Call Direct Express Before You File the Form
The phone call comes first. Dial 1-888-741-1115 and be ready to give your name, your card number, a description of the problem, the approximate date it happened, and the dollar amount.3Direct Express. If your card was lost or stolen, tell the representative on that same call so they can lock the card and send a replacement. You can also lock the card yourself in the mobile app or on the Direct Express website.

What to Put on the Transaction Dispute Form
The form asks for the information Direct Express needs to trace and investigate the charge. Work through it in order and answer every field that applies.
- Your name and full card account number as printed on the front of the card.
- The transaction amount, the date it happened, the date it posted, and a description (usually the merchant name shown on your statement).
- The reference number from your statement or online history, if one is listed.
- Whether you authorized the transaction. If someone else used the card, name that person if you know who they are and describe your relationship to them.
- Any other belongings lost or stolen along with the card.
Below those fields, the form leaves space for a written explanation. Use it to walk through the timeline: when you last had the card, when you noticed the charge, and why you believe it is unauthorized or wrong. Specific beats vague. “I did not make this purchase” is a start; “I was home in Ohio on March 12 and this charge posted from a gas station in Florida” gives the investigator something to work with.
Attach copies of anything that supports your account: a police report if the card was stolen, a receipt showing a different amount, emails with a merchant about goods that never arrived. Keep the originals.

Before you seal the envelope, photocopy or photograph every page you are sending. Write down the date you mailed it. Certified mail or a tracked service is worth the small cost because it proves when the form left your hands, and the date Direct Express receives your written notice is the date the investigation clock starts. Do not let the form sit on the counter for a week after the call.
What Happens After Direct Express Gets the Form

If it confirms the error in that window, it corrects your account within one business day and sends you the results within three business days of finishing the investigation.
If the bank needs more time, it can take up to 45 days, but only if it provisionally credits your account for the disputed amount within the first 10 business days. That provisional credit puts the money back in your account so you can use it during the investigation. The bank has to tell you the amount and date of the credit within two business days of issuing it.
The 45-day window stretches to 90 days for charges that started outside the United States, for point-of-sale debit card transactions, and for transfers that happened within 30 days of the first deposit to a new account. When the bank confirms your dispute, any provisional credit becomes permanent. When the bank finds no error, it sends you a written explanation and removes the provisional credit after notifying you. Report Fast to Limit What You Owe
For unauthorized transactions, how much you can be held responsible for depends on how quickly you report.5Consumer Financial Protection Bureau. 1005.6 Liability of Consumer for Unauthorized Transfers
- Report within 2 business days of learning your card was lost or stolen, and your liability caps at $50 or the total unauthorized charges, whichever is less.
- Report after 2 business days but within 60 days of the statement showing the charge, and your liability can reach $500.
- Wait more than 60 days after that statement, and every unauthorized transfer after the 60-day mark can fall on you, with no cap.
Direct Express cards also carry Mastercard Zero Liability protection for unauthorized use reported promptly.6Direct Express. A single extra day can move you into a higher liability bracket, so call the moment you notice something wrong.
If Direct Express Denies Your Dispute
Read the written explanation carefully and request the investigation documents if you have not already. A follow-up call with new information sometimes reopens the case, especially if the bank misidentified the transaction.
If that does not resolve it, file a complaint with the Consumer Financial Protection Bureau through its online portal.7Consumer Financial Protection Bureau. The CFPB forwards the complaint to the company, which generally responds within 15 days, though complex cases can take up to 60 days. You then have 60 days to review the response through the portal. A CFPB complaint creates a formal record and often reaches the company’s escalation team faster than a repeat call to customer service.
