When You Need to File
You file this form when two things are true: you saw a vision provider outside the Cigna Vision network, and your plan includes out-of-network benefits. Covered services include eye exams, prescription lenses, frames, and contact lenses bought from non-participating providers or retailers.2Cigna. Submitting the form doesn’t guarantee you’ll be paid, and out-of-network allowances are often well below what you actually spent. Check your benefits summary before you file so the reimbursement amount isn’t a shock.
Pick the Right Version
Cigna uses more than one vision claim form, and the correct one depends on how your plan is administered. Many Cigna vision plans are serviced by EyeMed; those members use a form with pre-printed service codes and a field for the provider’s NPI number.2Cigna. Filling Out the Form
Both versions follow the same general structure: your information, then the provider’s, then the services and charges, then your signature. A missing subscriber ID or an unsigned form is the fastest way to have the claim bounced back, so fill every required field.
Patient and Subscriber Information
Enter the patient’s full name, date of birth, and address. If the patient is a dependent such as a spouse or child, fill in the subscriber’s information separately and note the relationship. Include the subscriber’s customer ID number from your Cigna vision ID card.1Cigna. Provider Information
This section identifies where you received services. The EyeMed-administered form asks for the provider’s name, street address, and National Provider Identifier (NPI).2Cigna. Cigna Vision Claim Form If you bought glasses from an optical shop rather than a doctor’s office, list that retailer. Your receipt should have everything you need.
Services and Charges
The EyeMed version has pre-printed service lines, so you only need to enter the amount you paid next to each applicable one: eye exam, refraction, frame, lenses (check single vision, bifocal, trifocal, or progressive), lens options such as anti-reflective coating or polycarbonate, and contact lenses with their fitting if you bought contacts instead of glasses.2Cigna. On the other version, you may need to write in the service descriptions and codes yourself. Your provider’s itemized receipt should list them.
Signature
Sign and date the form. Your signature authorizes release of the medical information needed to process the claim and acknowledges the fraud warnings printed on the back.1Cigna. Cigna Vision Claim Form An unsigned form will not be processed.
Receipts and Other Documents
Attach original itemized receipts showing every service and material separately. A single-line total won’t work; the receipt needs to break out each charge, including the lens type if you bought eyeglasses.1Cigna. If you carry vision coverage through another insurer in addition to Cigna, include the Explanation of Benefits from that primary carrier.1Cigna. Cigna Vision Claim Form Keep copies of everything you send in case the claim is lost or Cigna asks you to resubmit.
Where to Mail It
Send the completed form and receipts to the address printed on your version of the form. The two currently in use are:

Most vision claims go by mail or fax as directed on the form rather than through an online portal. If your version lists a fax number, that’s a faster option, but mail is the standard route.
How Long You Have to File
Your plan documents set the deadline for submitting an out-of-network vision claim after the date of service. Some plans allow 12 months; others may differ. The claim form itself does not state a universal deadline, so check your benefits summary or certificate of coverage, or call 1-800-244-6224 if you’re unsure.6Cigna Healthcare. When Cigna is your secondary insurer, the filing clock may start from the processing date on the primary carrier’s EOB rather than the date of service.7Cigna Healthcare. Submit and Pay Claims for Providers
After You Submit
Cigna processes out-of-network vision claims within 10 business days of receiving a clean claim with an itemized receipt.8Cigna. Cigna Vision PPO A clean claim means every required field is filled in, the form is signed, the receipt is itemized, and nothing is missing. Incomplete submissions get kicked back, and the clock restarts when you resubmit.
Once processed, Cigna sends an Explanation of Benefits showing how much of each charge was applied to your allowance and how much is being reimbursed. Reimbursement typically arrives as a check mailed to the address on file. You can track the status in myCigna or by phone.
If Your Claim Is Denied
The EOB will include a reason code for any denial. Common triggers: missing or illegible information, a receipt that isn’t itemized, services not covered under your plan, or a late filing.
You generally have 180 days from the date of the denial notice to appeal.9Cigna. Customer Appeal Request Call the number on your EOB first. A missing document can often be fixed by resubmitting rather than appealing. If that doesn’t resolve it, submit a written appeal with the original EOB and any documentation that explains why the claim should be paid.
When the Standard Form Isn’t the Right One
If your plan has no out-of-network benefits, or you want an out-of-network visit processed at in-network benefit levels, Cigna uses a separate document called Claim Form 2: Exception Request. It applies only in narrow situations: no participating provider within a reasonable distance of your home or workplace, no in-network appointment available within 30 calendar days (15 days in Connecticut), or a state-declared emergency that prevented access to in-network providers.10Cigna. The exception form does not cover situations where you simply preferred an out-of-network provider or couldn’t fit an in-network appointment into your personal schedule. If you don’t qualify for an exception and your plan has no out-of-network benefits, Cigna won’t reimburse the visit regardless of which form you submit.10Cigna. Cigna Vision Exception Claim Form
