The Ameren medical hardship form, officially called the Medical Certification Form, is what stops Ameren Illinois from shutting off your electricity or gas when someone in your home has a medical condition that would worsen without service. You download it from Ameren’s website or request one by calling 800.755.5000, your doctor completes the clinical portion, and you fax or email it to Ameren’s credit department. Once accepted, it blocks disconnection for 60 days.1Ameren. Ameren Illinois Medical Certification Form
Who the Form Protects
The patient does not have to be the account holder. Anyone living at the service address qualifies as long as a healthcare provider certifies that losing electric or gas service would aggravate an existing medical emergency or create a new one.1Ameren. Ameren Illinois Medical Certification Form Common qualifying situations include dependence on electrically powered equipment like ventilators or oxygen concentrators, chronic respiratory or cardiovascular conditions sensitive to temperature extremes, and recovery from surgery or acute illness where climate control is medically necessary.
Illinois Commerce Commission rules do not list specific diagnoses. The regulation only requires the certificate to state that disconnection would aggravate or create a medical emergency, leaving the clinical judgment to the provider.2Legal Information Institute. Ill. Admin. Code tit. 83, 280.160 – Medical Certification
Who Can Sign the Certification
Ameren’s own form accepts four provider types: a physician, a registered nurse, a nurse practitioner, or a physician assistant.1Ameren. Ameren Illinois Medical Certification Form The Illinois Administrative Code is narrower, technically limiting certifying parties to a licensed physician or a local board of health.3Illinois General Assembly. Section 280.160 Medical Certification In practice Ameren will accept any of the four, but if there’s any chance of a dispute, a physician’s signature carries the strongest protection.
Filling Out the Form
The form is a single page. The top half is yours; the bottom half is your provider’s.
Your Section
Enter your Ameren Illinois account number exactly as it appears on the upper right corner of your monthly bill. Add the name or names on the account and the patient’s name. When the patient is someone other than the account holder, both names go on the form, each in its own labeled field.1Ameren. Ameren Illinois Medical Certification Form
Account-number mismatches are a common reason forms get kicked back, so check it twice against your statement before handing anything to your provider.
Provider’s Section
Your provider fills in their printed name, provider type, office address, phone number, and signature with a current date. The core statement they certify is that loss of electric or gas service will aggravate an existing medical emergency or create one for the patient.1Ameren. Ameren Illinois Medical Certification Form A detailed diagnosis is not required on the form itself. An undated or stale signature will slow things down, so make sure the date is recent.
How to Submit It
Ameren takes the completed form two ways:1Ameren. Ameren Illinois Medical Certification Form
- Fax to 217.424.6496, the dedicated line for medical certifications.
- Email to amerenilcredit@ameren.com.
A letter on the provider’s official letterhead can substitute for the form, as long as it includes the patient’s name, service address, account number, provider credentials, and the medical-necessity statement.4Ameren. Health, Safety and Financial Assistance Options This matters when a provider’s office cannot easily fill out a PDF.
Keep proof of what you sent. Save the fax confirmation or the sent email. If a disconnection notice arrives while the certificate is being processed, that proof is what shows Ameren already has the paperwork. Calling 800.755.5000 after you submit is worth the time: the representative can note your account so collections pauses while the medical department reviews.
What You Get Once It’s Approved
An accepted certificate blocks disconnection for 60 days from the date of certification.2Legal Information Institute. Ill. Admin. Code tit. 83, 280.160 – Medical Certification If service was already shut off when you filed, the 60-day clock does not start until Ameren restores it, so you don’t lose protection waiting on reconnection.3Illinois General Assembly. Section 280.160 Medical Certification
The certificate does not forgive what you owe. You continue to be billed for usage, and after the 60 days expire Ameren can resume standard collections unless the balance is resolved or a new certificate is in place.
Renewing After 60 Days
You can’t file one certificate after another to keep the protection running indefinitely. A new certification on the same account is only allowed after one of two things happens:3Illinois General Assembly. Section 280.160 Medical Certification
- You pay the account balance in full. Once current, you can file a new certificate right away.
- Twelve months pass from the start date of the prior certification, if the balance isn’t paid.
Treat the 60 days as a working window. Use it to apply for assistance, set up a payment plan, or both.
Using the 60 Days to Clear the Balance
Several programs can take pressure off the bill before protection ends:
- LIHEAP, the federal Low Income Home Energy Assistance Program, helps pay utility bills for income-eligible households. Illinois applications go through local Community Action Agencies.5LIHEAP Clearinghouse. LIHEAP Income Eligibility for States and Territories
- The Critical Medical Need Program, run by Ameren with the United Way and Spire, specifically targets customers whose loss of utility service could prevent the use of life-support equipment.4Ameren. Health, Safety and Financial Assistance Options
- Deferred payment agreements let you spread the past-due amount over several months while staying current on new charges. Ask about one when you call to confirm the certificate was received.
Starting these applications in the first week or two gives processing time to work in your favor. Waiting until day 55 rarely does.
Winter Months: Two Protections at Once
From December 1 through March 31, Illinois separately bars Ameren from disconnecting residential heat customers when temperatures are forecast at 32°F or below, and requires the utility to offer a deferred payment agreement with a down payment capped at 10 percent before disconnecting during that stretch.6Illinois Commerce Commission. Illinois Winter Disconnection Rule in Effect Through March Filing the medical certificate during winter is still worthwhile. The winter rule has exceptions, including refusal to enter a payment agreement, that the medical certificate does not share.
Ameren Missouri Customers
The form and the 60-day shield are Ameren Illinois. In Missouri, the Public Service Commission allows a utility to postpone disconnection for up to 21 days when it would aggravate an existing medical emergency, and the customer may be asked to provide reasonable evidence of the medical necessity.7Missouri Secretary of State. 20 CSR 4240-13 – Cold Weather Maintenance of Service Registration as a disabled or elderly customer must be renewed annually by October 1. Missouri customers should call Ameren Missouri’s customer service line to request the correct form for their territory, since the PDF above is an Illinois document.
