How to Fill Out and Submit the Fidelity Address Change Form

The Fidelity address change form is a one-page PDF you download from Fidelity’s forms library, fill out in five short sections, sign, and mail to Fidelity’s processing center in Cincinnati (or Covington for overnight delivery). For most individual and joint brokerage accounts you don’t need the form at all: the fastest route is Fidelity.com/address, where the update posts almost immediately. Use the paper form when you’re updating a trust, an entity account, or anything the online tool won’t touch, or when you prefer a signed paper record. You can also start the process by phone at 800-343-3548.

When the Form Is the Right Tool

If you hold an individual or joint brokerage account, log in at Fidelity.com, open your profile settings, and choose the address change option. You can set a legal residence and a separate mailing address in the same place. The legal address is what Fidelity uses for tax reporting, so it has to be your actual home, not a mail drop.

The online tool has two gaps worth knowing about before you reach for it. It does not update employer-sponsored accounts such as a 401(k) or 403(b); those run through NetBenefits and often through your employer’s HR system first. It also will not update Fidelity life insurance contracts. For trusts, corporations, partnerships, and other entity accounts, the paper form is the standard path.

Downloading the Form

Open Fidelity.com, go to the forms library, and search “Address Change” in the All Forms section. The PDF is a single page with five numbered sections. Print it, or complete it on screen before signing.

Section 1: Account Owners

Enter your full name and Social Security number or Taxpayer ID. If the account has a joint owner, trustee, custodian, or other authorized individual, their name and SSN go on the second line. You must already be listed as an owner or authorized individual on the account. Fidelity will reject a form submitted by anyone who isn’t.

Section 2: Which Accounts to Update

You can apply the change to every account tied to your SSN or limit it to specific account numbers. If you want only some accounts updated, check the “ONLY on these accounts” box and list each account number. A separate checkbox lets you leave the address unchanged on any account you don’t list, which matters if you keep accounts at different residences. There is also a box to exclude Fidelity life insurance contracts from the update.

Sections 3 and 4: The New Addresses

Section 3 is for the primary owner’s new legal and mailing addresses. The legal address has to be a residential street address, since Fidelity uses it for tax documents such as Form 1099-DIV. The mailing address can be a PO Box, drop box, or care-of location; check “Same as legal/residential address” if the two match. Include a primary phone number.

Section 4 only applies when a joint owner or authorized individual has a different address from the primary owner. If everyone shares an address, leave it blank.

The form carries one state-specific caution: Louisiana residents should consult a legal or tax professional before changing an address on a joint account, because Louisiana’s community property rules can affect how a joint account change is handled.

Section 5: Signatures

One owner’s signature is enough for most accounts. The exception is mutual fund accounts whose account numbers begin with “2” followed by two letters; those require every owner’s signature. If your statements list accounts belonging to other owners, each of those owners must sign as well.

Where to Send the Completed Form

Mail the signed form to one of two addresses, depending on the service you use:

  • Regular mail: Fidelity Investments, PO Box 770001, Cincinnati, OH 45277-0002
  • Overnight mail: Fidelity Investments, 100 Crosby Parkway, KC1E, Covington, KY 41015

You can also upload the signed form through the Secure Message Center after logging in, or use “Send a Document” in the Fidelity mobile app. Registration-related changes typically take about three business days to process once Fidelity receives the paperwork.1Fidelity. The 10-Day Hold on Loans and Withdrawals

Once the address change posts, Fidelity freezes loan and withdrawal activity on the account for 10 calendar days. The freeze is a fraud control: if someone redirected your address without permission, the hold gives you time to catch it. Fidelity also sends a confirmation letter to both your old and new addresses, so an unauthorized change surfaces quickly.2Fidelity NetBenefits. Update Name, Address, and/or Date of Birth

If you need to pull funds out before the 10 days are up, you can waive the freeze by having a Medallion Signature Guarantee stamped on the form before you submit it. A Medallion Signature Guarantee is not a notary stamp. It’s a specialized authentication offered by banks, credit unions, and brokerages that confirms your identity and your authority over the securities in the account. Expect to visit a participating institution in person with government-issued photo ID and the form itself.3Fidelity Investments. Update Name, Address, and/or Date of Birth Not every bank offers the service to walk-in customers; some require you to have held an account there for a minimum period, and many will not guarantee transfers out of their own accounts.4Bank of America. Trust and Entity Accounts

Updating an address on a trust, corporation, or partnership account takes more documentation than a personal account. The signer has to be an authorized individual already on file: a trustee for a trust, an officer for a corporation, a managing member for a partnership. Fidelity requires the entity’s Employer Identification Number along with the signer’s personal ID.

If the signer isn’t already on record with Fidelity, a Medallion Signature Guarantee is usually required to verify authority. Trust accounts may need extra paperwork when trustees have changed since the account opened.5Fidelity. Change of Account Registration

Workplace Retirement Accounts

If the account is a 401(k), 403(b), 457, or similar employer plan, the standard address change form does not apply. The form itself directs you to call your plan sponsor or use NetBenefits.fidelity.com.6Fidelity. Address Change

Many employers control the address records on their sponsored plans, so updates often need to go through the HR portal or payroll system first and then feed to Fidelity on whatever schedule your employer uses. Some large employers push the file weekly; smaller ones may send it quarterly. If timing matters, call Fidelity’s workplace line to confirm whether your plan allows direct updates through NetBenefits.

The NetBenefits form carries the same 10-day distribution freeze and the same Medallion Signature Guarantee waiver option, and confirmation letters still go to both addresses.2Fidelity NetBenefits. Update Name, Address, and/or Date of Birth If you mail the NetBenefits form, the addresses differ slightly from the brokerage ones:

  • Regular mail: Fidelity Investments, PO Box 770002, Cincinnati, OH 45277-0090
  • Overnight mail: Fidelity Investments, 100 Crosby Parkway, KC1E, Covington, KY 41015

Moving to a Foreign Address

Changing your Fidelity address to a foreign country adds tax paperwork. The IRS requires nonresident aliens and foreign residents to file Form W-8BEN to certify foreign status, and that certification has to be renewed every three years. Fidelity has an online tool to complete and eSign the W-8BEN, with confirmation in one to two business days if you choose electronic delivery.7Fidelity Investments. How to Update Your Foreign Status

Before you move, contact Fidelity to ask about restrictions in your destination country. Some countries trigger limits on trading, transfers, or withdrawals, and Fidelity advises reaching out beforehand because sorting out access from overseas is harder.8Fidelity. 6 Tips for Retiring Overseas

Updating an Address After a Death or Incapacity

An executor, guardian, or conservator managing an account for someone who has died or lost capacity cannot use the standard address change form. Fidelity requires a certified copy of the court appointment naming the representative, dated within 180 days. For a deceased owner, you’ll also need a copy of the death certificate and, in some states, a state tax waiver. Where two or more executors share responsibility, a Co-Fiduciary Certification form is required.5Fidelity. Why the Legal Address Can’t Be a PO Box

Federal anti-money-laundering rules require every broker-dealer to collect a residential or business street address when opening an account and to keep it current. Under the Customer Identification Program rule, individuals must provide a street address rather than a PO Box, and entities such as corporations or trusts must provide a principal place of business or other physical location.9eCFR. 31 CFR 1023.220 – Customer Identification Programs for Brokers or Dealers in Securities That’s why the form separates a legal/residential address from a mailing address. You can route statements and correspondence to a PO Box through the mailing field, but the legal field has to carry a real street address.
