The Medicare EDI enrollment form is the packet you submit to your Medicare Administrative Contractor to start sending claims and receiving payment information electronically. You download it from your assigned MAC, complete the EDI Enrollment Agreement and EDI Application using identifiers that match your provider enrollment record exactly, and return it by the MAC’s specified fax, email, or mail channel. Government processing is free; any fee you pay goes to a clearinghouse or billing service if you use one.
What to Have Ready Before You Start
You cannot complete EDI enrollment until your Medicare provider enrollment is already approved. That approval produces your Provider Transaction Access Number (PTAN), the Medicare-only number your MAC assigns after processing your CMS-855.1Noridian Healthcare Solutions. You also need your National Provider Identifier from NPPES. Individual practitioners have a Type 1 NPI; organizations have a Type 2. An incorporated solo practitioner can hold both, one for themselves and one for the entity.2Centers for Medicare & Medicaid Services. Gather the rest before you sit down with the packet:
- Legal business name, spelled and punctuated the same way across your IRS records, NPPES, and PECOS.3First Coast Service Options. What Is The Provider’s Legal Business Name That Should Appear On CMS-855 Medicare Enrollment Application?
- Tax Identification Number (EIN or SSN, as registered with the IRS).
- PTAN from your Medicare enrollment approval letter.
- NPI from NPPES.4NPPES NPI Registry. NPI Registry
- If a third party will submit claims for you, the clearinghouse or billing service’s name, contact information, and existing Submitter ID.

Getting the Right Form From Your MAC
There is no single CMS form. Each MAC publishes its own packet, and you download it from the MAC that covers your state or jurisdiction. CMS maintains a directory at cms.gov/mac-info that lists every MAC and its assigned states with links to each contractor’s site.6Centers for Medicare & Medicaid Services. The layouts differ, but the core documents inside are the same.
What the Packet Contains
Two documents do the real work:
- The EDI Enrollment Agreement, a short form in which the provider agrees to the terms governing electronic data exchange with Medicare, including HIPAA transaction standards. An authorized official signs it.7Palmetto GBA. A/B EDI Enrollment Packet
- The EDI Application, which captures provider identifiers, contact information, transaction types, and submitter details. If a billing service or clearinghouse will submit for you, they complete the submitter section and you still sign as the provider.5Palmetto GBA. Part A/Part B/HHH EDI Application
Some MACs bundle extras. CGS Administrators, for instance, asks for the EDI Application together with the EDI Enrollment Agreement, plus a Provider Authorization Form or Software Order Form when those apply.8CGS Administrators, LLC. Filling Out the Application
Submitter ID
The Submitter ID identifies whoever actually uploads the electronic files. If you are a new applicant sending claims directly, leave the Submitter ID field blank; the MAC assigns one after approval. If a clearinghouse or billing service sends claims for you, the Submitter ID belongs to them, not you. Enter their existing Submitter ID on the application rather than requesting a new one.8CGS Administrators, LLC. Transaction Types
Select the electronic transactions you plan to exchange. Professional providers (physicians, nurse practitioners, therapists) use the 837P claim format, which corresponds to the paper CMS-1500.9Centers for Medicare & Medicaid Services. Group Practices
Only one EDI Enrollment Agreement is needed per group. But each individual provider whose claims will be submitted under that arrangement completes a separate EDI Application.5Palmetto GBA. Part A/Part B/HHH EDI Application That is where large groups get bogged down; someone has to complete an application for every billing provider.
Signature
The person who signs the Agreement must have legal authority to bind the organization, typically a physician-owner, practice administrator, or executive already listed in PECOS. If the signer does not match Medicare’s authorized officials on record, the packet comes back. Some MACs accept digital signatures through secure portals, but most still want an ink-signed form for initial enrollment.
Submitting the Packet and How Long It Takes
Submission channels vary by MAC and usually include fax, email, or mail. Novitas Solutions accepts submissions by fax at (877) 439-5479 or by mail to their EDI offices in Mechanicsburg, Pennsylvania, with separate PO boxes for Jurisdictions H and L.11Novitas Solutions. Processing timelines depend on the MAC. Palmetto GBA processes completed applications within 15 business days.5Palmetto GBA. Part A/Part B/HHH EDI Application CGS Administrators asks you to allow roughly 20 business days.8CGS Administrators, LLC. Medicare EDI Enrollment Packet First Coast Service Options says to wait at least 10 business days before calling for a status.12First Coast Service Options. Submission of EDI Enrollment Forms Incomplete packets are the most common cause of delay. Missing forms, blank required fields, and mismatched identifiers will all stop processing.
After Approval: Testing Before You Go Live
Once the MAC approves you, you receive a notification with your Submitter ID (if you requested a new one) and the technical details to connect to the MAC’s gateway. Most MACs then require a testing phase before you can send real claims. Testing confirms that your billing software produces 5010-compliant files and that the MAC’s system can read them.13Centers for Medicare & Medicaid Services. Medicare Fee-for-Service Companion Guides You send a small batch of test claims and verify the MAC accepts and processes them.
If you use a clearinghouse, the clearinghouse handles most of the technical testing because it already has an established connection to the MAC. Direct submitters carry more of that burden themselves. When testing passes, the MAC activates your connection for production claims.
Enrolling for Electronic Remittance Advice
Electronic Remittance Advice, the 835 transaction, is the digital version of the payment explanation Medicare sends after processing your claims. It shows what was paid, denied, or adjusted per line. Some MACs include ERA authorization in the initial EDI packet; others treat it as a separate step. Why Electronic Enrollment Is Effectively Required
The Administrative Simplification Compliance Act prohibits Medicare from paying initial claims that are not submitted electronically.15Centers for Medicare & Medicaid Services. Administrative Simplification Compliance Act Enforcement Reviews For physicians, suppliers, and other healthcare providers billing Medicare fee-for-service, that makes EDI enrollment a practical prerequisite for getting paid.
A few narrow waivers exist, granted by CMS in situations such as a HIPAA claim standard that does not accommodate a particular claim type or a disability affecting all staff members.16Centers for Medicare & Medicaid Services. Administrative Simplification Compliance Act Waiver Application Providers who believe they qualify must get pre-approval before sending paper claims. Otherwise, paper claims go unpaid.
Changing or Terminating Your EDI Enrollment
Either party can end the EDI arrangement with 30 days’ written notice.5Palmetto GBA. Part A/Part B/HHH EDI Application Providers commonly update their enrollment when they switch between direct submission and a clearinghouse, change billing services, or add a new provider to a group arrangement. In most cases you submit a new or amended EDI Application rather than modifying the original.
