How to Fill Out and Submit the Computershare Notice to Withdraw Form

The Computershare Notice to Withdraw form — formally the DRS Transaction Request Form — is the paper document you use to move shares out of a Computershare Direct Registration System account when the request can’t be handled electronically. For the most common reason people want to withdraw shares, moving them to a brokerage account, you usually don’t file this form at all. Your broker pulls the shares from Computershare through the DTC’s DRS Profile system, and Computershare processes it on the back end.1Computershare. Looking to Transfer Shares to a Brokerage? The paper form comes into play for physical certificates, estate transfers, and other situations the electronic route can’t accommodate.

Moving Shares to a Brokerage Account

If you want your Computershare shares in a brokerage account where you can trade them, call your broker and ask them to pull the shares from DRS. You don’t mail anything to Computershare. The transfer runs through the Depository Trust Company’s DRS Profile system, and your broker drives the request.2DTCC. Direct Registration System

Your broker will ask you for several things to submit the request:

  • Computershare’s DTC number, which is 7807
  • The company whose shares you hold
  • Your Holder Account Number, which begins with the letter “C” followed by 10 digits and appears on your holding statement3Computershare. Account Documents
  • Your name and address exactly as registered at Computershare
  • The SSN or TIN tied to the account
  • The number of shares you want moved

Registration on the two sides has to match. Same legal name, same tax ID. A mismatch, even on a middle initial, is the usual reason these transfers get rejected, so ask your broker what their matching rules are before they submit.1Computershare. Looking to Transfer Shares to a Brokerage?

Under industry DRS Profile guidelines Computershare is expected to process the request within 48 hours.4Securities Transfer Association. Profile Direct Registration Processing Guidelines Shares generally settle in your brokerage account within three to five business days, with another day or two possible on the broker’s side. When You Actually Need the Paper Form

The DRS Transaction Request Form is the right tool when the electronic broker-pull isn’t available or isn’t what you want. The typical situations are:

  • Requesting a physical stock certificate from the issuer
  • Transferring shares out of a deceased shareholder’s account
  • Any withdrawal that requires a medallion signature guarantee or signed paperwork rather than an electronic Profile instruction

You can download the form through Computershare’s Investor Centre portal in the documents or forms section, or request it by phone.

Filling Out the DRS Transaction Request Form

The form asks for the same core identifying information your broker would need, with the difference that here you’re providing it directly:6Computershare. Direct Registration (DRS) Transaction Request Form

  • Your full legal name, matching the account registration exactly. Small differences such as a middle initial versus a spelled-out middle name can cause a rejection.
  • Your Holder Account Number, the “C” followed by 10 digits from your most recent statement.3Computershare. Account Documents
  • The SSN or TIN on file.
  • The exact number of unrestricted shares you want withdrawn.
  • Transfer destination: if you’re moving to a broker, their DTC participant number and your account number there; if you want a certificate, indicate that instead.

Pull your most recent holding statement before you start the form. Every field needs to match what Computershare already has on record, and the statement is where the exact registration details, account number, and share balance live. If a submitted form is incomplete or the information doesn’t match, Computershare typically sends a notification through the Investor Centre message center explaining what to fix.

Medallion Signature Guarantee

Many paper withdrawal requests require a Medallion Signature Guarantee, a specialized stamp from a participating bank, credit union, or brokerage that verifies your identity and protects against unauthorized transfers.7U.S. Securities and Exchange Commission. Medallion Signature Guarantees – Preventing the Unauthorized Transfer of Securities Whether you need one depends on the type and value of the transaction.

Computershare offers a Medallion Waiver Option for smaller accounts. You may qualify if you’re a U.S. resident and the total account value for the stock in question is under $10,000. Using the waiver requires a non-refundable fee (typically around $50) and a copy of a government-issued photo ID submitted with your paperwork.8Computershare. What Is a Medallion Guarantee At $10,000 or more, the full medallion guarantee is generally required.

Each medallion stamp carries a coverage limit shown by an alpha prefix. If the transaction value exceeds the stamp’s limit, Computershare will reject it, so the stamp has to cover the full value of the shares being moved.8Computershare. What Is a Medallion Guarantee

To get the stamp, bring your Computershare holding statement and a valid government photo ID to a bank, credit union, or brokerage that participates in a Medallion program. Most institutions only stamp for their own customers, so plan on using one where you already have an account, and call ahead to confirm the branch handles your dollar amount. Not every branch does.

Mailing the Form

Once the form is complete and any required medallion stamp is on it, mail the original to Computershare at:

Some issuers use a dedicated Computershare address different from the general one. Check the instructions on your specific form or your holding statement for issuer-specific mailing details. Send it by certified mail or another trackable method. A medallion stamp is tied to that specific transaction, and replacing a lost form means starting the guarantee process over.

Requesting a Physical Stock Certificate

If you want paper certificates instead of book-entry shares, the DRS Transaction Request Form is also how you ask Computershare to issue them. Not every company still allows this. Many issuers have stopped offering physical certificates entirely and keep shares only in book-entry form on the register.5Computershare. Becoming a Registered Shareholder in US-Listed Companies Before you submit anything, check with Computershare, through the Investor Centre portal or the shareholder services line, whether the company whose shares you hold still permits certificates. If they do, Computershare charges an issuance fee, and higher DTC withdrawal fees may apply depending on the issuer.

Paper certificates can be lost, stolen, or destroyed, and replacing one is expensive. You’ll need to purchase an indemnity bond before the company issues a replacement, typically at a cost of two to three percent of the current market value of the missing certificates.10U.S. Securities and Exchange Commission. Withdrawals From a Deceased Shareholder’s Account

Transferring shares out of a deceased shareholder’s Computershare account requires more than the standard withdrawal form. The executor or administrator will need to gather:

  • A certified copy of the death certificate, not a photocopy
  • Letters testamentary or letters of administration from the probate court establishing legal authority over the estate; many transfer agents require these to be dated within 60 days of the transfer request
  • An affidavit of domicile, a notarized statement of the deceased shareholder’s state of legal residence
  • A stock power or assignment form authorizing the transfer, signed by the executor or administrator with a medallion signature guarantee

Requirements vary by issuer, so call Computershare’s shareholder services line about the specific company’s shares before you assemble the paperwork. Estate transfers take longer than standard withdrawals — plan on several weeks from submission to completion, and more if any document has to be corrected or reissued.

If someone is acting on a shareholder’s behalf under a power of attorney rather than estate documents, the POA must specifically grant authority to manage securities or transact with financial institutions. A general power of attorney that doesn’t mention investment accounts may be rejected.

After the Shares Arrive

When shares move from Computershare to a brokerage, the transfer agent is required to send a written transfer statement to the receiving broker within 15 days of settlement. That statement carries the security’s total adjusted basis, original acquisition date, holding period adjustments, CUSIP number, and share count — the information your broker needs to report an eventual sale to the IRS.11Internal Revenue Service. Instructions for Form 1099-B (2026)

Once the shares show up in your brokerage account, check that the cost basis came across correctly. Brokers sometimes display transferred shares with an “unknown” basis until they process the transfer statement, which can take a few weeks. If the basis still looks wrong after that, contact both your broker and Computershare; you may need to provide purchase confirmations or dividend reinvestment records to get it corrected.
