Filling Out the Form

Use capital letters and black ink throughout.2Fidelity. Fidelity Check Writing Form You will need:

  • Your nine-digit Fidelity account number.
  • The full legal names of all registered owners, matching Fidelity’s records exactly.
  • A clean signature from each required signer in the signature section. Fidelity stores these as the master reference and uses automated scanning to compare every check you write against them, so keep your signature consistent.

If your legal name has changed, attach a completed Name Change form along with supporting documentation such as a marriage certificate or divorce decree.2Fidelity. Fidelity Check Writing Form Submitting a form with a new name but no name-change paperwork is a common reason for rejection.

IRA Withholding Election

The IRA version adds a section where you set your tax-withholding preference. Every check written from an IRA is a distribution and gets reported to the IRS on Form 1099-R for the year the check clears. If you make no election, Fidelity withholds 10% for federal income tax by default, plus any state tax that applies when federal tax is withheld.4Fidelity. Checkwriting – IRA Information and Terms You can elect zero withholding, but withholding increases the total distribution reported to the IRS and reduces the maximum check you can write against your balance.

Who Has to Sign

Signature requirements depend on how the account is registered, and getting this wrong is the fastest way to have the form bounced back.

Know Before You Write the First Check

The form is straightforward. What each check actually does depends on the account.

IRA Checks Are Distributions

A check written from a Traditional, Rollover, or SEP IRA is a distribution.4Fidelity. Checkwriting – IRA Information and Terms It lands on your 1099-R as taxable income for the year the check clears, even if you wrote it in December and it wasn’t cashed until January. If you kept the default 10% federal withholding, Fidelity adds that to the distribution, so writing a $1,000 check pulls more than $1,000 from the account.

Anyone under 59½ who writes a check from a Traditional, Rollover, or SEP IRA faces a 10% early-withdrawal penalty from the IRS on top of ordinary income taxes, unless an exception applies.7Fidelity. IRA Early Withdrawals That is why Fidelity restricts IRA check writing to account holders at least 59½.8Fidelity. Checkwriting Terms and Conditions Minimum and maximum check amounts may also apply to retirement and mutual fund accounts per the fund prospectus.1Fidelity. Spend and Save FAQs: Payment Services

HSA Checks Depend on What You Buy

Checks used to pay qualified medical expenses come out of an HSA tax-free and penalty-free. A check written for something that does not qualify as a medical expense before age 65 triggers income tax plus a 20% penalty on the withdrawal.9Fidelity. Health Savings Account (HSA) Spending Options After age 65 the 20% penalty disappears, though non-medical withdrawals still count as gross income. Keep receipts for every HSA check in case the IRS asks.

Margin Accounts Can Turn a Check Into a Loan

If your brokerage account has margin enabled, a check larger than your settled cash balance can create or increase a margin loan. Fidelity warns that check writing, debit cards, and bill-pay services on a margin account can reduce equity and raise the risk of a margin call.10Fidelity.
