How to Fill Out and Submit the Allianz Withdrawal Request Form (S2067)

The Allianz withdrawal request form is the document you use to take money out of an Allianz annuity or life insurance contract. You can submit it online, by fax, or by mail to Allianz Life’s Minneapolis office, and the company processes it within 10 business days of receiving a complete form.1Allianz Life. Frequently Asked Questions Direct deposit puts the money in your account faster than waiting on a paper check, so most of the decisions on the form come down to picking the right withdrawal type, electing a tax withholding rate, and making sure every signature and supporting document is there.

What to Have Ready Before You Open the Form

Pull a few things together first so you can finish the form in one sitting:

  • Your contract or policy number, from the original contract, an annual statement, or your Allianz online account.
  • The primary owner’s Social Security number or Taxpayer Identification Number.
  • The current mailing address and phone number on file. If they’ve changed, update your record with Allianz first.
  • Bank routing and account numbers if you want direct deposit, plus a voided check. If Allianz doesn’t already have a voided check on file and you don’t include one, the company will mail a paper check regardless of what you selected. The bank must be an ACH member and the account owner’s name must match the contract owner’s.2Allianz Life Insurance Company of North America. Withdrawal Request Form

Decide ahead of time whether this is a one-time withdrawal, a full surrender of the contract, or something else. If your annuity is a qualified contract (IRA or employer plan) and you’re taking a required minimum distribution, Allianz has a separate form for that; see the RMD section below.

Getting the Right Version of the Form

Log in at allianzlife.com and go to Personal Accounts to find the current form, or to initiate a withdrawal directly through the portal if you own a fixed annuity.1Allianz Life. Frequently Asked Questions Fixed annuity owners can complete the whole request online without ever downloading a PDF.

If you want the paper form, Allianz posts fillable PDFs on its website. The version for contracts issued by Allianz Life Insurance Company of North America is different from the one for contracts issued by Allianz Life Insurance Company of New York. Grab the one that matches your contract; your contract or statement will tell you which company issued it.

Choosing a Withdrawal Type

After the identifying information at the top, the form asks what kind of withdrawal you want. The main choices:

  • Free withdrawal. The amount you can take in a contract year without a surrender charge. On many Allianz annuities this is up to 10% of the contract value in the contract year after your last premium payment, and unused room can carry forward. You can request the maximum free amount or a specific dollar figure within the limit.3Allianz Life. Allianz Accumulation Advantage+ Annuity
  • Partial surrender. A withdrawal that may exceed the free allowance and can trigger a surrender charge. Enter the net dollar amount you want to receive; the form grosses up the withdrawal to cover any tax withholding you elect, so the deposit matches the number you wrote.2Allianz Life Insurance Company of North America. • Full surrender. Liquidates the entire contract. The contract is closed permanently.

Surrender-charge percentages and the length of the surrender period vary by product. Check your contract or latest annual statement for the schedule that applies to you, and note the year the surrender period ends; once it ends, you can withdraw any amount without a charge. If you’re drawing lifetime income through a benefit rider, a partial surrender can reduce your future income payments.

Payment Method and the Voided-Check Trap

Mark either ACH direct deposit or paper check. ACH is free and arrives in your account within a few business days after Allianz releases the payment. Paper checks go to the address on file and depend on mail time from Minneapolis.

The common mistake: selecting ACH without providing a voided check when Allianz doesn’t already have one on file. The company defaults to mailing a check in that case, regardless of what you marked.2Allianz Life Insurance Company of North America. Withdrawal Request Form

Federal and State Tax Withholding

The form asks you to elect a federal withholding percentage. Leave it blank and the payer withholds 10% of the taxable amount by default on nonperiodic distributions.4Office of the Law Revision Counsel. 26 U.S. Code 3405 – Special Rules for Pensions, Annuities, and Certain Other Deferred Income You can elect any rate from 0% to 100%, which is what IRS Form W-4R exists to document.5Internal Revenue Service. 2026 Form W-4R

How much of the distribution is actually taxable depends on whether the annuity is a qualified contract inside a retirement plan or a non-qualified contract, so settle that before picking a rate. Some states require mandatory withholding on annuity distributions and the form includes a state section; if your state mandates withholding, you can’t opt out of it on the state side even if you elect zero federal withholding.

Signatures

Every owner on the contract has to sign and date the form. If there are joint owners, both signatures are required, and a missing one is among the most common reasons the form comes back.

For contracts held inside an employer-sponsored retirement plan governed by ERISA, a married participant’s spouse generally must consent in writing to a distribution that waives survivor benefits, and the consent must be witnessed by a notary public or plan representative.6Internal Revenue Service. What Happens After Allianz Receives the Form

Processing takes up to 10 business days after Allianz receives a complete form.1Allianz Life. Frequently Asked Questions “Good order” means every required field is filled in, all signatures are present, and any supporting documents (voided check, spousal consent) are attached. The contract value used for your withdrawal is set at the end of the business day Allianz receives the completed request.8Allianz Life Insurance Company of North America. Withdrawal Request Form For indexed annuities, that timing matters because the contract value moves.

If something is missing, Allianz will contact you, and the clock resets when the correction comes back. The most frequent reasons forms get kicked back: a missing owner signature, no voided check with ACH selected, and an outdated or wrong contract number. You can check status by logging in to your online account or calling customer service.

Taxes You’ll See Later

Allianz reports distributions of $10 or more on Form 1099-R and sends your copy by January 31 of the following year.9Internal Revenue Service. Keep it with your tax records.

If you’re under age 59½ when you take money from a non-qualified annuity, the IRS adds a 10% penalty on the taxable portion of the distribution; the same 10% penalty applies to early distributions from qualified retirement plans under a separate provision of the statute.10Office of the Law Revision Counsel. 26 U.S. Code 72 – Annuities; Certain Proceeds of Endowment and Life Insurance Contracts Exceptions exist, including distributions after the owner’s death, distributions due to disability, and substantially equal periodic payments. The penalty is on top of ordinary income tax and separate from any surrender charge Allianz applies, so a premature withdrawal can be taxed, penalized, and charged all at once.

If You’re Taking a Required Minimum Distribution, Use a Different Form

Qualified annuity owners who have reached their RMD age shouldn’t use the standard withdrawal request form for an RMD. RMDs begin at age 73 for people born between 1951 and 1959 and at age 75 for those born in 1960 or later. Allianz has a dedicated Required Minimum Distribution Election Form (SFAL0021) that lets you set the payment frequency (monthly, quarterly, semi-annually, or annually) and indicate whether you’re satisfying a deferred first-year RMD or making up a missed one.11Allianz Life Insurance Company of North America.
