picked funds yet, call 800-959-4246 for help.

Signatures and When a Medallion Guarantee Is Needed

Sign and date the form in blue or black ink. The form is scanned digitally during processing, so every entry needs to be legible; smudged or faint writing gets rejected.

A Medallion Signature Guarantee may be required depending on the dollar amount or the type of transfer. It’s a specialized verification stamp issued by participating banks, credit unions, or broker-dealers, and a regular notary stamp does not satisfy it.8U.S. Securities and Exchange Commission. Medallion Signature Guarantees Not every institution issues them, so call your bank ahead of time to confirm participation. The guarantee carries a surety bond with coverage that can range from $100,000 to $10,000,000 depending on the guarantor’s participation level.9Investment Company Institute. Medallion Signature Guarantee Considerations and Alternatives If you’re unsure whether your amount triggers the requirement, call Invesco at 800-959-4246 before submitting. Getting the guarantee up front beats having the packet mailed back.

Where to Mail the Completed Form

Once the form is signed and any required Medallion guarantee is on it, mail everything to Invesco Investment Services. Use the address that matches your delivery method:10Invesco. Investors – Contact Us

What Happens After Invesco Receives It

Once the form is in good order, Invesco sends a transfer request or letter of acceptance to your current custodian. The old custodian then liquidates the relevant assets (if the account holds fund positions rather than cash) and sends the proceeds to Invesco by check or wire. Invesco deposits the money into the funds you selected.

Timing depends largely on how quickly your old custodian processes the outgoing transfer. Some release assets within a few business days; others take two to three weeks, particularly employer-sponsored plans that need additional paperwork from the plan administrator. You can track progress through your Invesco online account dashboard or by calling Client Services. A confirmation statement arrives by mail once the funds settle and shares appear in your account.

Mistakes That Get the Form Rejected

Most rejections trace back to a short list of avoidable errors:

  • Mismatched account types (pretax rolled into Roth or vice versa).
  • Missing or outdated custodian information. If your old custodian has changed names through a merger, use the current legal name, not the name on old statements.
  • No Medallion Signature Guarantee when one is required. Invesco returns the form rather than processing it.
  • Forgetting to include a recent statement from the resigning custodian.
  • Illegible handwriting. Print clearly in blue or black ink.
  • Trying to roll over a required minimum distribution. Take the RMD first, then roll over the remainder.

If an indirect rollover missed the 60-day window for reasons beyond your control, Invesco will accept a Self-Certification for Late Rollover Contribution with the funds and report the contribution with a waiver notation on Form 5498.3Invesco. Traditional and Roth IRAs The IRS allows self-certification under Revenue Procedure 2020-46 for specific qualifying reasons, such as hospitalization, postal errors, or a death in the family.5Internal Revenue Service. Two Things the Form Won’t Do

Required minimum distributions cannot be rolled over. The form includes a certification to that effect. Under SECURE Act 2.0, RMDs begin at age 73 for people born between 1951 and 1959 and at age 75 for those born after 1959.11Internal Revenue Service. Retirement Plan and IRA Required Minimum Distributions FAQs If you’re rolling assets from an employer plan in a year an RMD is due, the plan must distribute the RMD to you separately before processing the rollover; that portion is taxable income and cannot go into the Invesco IRA.

The form also won’t shield you from tax reporting on a move that is itself tax-free. Around tax season you’ll receive Form 1099-R from the distributing custodian and Form 5498 from Invesco showing the rollover contribution received. For a direct rollover, the 1099-R should show zero in the taxable amount box and distribution Code G in Box 7.12Internal Revenue Service. 2025 Instructions for Forms 1099-R and 5498 Keep both forms with your tax records; the IRS matches them, and a missing 5498 against a 1099-R distribution can trigger a notice.
