The Citibank IRA contribution form is a one-page document you complete to deposit money into an existing Citibank Traditional, Roth, SEP, or Rollover IRA. You can download it from Citibank’s online forms portal or pick one up at any branch. Once it’s signed, you mail it to Retirement Plan Services, PO Box 790012, St. Louis, MO 63179, or hand it to a banker in person.1Citibank. Citibank IRA Contribution Form
One thing to know up front: the form deposits money into an IRA you already have. If you don’t yet have a Citibank IRA, you also need to submit the corresponding Citibank IRA Application. The contribution form alone will not open an account.2Citibank. Citibank IRA Transfer and Direct Rollover Form
What to Have Ready Before You Start
Gather these before you sit down with the form:
- Your Social Security number. Citibank identifies the IRA by SSN rather than by a separate account number.1Citibank. Citibank IRA Contribution Form
- Your Citibank checking or savings account number, if you plan to fund the contribution by debiting an existing deposit account instead of attaching a check.
- For a rollover: the plan type the money came from (401(k), profit-sharing plan, another IRA, and so on), the original distribution amount, the rollover amount, and the dates funds were distributed and received.1Citibank. Citibank IRA Contribution Form
- If you’re moving assets from an outside custodian, a copy of your most recent statement from that institution, dated within 90 days.2Citibank. Citibank IRA Transfer and Direct Rollover Form
Filling Out the Five Sections
The form is divided into five numbered sections, each labeled “Must Complete.” Citibank also asks you to initial the bottom of every page.
Section 1: Plan Owner Information
Enter your full legal name, mailing address, Social Security number, and daytime and evening phone numbers. This is how Citibank links the deposit to your IRA and reports the contribution to the IRS on Form 5498.1Citibank. Citibank IRA Contribution Form
Section 2: Contribution Type and Amount
This section does most of the work. Start by writing in the tax year the contribution applies to. That entry determines whether Citibank reports the deposit as a current-year or prior-year contribution, so be deliberate. Then check the box for the plan you’re funding and enter the dollar amount:
- IRA (Traditional)
- SEP IRA
- Rollover IRA
- Roth IRA
If you’re rolling money over, a subsection asks for the plan type it came from, the original distribution amount, the amount you’re rolling over, and the relevant dates. For a recharacterization or conversion, you’ll need to attach a completed Citibank IRA and Roth IRA Withdrawal and Tax Withholding Election Form.1Citibank. Citibank IRA Contribution Form
Section 3: Contribution Method
You have two ways to fund the contribution:
- Check. Make it payable to “Citibank, N.A.” or “Citibank, N.A. as Successor Custodian” and attach it to the form.
- Debit from a Citibank account. Provide your Citibank checking or savings account number. You can set the debit up as a one-time transfer or as a recurring monthly debit by specifying the dollar amount, the day of each month, and the starting month.1Citibank. Citibank IRA Contribution Form
The recurring option is useful. If you want to spread a $7,500 annual contribution across twelve months, you can set that up on a single form rather than submitting one each time.
Section 4: Investment Options
Once the money reaches your IRA, Citibank needs to know where to place it. The form offers two choices:
- Certificate of Deposit. Specify the term and amount. The minimum CD deposit is $250.
- Insured Money Market Account. Provide the IMMA account number and the amount to deposit.1Citibank. Citibank IRA Contribution Form
Citibank IRAs are bank deposit products only. If you want IRA funds invested in stocks or mutual funds, this form is not the vehicle for that.
Section 5: Acceptance and Signature
Sign and date the form. Without a signature, Citibank will not process the contribution. If you submit at a branch, the banker fills in a separate “Branch Use Only” area with their name and representative ID.1Citibank. Citibank IRA Contribution Form
Contribution Limits and Which Tax Year to Write In
The dollar amount you enter has to fit within the annual IRS limit, which applies across all your IRAs combined rather than per account. For 2026:
- Under age 50: $7,500
- Age 50 or older: $8,600 (the $7,500 base plus an $1,100 catch-up)3Internal Revenue Service. 401(k) Limit Increases to $24,500 for 2026, IRA Limit Increases to $7,500
Your contribution also can’t exceed your taxable compensation for the year. If you earned $5,000, that’s your ceiling regardless of the IRS limit.4Internal Revenue Service. Retirement Topics – IRA Contribution Limits Roth IRA eligibility and Traditional IRA deductibility phase out at higher incomes, so if you’re near those thresholds, verify the current phase-out range on the IRS site before deciding which box to check.3Internal Revenue Service. 401(k) Limit Increases to $24,500 for 2026, IRA Limit Increases to $7,500
SEP IRA contributions follow a separate, higher cap. For 2026, an employer can contribute up to 25 percent of an employee’s compensation, capped at $72,000, and SEP contributions can be made up to the due date of the business’s income tax return, including extensions.5Internal Revenue Service. Retirement Plans FAQs Regarding SEPs
The tax year you write in Section 2 controls how Citibank reports the deposit on Form 5498. For Traditional and Roth contributions made between January 1 and the April 15 filing deadline, you can designate the money for either the current year or the prior year.6Internal Revenue Service. Form 5498 – IRA Contribution Information That’s a common way to top up last year’s IRA before you file. Citibank applies whatever year you write. Fixing a mistake later means contacting Retirement Plan Services and potentially filing corrective paperwork, so double-check before you sign.
Rollover Contributions
The same contribution form handles rollovers from a former employer’s 401(k), profit-sharing plan, ESOP, or another IRA. Check “Rollover IRA” in Section 2 and complete the additional fields: source plan type, original distribution amount, rollover amount, and the dates of distribution and receipt.1Citibank. Citibank IRA Contribution Form
Rollovers don’t count against your annual contribution limit, so a $50,000 rollover from a 401(k) won’t trigger an excess-contribution penalty. For an indirect rollover, where the old plan sends the check to you, you generally have 60 days from the date you receive the funds to deposit them into the IRA.
For a direct rollover, where your former plan sends the check straight to Citibank, expect a coordination step. The old plan administrator may require its own distribution form or an acceptance letter before releasing the funds. Contact them first to learn what they need, then submit Citibank’s contribution form and, if applicable, the IRA Transfer and Direct Rollover Form.2Citibank. How to Submit the Completed Form
You have two options once the form is signed:
- By mail: send the form (with a check attached, if that’s your funding method) to Retirement Plan Services, PO Box 790012, St. Louis, MO 63179.1Citibank. Citibank IRA Contribution Form
- At a branch: hand it to a Citibank representative, who can verify your signature and confirm the form is complete before it leaves the counter.
If you’re mailing near the April 15 deadline for a prior-year contribution, use certified mail or another trackable method. A contribution that arrives after the deadline will be applied to the current tax year instead, which may not be what you wanted.
Citibank confirms the deposit through your monthly IRA statement or a notification in the online message center, and you can also check the balance in the Citibank mobile app. Debits from an existing Citibank checking or savings account tend to post faster than mailed checks, which have to clear.
If You Contribute Too Much
Contribution mistakes are easy if you have IRAs at more than one institution. The IRS charges a 6 percent excise tax on any excess amount for every year it stays in the account. To avoid the penalty, withdraw the excess and any earnings it generated by the due date of your individual income tax return, including extensions.4Internal Revenue Service. Retirement Topics – IRA Contribution Limits
The contribution form itself doesn’t process an excess-contribution withdrawal. For that, you’ll need the Citibank IRA and Roth IRA Withdrawal and Tax Withholding Election Form.
