How to Fill Out and Submit the Assurance Wireless Application Form

To apply for Assurance Wireless, confirm you qualify for the federal Lifeline program either through your income or through a qualifying assistance program, gather proof of identity and eligibility, then submit an application online at assurancewireless.com or by mailing a paper form. Approved households get unlimited talk, unlimited texting, and 10 GB of high-speed data each month on the T-Mobile network at no cost.¹ Most applicants finish the online form in about ten minutes. The slower part is pulling documents together.

Confirm You Qualify

Lifeline has two eligibility paths. You qualify by income if your household’s gross annual income is at or below 135% of the Federal Poverty Guidelines for your household size.² You qualify by program if you or anyone in your household participates in one of these:³

SNAP (Supplemental Nutrition Assistance Program)
Medicaid
Supplemental Security Income (SSI)
Federal Public Housing Assistance, including Section 8 vouchers and public housing
Veterans Pension and Survivors Benefit
Certain Tribal assistance programs

Only one Lifeline benefit is allowed per household, not per person. If anyone at your address already receives a Lifeline discount on phone or internet from any provider, a second application from the same address will be flagged and denied.³ A “household” is people at the same address who share income and expenses, so unrelated roommates who keep their finances separate can each apply.

2026 Income Limits

For the 48 contiguous states and Washington, D.C., these are the maximum gross annual incomes for the income-based path:⁴

8 people: $75,222

Limits are higher in Alaska and Hawaii. For each person beyond eight, add $7,668.

Gather Your Documents First

Applications stall most often on paperwork. The National Verifier, the federal system that decides every Lifeline application, will try to confirm your eligibility automatically by checking government databases. When it can’t, you have to upload or mail proof.⁵ Have these ready before you start.

Proof of Identity

You need one document showing your full legal name and date of birth. Any of the following works:⁶

A current, unexpired driver’s license
An unexpired U.S. passport
An unexpired U.S. government, military, state, or Tribal ID
A U.S. birth certificate
A Certificate of Naturalization, Certificate of U.S. Citizenship, or Permanent Resident Card

If you’re asked to verify the last four digits of your Social Security number, a Social Security card, an SSA-1099 benefit statement, a W-2 from the last two years, or a prior year’s tax return will satisfy it.⁶ Tribal members applying without a Social Security number can use a Tribal ID card, a letter from their tribe’s enrollment office, or a Certificate of Degree of Indian Blood.

Proof of Eligibility

If you’re applying through a qualifying program, you need a document from that program showing your name, the program’s name, the issuing agency, and a date within the last twelve months. A current benefit statement, award letter, or notice of participation all qualify.⁷ An award letter from two years ago will be rejected.

If you’re applying by income, bring your prior year’s federal, state, or Tribal tax return, or income documentation covering three consecutive months within the past year, such as pay stubs. Social Security benefit statements, VA benefit statements, unemployment or workers’ compensation statements, and child support awards are also accepted.⁷ Two months of pay stubs isn’t enough. It has to be three consecutive months.

Submit Your Application

Online

The online application at assurancewireless.com is the faster route. It feeds directly into the National Verifier, so your eligibility check begins immediately, and you upload photos or scans of your documents as you go.⁸ If the National Verifier can confirm your eligibility through its automated database connections with CMS, HUD, the VA, and state agencies, you may not need to upload anything at all.⁵

The form asks for your full legal name (matching your ID), your physical residential address (P.O. boxes and general delivery addresses aren’t accepted), your date of birth, the last four digits of your Social Security or Tribal ID number, and either your qualifying program or an income declaration.⁹ The last section is a certification that everything on the form is true and that you understand the one-per-household rule. Applications without a signature are automatically rejected.

By Mail

You can request a paper application by calling Assurance Wireless customer service. Fill it out, attach photocopies of your supporting documents, and mail the package to the address on the form. Confirm the current mailing address on the Assurance Wireless contact page before sending, since it has changed in the past.¹⁰ Use a tracked mailing method; you’re sending personal information, and a lost envelope means starting over.

After You Submit

The National Verifier runs your information against federal and state databases. USAC connects with CMS for Medicaid, HUD for housing assistance, the VA for veterans benefits, and databases in more than 25 states for SNAP and SSI.¹¹ Automated matches process fast. Uploaded documents go to manual review, which takes longer.

You can track your application at any time through the Assurance Wireless site.¹² If you’re approved, Assurance Wireless ships a SIM card kit or device to your residential address. If more documentation is needed, the notice will spell out exactly what’s missing.

Why Applications Get Denied

A rejection is almost always one of these:

Someone at your address already receives Lifeline. They need to de-enroll before you can apply.
The address you entered doesn’t match official records. A missing apartment number, a typo, or a new building not yet in the postal database can trigger this.
Your proof of program participation is older than twelve months.
Your name, date of birth, or Social Security digits don’t match federal records. Even a hyphenated last name on one document but not another can cause a mismatch.
Your income is over the limit. For 2026, a single-person household is capped at $21,546 in gross annual income.⁴

Denials aren’t permanent. Fix the issue and reapply.

Activate Within 30 Days

Once you’re approved, you have 30 days to use your service — make a call, send a text, or use data.¹³ Miss that window and your enrollment can be canceled. If you’re using your own phone, the SIM card kit comes with instructions for swapping it in and activating online.

Keep Your Benefit Active

Two rules can cost you service after you’re enrolled.

Every year, USAC tries to re-verify your eligibility automatically. If it can confirm you still qualify, you don’t have to do anything. If it can’t, you’ll get a notice, and you have 60 days to respond and prove you’re still eligible online at getinternet.gov, by mail, or by phone.¹⁴ Miss the 60 days and you lose the benefit and have to reapply from scratch.

The second rule is usage. If you go 30 consecutive days without a call, text, or data session, your carrier must send a 15-day warning. If you still don’t use the service in those 15 days, your Lifeline benefit is terminated.¹⁵ A single text resets the clock. If you’re not a heavy user, set a monthly reminder.

If You Live on Tribal Lands

Residents of qualifying Tribal lands get a larger monthly discount, up to $34.25 instead of the standard $9.25.¹⁶ In addition to the standard qualifying programs, Tribal residents can qualify through:¹⁷

Tribal Temporary Assistance for Needy Families (Tribal TANF)
Head Start, for households meeting the income qualifying standard
Food Distribution Program on Indian Reservations

The application process is the same. Apply online or by mail through Assurance Wireless, and the National Verifier handles eligibility.
