The HealthEquity HSA contribution form is a one-page PDF you use to deposit personal, after-tax money into your Health Savings Account outside of payroll. Download it from the Forms and Documents section of the member portal, fill in your account details and the tax year, and mail it with a check to HealthEquity’s processing center in Lexington, Kentucky. You can also skip the paper entirely and move the same funds through the online portal from a linked bank account.
Most people reach for the form when payroll deductions alone won’t get them to the annual limit, when they’re self-employed, or when they want to make a prior-year contribution before the April tax-filing deadline.
What to Have Ready
Pull these together before you open the PDF:
- Your HealthEquity account number or member ID. It’s on the front of your HealthEquity debit card and inside the member portal.
- Your Social Security Number. The form uses it to tie the deposit to your tax identity.
- Your employer’s name, if your HSA is linked to a workplace plan. The form asks for the name, not a separate employer ID.
- A personal check made payable to HealthEquity, or a verified bank account already linked in the portal.
- The tax year you want the contribution applied to. Prior-year contributions are allowed up to the tax-filing deadline, typically April 15. If you leave the year blank, HealthEquity applies the deposit to the tax year matching the check date.1Internal Revenue Service. Before you settle on the dollar amount, log into the portal and check your year-to-date contributions. Any employer contributions that have already posted count toward the same annual cap, so subtract those from your remaining room.
Filling Out the Form
The form is a single page and reads straight through, but a few fields cause problems if you rush.
The top section asks for your full legal name, mailing address, email, daytime phone, and either your SSN or HealthEquity ID. Put your employer’s name in the employer field if your account is workplace-linked.2HealthEquity. HSA Contribution Form
In the contributions section, check the box for the tax year you want the deposit applied to and write in the exact dollar amount. The amount on the form has to match the check exactly. A mismatch delays processing.
Sign and date the bottom. An unsigned form gets sent back.
One boundary worth flagging: if you’re returning money you withdrew by mistake, that’s not a contribution and it doesn’t go on this form. HealthEquity has a separate Mistaken HSA Distribution form for that, covered below.
Where to Send It
Mail the completed form and check to the address printed on the form:2HealthEquity. HSA Contribution Form

Make the check payable to HealthEquity. Writing your member ID and the tax year on the memo line helps the processing team match the payment to your account. Paper-clip the check to the form rather than stapling it, so document scanners don’t jam. If you’re mailing close to the April deadline, use a service with tracking so you have proof of the postmark.
HealthEquity also accepts forms by fax at 801-999-7829, the same Client Services line listed on the mistaken distribution form.3HealthEquity. Mistaken HSA Distribution Form If you fax the form, you still have to fund the contribution somehow: mail the check separately, or authorize a pull from a verified bank account on file.
Skipping the Paper Form
The member portal handles the same deposit without any PDF at all:4HealthEquity. HSA — Contributions and Transfers
- For a one-time contribution, open the My Account tab, choose Make Contribution from the HSA menu, pick your linked bank account, enter the amount, select the tax year, and confirm.
- For a recurring monthly contribution, open the HSA tab, click the Contribute icon, and follow the link to set up automatic contributions. Choose a bank account, a monthly amount, and a start date. The transfer repeats on the same day each month until you cancel.
A newly added bank account takes three to five business days to verify before HealthEquity can pull funds from it, so link the account well ahead of any deadline you’re working against. The online route removes mail time and the risk of a lost check.
When It Posts and How to Check
Paper contributions typically post three to five business days after HealthEquity receives and processes the form.2HealthEquity. Watch for the deposit under Transaction History or Activity in the portal. It shows up labeled as an “Individual Contribution.” If the posted amount doesn’t match what you sent, contact HealthEquity member services right away. Unresolved discrepancies turn into problems at tax time.
How Much You Can Put In
For 2026, the IRS caps total annual HSA contributions at $4,400 for self-only coverage and $8,750 for family coverage.6Internal Revenue Service. Rev. Proc. 2025-19 Those numbers cover everything going into the account combined: your own deposits, payroll deductions, and any employer contributions. If your employer is putting $1,500 into a family-coverage account, your own room for the year is $7,250. Account holders aged 55 or older can add another $1,000 as a catch-up contribution on top of the base limits.7Office of the Law Revision Counsel. 26 U.S. Code 223 – Health Savings Accounts
The tax-year box on the form matters because the IRS lets you make prior-year contributions up to the April filing deadline. If you’re catching up on last year’s limit in March, check the prior-year box. If you don’t specify, the deposit lands in whatever year the check is dated.2HealthEquity. HSA Contribution Form
Claiming the Deduction
Money you send in with the paper form or through the portal is after-tax, and you claim the deduction on IRS Form 8889. Line 2 captures the personal contributions you made during the year, and the deduction flows to Schedule 1 (Form 1040), line 13.8Internal Revenue Service. 2025 Instructions for Form 8889 It’s above-the-line, so you get it whether or not you itemize.
Don’t double-count payroll contributions. Those were already excluded from the wages reported on your W-2, and Form 8889 separates them from the after-tax amounts you’re actually deducting.
HealthEquity reports your total annual HSA activity to you and the IRS on Form 5498-SA, which arrives by late May or early June of the following year.9Internal Revenue Service. Form 5498-SA Cross-check that total against your own records. Catch mismatches before the IRS does.
Returning a Mistaken Distribution
If you took money out of the HSA for something that turned out not to qualify, for example an expense your insurer reimbursed later, you can return the funds using HealthEquity’s Mistaken HSA Distribution form. A redeposit is not a contribution and doesn’t count against your annual limit.3HealthEquity. Mistaken HSA Distribution Form
That form asks you to certify the original distribution was a mistake of fact, and it needs the amount and year of the withdrawal, your contact information, and the last four digits of your SSN or HealthEquity ID. Send it to the same Lexington PO box or by fax to 801-999-7829, along with a check to HealthEquity or authorization for an electronic pull from a bank account already on file. Write your member ID, the tax year, and “Redeposit” on the check memo line.10HealthEquity. Download HSA Tax Forms
HealthEquity states it is not required to accept mistaken distribution repayments, and any tax consequences from a mishandled redeposit stay with you. Submit the form as soon as you spot the mistake rather than waiting for tax season.
