Reconsideration Comes Before the Appeal

The Filing Deadline

The Claim Payment Appeal must be submitted within 60 calendar days of the date the reconsideration determination letter was mailed.1Amerigroup. Getting the Right Form

Completing the Form

Member and Provider Fields

The top of the form asks for the member’s first and last name, Member ID, and date of birth. Pull these from the original claim or the member’s insurance card and check each character against that source. The provider block takes your legal name, street address, city, state, ZIP code, and your ten-digit National Provider Identifier. You will also mark whether you are participating or non-participating. If a billing company or outside representative is filing the appeal for you, a separate block captures their contact name, phone, email, and mailing address.

Claim Details

Enter the claim number from the Explanation of Payment, the billed amount, the amount received, the start and end dates of service, the authorization number if one was issued, and the reconsideration reference number from the first-level decision.1Amerigroup. Reason for the Appeal

The form includes a checklist. Pick the category that matches the determination on the Explanation of Payment, because the selection shapes how the reviewer approaches the file:

  • Untimely filing, when the claim was denied as late but you have proof it was timely.
  • No authorization, when the denial cites missing prior authorization but authorization was in fact obtained.
  • Other health insurance denial, when Amerigroup believes the member has other coverage and the member does not.
  • Experimental or investigational denial, when you have evidence that the service is medically accepted.
  • Claim code editing denial, when automated bundling, downcoding, or edits were misapplied.
  • Contract payment disagreement, when the paid amount does not match the contracted fee schedule.
  • Duplicate denial, when the services were distinct and only flagged as duplicates.
  • Member retro-eligibility issues, when coverage changed retroactively after the service date.

Documents to Attach

The form tells the reviewer what you are disputing. The attachments are what actually change the outcome. Every appeal should include a copy of the Explanation of Payment showing the original processing decision and the claim adjustment reason codes. Without it, the reviewer has to pull the record internally, which slows the file down.

For medical necessity denials, include the clinical records that support the services billed: physician notes, diagnostic results, and the treatment plan, showing why the service was appropriate for the patient’s condition on that date. A letter from the treating physician explaining the clinical rationale strengthens the case.

For coding disputes involving bundling, modifier issues, or procedure-to-diagnosis mismatches, the record must demonstrate that the services were distinct and separately identifiable. If modifier 59 was used to bypass a National Correct Coding Initiative edit, for instance, the operative report or clinical notes should show a different site, a different session, or a separate procedure.4Amerigroup District of Columbia. For denials based on missing prior authorization where one was in fact obtained, attach the authorization number and the approval letter or a screen capture from the authorization portal. If the authorization was retrospective, document the dates it was requested and approved.

Label every attachment with the matching claim number and date of service. Reviewers handle high volumes, and clearly organized packets move faster.

Through Availity

The Availity portal at availity.com handles electronic submission of claim disputes. Your Availity administrator must assign the Claim Status role to your user account first. From the claims management section, locate the finalized claim, select the dispute option, pick a request reason from the drop-down, enter your supporting rationale, and upload digitized copies of the supporting documents. The system returns a case number and confirmation message once you submit; save both.3Amerigroup. What Happens After You Submit

Amerigroup resolves Claim Payment Appeals within 30 calendar days of receipt and sends the decision to the provider in writing.2Amerigroup. If a payment adjustment is approved, the funds are typically included in the next regular payment cycle. You can track the file through Availity or confirm it is active by calling provider services.

If the Appeal Is Denied

A denied first-level Claim Payment Appeal can go to a second-level appeal, which must be submitted in writing within 30 calendar days of the date on the first-level determination letter.2Amerigroup. Errors That Get Appeals Rejected at Intake

  • Filing the appeal form without a completed reconsideration, which causes the submission to be returned or reclassified.
  • Missing the 60-day window from the reconsideration determination letter.
  • A wrong Member ID or claim number that prevents the appeals department from locating the claim.
  • Submitting the form with no supporting documents, so the reviewer has the complaint but none of the proof.
  • Illegible faxes or uploads without claim numbers on each page, which leads to evidence being overlooked.
  • Using a state version of the form that does not match the plan the claim was billed under.
